Methods: This study was conducted to explore the challenges of home nursing care using a qualitative content analysis method. Purposeful and snowball sampling methods were used for sampling. The study was conducted from September 2016 to September 2017 in the provinces of Khorasan and Tehran in Iran. Semi-structured interviews were conducted on 33 nurses who were providing home care. After data saturation, the data were analyzed. Background and aims: Nurses have a responsibility in the wider context, ensuring health indicator... more Background and aims: Nurses have a responsibility in the wider context, ensuring health indicators and improving the quality of nursing services. Nursing literature review shows that it has been a very different opinion in relation to the concept of responsibility in nursing. The aim of this study was to analyze the concept of responsibility by Walker Avant (2005). Methods: Texts search using key words in nursing responsibilities in databases, Iranmedex, SID, Medline, Scopus, Proquest, and Ovid was performed. Domestic and foreign papers and books in English and Farsi languages associated with the concept of responsibility in nursing published between 2000 and 2013 were studied. After the search, 18 articles and five books related to the subject were available for selection and analysis of Walker Avant, and with the help of these resources, related terms were defined. Results: Analysis showed that the concept of responsibility in nursing with words such as accuracy, accountability and commitment are in a line. The responsibility and spend time with the word apathy in conflict with the concept of professional responsibility will not converge. Nursing is a complex process that is identified with the concepts of accountability, responsibility, commitment, altruism and consciousness. Conclusion: When professional ethics in nurses and a sense of spirituality and signifying find manifestation, it will have a significant effect on their responsibility. A responsibility nurse is a responder individual in careing and its goal is satisfaction of nursing services recivers. On the other hand, voluntary work and attention to equality in delivering services, respect to needs and their personality and try to obtaining trust of patients will consider as values of nursing responsibilities.

Background: The families of children with Congenital Heart Disease (CHD) experience challenges in... more Background: The families of children with Congenital Heart Disease (CHD) experience challenges in taking care of their child, which may affect the whole family. Therefore, the families need to manage and organize the caregiving process for the child. In order to help families provide optimized and quality care for their child, it is important to understand how they manage caregiving challenges. This study was conducted with the aim to explore the strategies used by families for managing family caregiving for their child. Materials and Methods: This qualitative study was conducted on families of children with CHD referred to hospitals in Mashhad, Iran. The participants were selected using a purposive sampling method. The data were collected from among 40 eligible participants using in-depth and semi-structured interviews from November 2017 to December 2018. Conventional content analysis was used for data analysis and MAXQDA software for managing the coding process. Results: According to the results, effort to manage caregiving emerged as the main theme, which included the 4 categories of "monitoring the child's health conditions," "organizing family life," "optimizing family life," and "establishing interaction." Conclusions: The families used various strategies to manage caregiving including monitoring of the child's health conditions, organization, and optimization of family life, and effective interaction based on their knowledge, experiences, beliefs, and available sources. The results of the present study can help healthcare professionals and nurses to develop family-centered empowerment programs in order to promote families' abilities to manage family caregiving for a child with CHD.

Such pediatric illness can also lead to negative effects on mothers' physical and mental health. Knowledge about mothers' experiences can aid medical teams to support mothers in playing their roles as care managers. Providing supports to mothers can exert significant effects on mothers' health status and indirectly improve patient outcomes and whole family functioning. This study was aimed at understanding the meanings of care for children undergoing hemodialysis based on mothers' lived experiences. Materials and Methods: This study is a qualitative research using hermeneutic phenomenology. A total of 17 interviews were conducted with 11 mothers of children undergoing hemodialysis. The interview sessions were recorded and transcribed, after which the data were analyzed using van Manen's methodology. Results: The main themes identified in this study was "immersion in an ocean of psychological tension," which suggests that the mothers of the children undergoing hemodialysis are overwhelmed by the numerous psychological pressures that they encounter during their children's treatment. This theme was constituted by the subthemes "bewilderment between hope and despair," "endless concerns," "agony and sorrow," and "a sense of being ignored." Conclusions: The findings indicated the need to implement multilateral support measures that align with the educational, emotional-psychological, and financial needs of mothers with children receiving hemodialysis treatment. Such measures should be taken with the participation of multidisciplinary teams.

