Chronic and Terminal illnesses in Children are on the rise in Sub Saharan Africa. Kenya has lagged in implementation of Home Based Care to mitigate effects of CI/TI [3]. According to WHO, 2017 and Ministry of Health-Kenya 2013, millions of Children are affected by these Illnesses such as Tuberculosis, Asthma, Congenital abnormalities, HIV/AIDS and Cancer among others [4,5]. Background: Anti-retroviral Therapy (ART) is a major intervention for children infected and expos... more Background: Anti-retroviral Therapy (ART) is a major intervention for children infected and exposed to HIV infection and it influences their morbidity and mortality. Objective: To investigate factors influencing ARV uptake in HIV positive and exposed children aged below 14 years in Meru North District. Design: A descriptive cross-sectional study. Setting: The Nyambene District Hospital, Miathene Sub-District Hospital and Maua Methodist Hospital paediatrics comprehensive care clinics (PCCC) in Meru North District. Subjects: Parents or guardians with children less than 14 years of age exposed or infected with HIV who visited the selected health facilities. Results: About 222 caregivers were interviewed with mean age of 38.4 + 10.3 years ranging from 21 to 70 years. About 120 (54.0%) were aged between 30 to 39 years, Most of the caregivers 106 (47.7%) had acquired education up to primary level. Marital status of the caregivers revealed that 104 (46.8%) were married while 16 (6.3%) were cohabiting. Relationship between children ARV uptake and marital status of caregiver was significant (OR= 2.1, 95% C.I= 1.0-4.6, P=0.050). Significant association between source of medical advice and children ARV uptake (P<0.05) was evident. Conclusion: Achieving widespread public health benefits of ARV roll-out requires community-level interventions in marriages and information distribution to ensure local acceptability of antiretroviral drugs. Better management and improvement of health status of HIV exposed and infected children will be enhanced by improved ARV uptake. Recommendation: The study recommends integration and decentralization of ARV services through social supports, encouraging disclosure of HIV statuses for EAST AFRICAN MEDICAL JOURNAL January 2018 positive living and enhanced ARV uptake and enhanced awareness creation on ARVs and HIV information to caregivers at all levels.

The specific objectives of the study reported in the paper were to estimate for EAC the monetary value of DALYs sustained by all ages from NTDs, and the potential productivity losses within the working age bracket of 15 years and above. Methods: The EAC total monetary value of DALYs TMVD EAC ð Þ lost from all 20 NTDs is the sum of each partner state's monetary value of DALYs lost from all 20 NTDs. The ith partner state's monetary value of DALY from jth disease equals ith state's GDP per capita net of current health expenditure multiplied by DALYs lost from jth disease in 2019. The EAC total productivity losses TEPL EAC ð Þ attributable to DALYs lost from all 20 NTDs is the sum of lost productivity across the seven partner states. The ith partner state's productivity loss associated with jth disease equals ith state's GDP per capita net of current health expenditure multiplied by DALYs lost from jth disease and the ith state's labour force participation rate adjusted for underutilization (unemployment and timerelated underemployment) in 2019. Annually, more than 19,500 people die from diarrhea of which 17,100 are children. Diarrhea, which accounts for 16% of deaths among the children below 5 years, is highly linked to open defecation (OD). Poor excreta disposal remains a major challenge to improved sanitation and hygiene in many communities of Kenya and therefore they continue to practice open defecation. Construction and utilization of a latrine at home is a protective factor for communicable diseases. About 52% of the population practice proper utilization of latrine in low-income countries. Improper utilization of latrines leads to the contamination of the water sources.
