The predictive value of a developmental scale used during the first year of life is of great inte... more The predictive value of a developmental scale used during the first year of life is of great interest when planning early interventions. The predictive value of an instrument is the probability of hitting the diagnosis of disorder or developmental delay of a child. The cut-off point between normal and disability development recommended by the Merrill-Palmer-R Scale (MP-R) is the mean - 1 σ, assuming a normal distribution. The MP-R scores in a sample of 291 children under one year old from the Valencian community were analysed. Even though the distribution of the MP-R scale in this sample was not normal, the forecast results were good. Additionally, the development scores using a new version of the scale were assessed using the Rasch model. Comparing the predictive value of the MP-R using two calculated cut-off points, both methods achieved good predictive values. We discuss if the cut-off point scores based on criteria should be used instead of typical scores.
En la literatura podemos encontrar referencias sobre las consecuencias del consumo de bebidas alc... more En la literatura podemos encontrar referencias sobre las consecuencias del consumo de bebidas alcohólicas durante la gestación y la lactancia desde antes del siglo XIX. Sin embargo, los efectos del alcohol como agente teratógeno fueron descritos relativamente hace poco tiempo, (Lemoine et al (1968) y Jones y Smith (1973)). A estos últimos autores se les atribuye la definición del Síndrome Alcohólico Fetal. El SAF es el problema más conocido derivado del consumo de bebidas alcohólicas durante el embarazo, no obstante se conocen otras muchas consecuencias y trastornos más allá del retraso en el crecimiento, alteraciones en el sistema nervioso central y malformaciones faciales características. This article introduces the System for Early Detection of Developmental Disorders (referred to as... more This article introduces the System for Early Detection of Developmental Disorders (referred to as SDPTD for its abbreviation in Spanish), a system developed in previous papers. The SDPTD is a developmental screening test that includes seven questionnaires, one for each cutoff of age (3, 6, 9, 12, 18, 24 and 36 months). These questionnaires have been designed to be answered by parents. To study its validity, SDPTD was administered to a sample of 728 children (approximately 100 children in each of the seven cutoff age groups). A development scale known as Merrill-Palmer-Revised (MP-R) was used as a criterion test. The development state of the children was tested again one year later. The results show a high level of agreement between parents and professionals. The concurrent validity is high although it varies by cutoff age. Regarding the diagnostic validity a year after the original evaluation, levels of sensitivity and specificity are high enough to consider the system reliable, valid and suitable for screening purposes.
The goal of this article is to analyze the quality and applicability screening instruments aimed ... more The goal of this article is to analyze the quality and applicability screening instruments aimed at detecting developmental delays or disorders. A systematic review of the scientific literature published during the last 25 years (1990-2014) retrieved from different databases is carried out. The study is limited to detection systems based on questionnaires, tests or observation of children`s behavior. The analyzed systems are rated on a scale of quality built “ad hoc”. The results show a high variability of tools and detection systems, many of which are research questionnaires, while others are diagnostic tools used for screening purposes. They can be classified in two large groups of instruments: 1), the ones based on developmental milestones whose temporary failure can mean a delay in the development and 2), those who, assuming the first principle, have identified the milestones that can be precursors of developmental disorders. In the conclusions, recommendations are offered regarding the validation of screening systems in Spain and for the construction of new ones.

The SDPTD (early detection system of developmental disorders) was developed with the main objecti... more The SDPTD (early detection system of developmental disorders) was developed with the main objective of any problems that may arise in the development of a child, in order to begin the necessary interventions. It is designed to be administered via the web and answered by parents or caregivers of the child (although the same way can be professionals who administer it). The SDPTD is composed of two types of questionnaires, which detect the overall psychomotor retardation and other specific target specific disorders such as autism. The questionnaires that detect psychomotor retardation are aimed at children from three months to three years old divided into age brackets (3, 6, 9, 12, 18, 24, 36 months). All questionnaires were administered through a web application so that they are accessible to all people with the restriction of access to the network and sufficient for navigating it through a standard browser.

This evidence does not however tally with social and healthcare workers’ strict recommendations to pregnant women. The purpose of this study is to obtain general information from healthcare workers on practices, knowledge and attitudes, in both the diagnosis, treatment, and prevention of Fetal Alcohol Spectrum Disorders (FASD) and Fetal Alcohol Syndrome (SAF). To obtain this information, we designed a questionnaire that was completed by 278 social and healthcare workers in the city of Valencia. The results suggest the existence of permissive attitudes by social-healthcare workers and lack of training about preventive measures, alcohol detection, diagnosis, prognosis and intervention in children exposed to alcohol during gestation and lactation. The results show that we have a simple tool with which to manage results very acceptable sensitivity and specificity for this type of tool is obtained.