For centuries, psychiatric patients have been sent to psychiatric hospitals that often stigmatized and located out of the community. Moreover, these patients are stigmatized by the hospital staff because they are not aware of patients' experiences in this domain. This humiliating attitude leads to low self-esteem, isolation, and frustration, and prevents patients from seeking treatment. Aim: This study aimed to explain the lived experiences of patients with psychiatric disorders on the consequences of stigma in mental health centers. Methods: This hermeneutic phenomenological study is a part of a larger study undertaken for partial fulfillment of the requirement for PhD dissertation in nursing. The main study was conducted on 12 psychiatric patients during 2014-2015. They were selected based on purposeful sampling method. Data were collected using unstructured interviews and analyzed by an interpretative method. Results: Psychiatric hospital as an unsafe place is one of the main themes of the phenomenon under study in the original project. It consists of two sub-themes (i.e., an egregious hospital and cold-hearted white collars) each of which is supported by a number of common meanings. Implications for Practice: The results of this study can shape the interventions and policies to combat and prevent the spread of stigma through health centers about people with psychiatric disorders.

BACKGROUND postpartum depression (PPD) is one of the psychological complications of mothers who h... more BACKGROUND postpartum depression (PPD) is one of the psychological complications of mothers who have experienced severe maternal morbidity/maternal near miss (SMM/MNM) which can adversely affect the wellbeing of mothers, new born infants and other family members, but the risk level in this group is unclear. Therefore, we did a meta-analysis to ascertain the relationship PPD with MNM/SMM. MATERIAL AND METHODS The authors searched relevant studies in databases (Web of Science, PubMed, EMBASE, Clinikalkey, Scopus).The summary odds ratio (OR) along with 95% confidence interval (CI) was calculated by use of random or fixed effects models. RESULTS Four studies were included in qualitative synthesis. The pooled analysis revealed that PPD was significantly associated with an increased risk of MNM/SMM (OR = 1.83; 95% CI 1.37-2.44, p = 0.027). CONCLUSION The results show that the risk of PPD in the MNM mothers are twice as likely as women without MNM. Therefore, more attention should be paid to psychological symptoms such as depression in MNM in order to reduce the long-term burden of maternal morbidity.

Background: Caring for brain-dead patients is one of the hardest duties for nurses, particularly ... more Background: Caring for brain-dead patients is one of the hardest duties for nurses, particularly in Intensive Care Units (ICUs). Objective: This study aimed to explore the nursing challenges of caring for patients diagnosed with brain death. Methods: The present study was conducted as a qualitative conventional content analysis, and data collection was carried out by field notes and 15 semi-structured and in-depth interviews with nurses working in the ICUs. Data analysis co-occurred simultaneously with data collection. Results: Seven themes were extracted from the data: uncertainties and conflicts between physicians and nurses, tensions in breaking the news of patient's brain death to families, stressful experience of caring for the first time, nurses' physical and psychological afflictions due to complex care tensions, stress of being blamed by patients' family, difficulty in tackling the emotions of patients' family, and finally, a sense of lack of support and protection in care. Finally, the main theme of turbulent confrontation with successive chains of tension in caring brain-dead patients was abstracted. Conclusions: Since nurses confront chain of tensions while caring for a brain-dead patient, this can affect the quality of this vital role to keep the transplantable organs viable; furthermore, authorities should implement special support programs for nurses.

Background: The role of women and men is changing across the world, and women, including pregnant... more Background: The role of women and men is changing across the world, and women, including pregnant women, are adopting newer roles in traditional societies like Iran. This study aimed to explore the meaning of pregnant women's experiences regarding their social roles in the sociocultural context of Iran. Materials and Methods: This study was carried out using an ethnophenomenological approach. Participants included 16 pregnant women who attended health centers, hospitals, and private obstetric clinics in Mashhad, Iran, between 2016 and 2017 and were selected based on purposive sampling. In-depth semistructured interviews, vignette interviews, participant observations, and field notes were used to collect data. To analyze data, six-step van Manen's (1997) descriptive-interpretive phenomenological approach was used. Results: Through data analysis, the overarching theme of “selection, management, and adjustment of various roles to play social roles” was emerged. This was consisted of four themes: “Mother's perspective regarding out-of-home employment, incompatibility between pregnancy and social roles, mother's management strategies to play different roles, and husband's authority regarding mother's employment.” Conclusions: The consequence of reciprocal endeavors of pregnant women along with their husbands as well as their work environment expectations tends to selection, management, and adjustment of feminine roles. Since the employment of pregnant women leads to their more physical and psychological involvement, not only the problems of working women but also the expectations and rules of the workplaces as well as the requests of their husbands should be taken into account.

Background: Despite the clinical importance of adherence to treatment in β-Thalassemia Major (β-T... more Background: Despite the clinical importance of adherence to treatment in β-Thalassemia Major (β-TM) patients, poor adherence remains a prevalent and persistent problem in these patients. Objective: The aim of this study was to explore Iranian β-TM patients' perception regarding the barriers and facilitators of adherence to their treatments. Methods: In this qualitative study, data were collected using purposeful sampling and semi-structured interviews with 18 β-TM patients, 3 family members and 5 healthcare professionals (total=26) in Sari, northern Iran, between August 2015 and May 2017. Data were analyzed using conventional content analysis with the support of MAXQDA software. All the interviews were transcribed verbatim. Data were coded and grouped under specific categories. Results: We identified and classified the perceived barriers of patients' adherence to treatment into six main categories including: (I) long distance challenges, (II) delayed occurrence of complications, (III) fatigue of healthcare providers, (IV) staff workload, (V) insufficient follow-up of healthcare workers, and (VI) busy everyday life. Also, nine main categories emerged as facilitators including: (I) concern of appearance changing, (II) outcomes of treatment, (III) good awareness, (IV) parental monitoring, (V) fear of complications, (VI) social activity, (VII) marriage as a facilitator, (VIII) friendly interaction with the medical staff, and (IX) trusting the medical staff. Conclusion: Our findings provide a broad range of context-specific facilitators and barriers to treatment adherence perceived by Iranian β-TM patients. These factors improve our understanding of the complex and multifactorial nature of adherence to treatments. Health care providers can use these findings in designing and implementing appropriate interventions to promote β-TM patients' adherence to treatment.

The aim of this study was to analyze the concept of responsibility by Walker Avant (2005). Methods: Texts search using key words in nursing responsibilities in databases, Iranmedex, SID, Medline, Scopus, Proquest, and Ovid was performed. Domestic and foreign papers and books in English and Farsi languages associated with the concept of responsibility in nursing published between 2000 and 2013 were studied. After the search, 18 articles and five books related to the subject were available for selection and analysis of Walker Avant, and with the help of these resources, related terms were defined. Results: Analysis showed that the concept of responsibility in nursing with words such as accuracy, accountability and commitment are in a line. The responsibility and spend time with the word apathy in conflict with the concept of professional responsibility will not converge. Nursing is a complex process that is identified with the concepts of accountability, responsibility, commitment, altruism and consciousness. Conclusion: When professional ethics in nurses and a sense of spirituality and signifying find manifestation, it will have a significant effect on their responsibility. A responsibility nurse is a responder individual in careing and its goal is satisfaction of nursing services recivers. On the other hand, voluntary work and attention to equality in delivering services, respect to needs and their personality and try to obtaining trust of patients will consider as values of nursing responsibilities.

Therefore, the families need to manage and organize the caregiving process for the child. In order to help families provide optimized and quality care for their child, it is important to understand how they manage caregiving challenges. This study was conducted with the aim to explore the strategies used by families for managing family caregiving for their child. Materials and Methods: This qualitative study was conducted on families of children with CHD referred to hospitals in Mashhad, Iran. The participants were selected using a purposive sampling method. The data were collected from among 40 eligible participants using in-depth and semi-structured interviews from November 2017 to December 2018. Conventional content analysis was used for data analysis and MAXQDA software for managing the coding process. Results: According to the results, effort to manage caregiving emerged as the main theme, which included the 4 categories of "monitoring the child's health conditions," "organizing family life," "optimizing family life," and "establishing interaction." Conclusions: The families used various strategies to manage caregiving including monitoring of the child's health conditions, organization, and optimization of family life, and effective interaction based on their knowledge, experiences, beliefs, and available sources. The results of the present study can help healthcare professionals and nurses to develop family-centered empowerment programs in order to promote families' abilities to manage family caregiving for a child with CHD.

Such pediatric illness can also lead to negative effects on mothers' physical and mental health. Knowledge about mothers' experiences can aid medical teams to support mothers in playing their roles as care managers. Providing supports to mothers can exert significant effects on mothers' health status and indirectly improve patient outcomes and whole family functioning. This study was aimed at understanding the meanings of care for children undergoing hemodialysis based on mothers' lived experiences. Materials and Methods: This study is a qualitative research using hermeneutic phenomenology. A total of 17 interviews were conducted with 11 mothers of children undergoing hemodialysis. The interview sessions were recorded and transcribed, after which the data were analyzed using van Manen's methodology. Results: The main themes identified in this study was "immersion in an ocean of psychological tension," which suggests that the mothers of the children undergoing hemodialysis are overwhelmed by the numerous psychological pressures that they encounter during their children's treatment. This theme was constituted by the subthemes "bewilderment between hope and despair," "endless concerns," "agony and sorrow," and "a sense of being ignored." Conclusions: The findings indicated the need to implement multilateral support measures that align with the educational, emotional-psychological, and financial needs of mothers with children receiving hemodialysis treatment. Such measures should be taken with the participation of multidisciplinary teams.

For centuries, psychiatric patients have been sent to psychiatric hospitals that often stigmatized and located out of the community. Moreover, these patients are stigmatized by the hospital staff because they are not aware of patients' experiences in this domain. This humiliating attitude leads to low self-esteem, isolation, and frustration, and prevents patients from seeking treatment. Aim: This study aimed to explain the lived experiences of patients with psychiatric disorders on the consequences of stigma in mental health centers. Methods: This hermeneutic phenomenological study is a part of a larger study undertaken for partial fulfillment of the requirement for PhD dissertation in nursing. The main study was conducted on 12 psychiatric patients during 2014-2015. They were selected based on purposeful sampling method. Data were collected using unstructured interviews and analyzed by an interpretative method. Results: Psychiatric hospital as an unsafe place is one of the main themes of the phenomenon under study in the original project. It consists of two sub-themes (i.e., an egregious hospital and cold-hearted white collars) each of which is supported by a number of common meanings. Implications for Practice: The results of this study can shape the interventions and policies to combat and prevent the spread of stigma through health centers about people with psychiatric disorders.

BACKGROUND postpartum depression (PPD) is one of the psychological complications of mothers who h... more BACKGROUND postpartum depression (PPD) is one of the psychological complications of mothers who have experienced severe maternal morbidity/maternal near miss (SMM/MNM) which can adversely affect the wellbeing of mothers, new born infants and other family members, but the risk level in this group is unclear. Therefore, we did a meta-analysis to ascertain the relationship PPD with MNM/SMM. MATERIAL AND METHODS The authors searched relevant studies in databases (Web of Science, PubMed, EMBASE, Clinikalkey, Scopus).The summary odds ratio (OR) along with 95% confidence interval (CI) was calculated by use of random or fixed effects models. RESULTS Four studies were included in qualitative synthesis. The pooled analysis revealed that PPD was significantly associated with an increased risk of MNM/SMM (OR = 1.83; 95% CI 1.37-2.44, p = 0.027). CONCLUSION The results show that the risk of PPD in the MNM mothers are twice as likely as women without MNM. Therefore, more attention should be paid to psychological symptoms such as depression in MNM in order to reduce the long-term burden of maternal morbidity.

Objective: This study aimed to explore the nursing challenges of caring for patients diagnosed with brain death. Methods: The present study was conducted as a qualitative conventional content analysis, and data collection was carried out by field notes and 15 semi-structured and in-depth interviews with nurses working in the ICUs. Data analysis co-occurred simultaneously with data collection. Results: Seven themes were extracted from the data: uncertainties and conflicts between physicians and nurses, tensions in breaking the news of patient's brain death to families, stressful experience of caring for the first time, nurses' physical and psychological afflictions due to complex care tensions, stress of being blamed by patients' family, difficulty in tackling the emotions of patients' family, and finally, a sense of lack of support and protection in care. Finally, the main theme of turbulent confrontation with successive chains of tension in caring brain-dead patients was abstracted. Conclusions: Since nurses confront chain of tensions while caring for a brain-dead patient, this can affect the quality of this vital role to keep the transplantable organs viable; furthermore, authorities should implement special support programs for nurses.

This study aimed to explore the meaning of pregnant women's experiences regarding their social roles in the sociocultural context of Iran. Materials and Methods: This study was carried out using an ethnophenomenological approach. Participants included 16 pregnant women who attended health centers, hospitals, and private obstetric clinics in Mashhad, Iran, between 2016 and 2017 and were selected based on purposive sampling. In-depth semistructured interviews, vignette interviews, participant observations, and field notes were used to collect data. To analyze data, six-step van Manen's (1997) descriptive-interpretive phenomenological approach was used. Results: Through data analysis, the overarching theme of “selection, management, and adjustment of various roles to play social roles” was emerged. This was consisted of four themes: “Mother's perspective regarding out-of-home employment, incompatibility between pregnancy and social roles, mother's management strategies to play different roles, and husband's authority regarding mother's employment.” Conclusions: The consequence of reciprocal endeavors of pregnant women along with their husbands as well as their work environment expectations tends to selection, management, and adjustment of feminine roles. Since the employment of pregnant women leads to their more physical and psychological involvement, not only the problems of working women but also the expectations and rules of the workplaces as well as the requests of their husbands should be taken into account.

Objective: The aim of this study was to explore Iranian β-TM patients' perception regarding the barriers and facilitators of adherence to their treatments. Methods: In this qualitative study, data were collected using purposeful sampling and semi-structured interviews with 18 β-TM patients, 3 family members and 5 healthcare professionals (total=26) in Sari, northern Iran, between August 2015 and May 2017. Data were analyzed using conventional content analysis with the support of MAXQDA software. All the interviews were transcribed verbatim. Data were coded and grouped under specific categories. Results: We identified and classified the perceived barriers of patients' adherence to treatment into six main categories including: (I) long distance challenges, (II) delayed occurrence of complications, (III) fatigue of healthcare providers, (IV) staff workload, (V) insufficient follow-up of healthcare workers, and (VI) busy everyday life. Also, nine main categories emerged as facilitators including: (I) concern of appearance changing, (II) outcomes of treatment, (III) good awareness, (IV) parental monitoring, (V) fear of complications, (VI) social activity, (VII) marriage as a facilitator, (VIII) friendly interaction with the medical staff, and (IX) trusting the medical staff. Conclusion: Our findings provide a broad range of context-specific facilitators and barriers to treatment adherence perceived by Iranian β-TM patients. These factors improve our understanding of the complex and multifactorial nature of adherence to treatments. Health care providers can use these findings in designing and implementing appropriate interventions to promote β-TM patients' adherence to treatment.
