Background: Acute disseminated encephalomyelitis (ADEM) is an inflammatory demyelinating disease ... more Background: Acute disseminated encephalomyelitis (ADEM) is an inflammatory demyelinating disease of the CNS, characterized by fulminant multifocal neurologic injury and distinct neuropathologic findings. Case Report: We report a case of a forty-nine-year-old female who presented with progressive weakness of right side of the body of one-week duration. Clinical, laboratory and radiological features favored demyelination. There was temporal association with taking SARS COV 2 vaccination (ChAdOx1 n cov -19 (AZD1222)). She was treated with a course of steroids and made a good recovery. Conclusion: To our knowledge this is a rare case of ADEM noticed after ChAdOx1 nCoV-19 vaccine (AZD1222).

Here, we report an unusual complication of intracerebral hemorrhage following a snake bite. Case Report A 48-year-old diabetic and hypertensive male patient presented with loss of consciousness following a suspected snake bite. Unconsciousness was preceded by vomiting. His neurological status deteriorated to a Glasgow coma score of 7/15 (E1M5V1). Pupils showed anisocoria. Computed tomographic scan of the brain revealed large intracerebral hemorrhage in the left temporal lobe and another large intracerebral hemorrhage in the left parietal lobe with significant mass effect and signs of early transtentorial herniation. He was taken up for left frontotemporoparietal craniotomy and evacuation of left temporoparietal intracerebral hemorrhage. Rupture of an intracranial aneurysm is the most common cause of subarachnoid hemorrhage in pregnancy and during puerperium. Intracranial aneurysm rupture during pregnancy is disastrous for the mother as well as the fetus. We report a case of a dissecting left middle cerebral artery aneurysm in a 25-year-old woman, G2P1L1, at 18 weeks of gestation, who presented with a 1-day history of giddiness, slurring of speech, and facial deviation to the left.

Study Setting: This study was conducted in Family Medicine outpatient clinics in Thrissur, Kerala. Subjects and Methods: A total of 50 cases (diagnosed case of Type 2 DM) and 50 healthy controls were taken into consideration randomly. Thyroid function tests were conducted using the chemiluminescence assay. Then, it was analyzed statistically after tabulation. Statistical Analysis: Statistical analysis was performed using the Chi-square test for categorical variables. Student's t-test was used for finding significance between the means. We describe a patient with GPA who initially presented with multiple ischemic toes. The diagnosis was supported by a positive c-ANCA (anti-neutrophil cytoplasmic antibody). The involvement of digital arteries in this condition is very unusual, and only a few cases have been reported so far.

soft swellings (most of them pedunculated) along with multiple café-au-lait spots were seen all o... more soft swellings (most of them pedunculated) along with multiple café-au-lait spots were seen all over his body, including the scalp [Figure 1]. He also had inguinal and axillary freckling. Ophthalmologic examination was normal. The patient had no bone lesions. There was no family history of neurofibromatosis. Abdominal imaging showed a well-defined cystic lesion in the right adrenal gland with multiple enhancing septations and solid areas with hemorrhage, suggestive of cystic pheochromocytoma. Computed tomography (CT)-brain showed a variegated space occupying lesion (SOL) in the right-frontal-lobe with a large cystic component and an enhancing solid component. Moderate surrounding edema was noticed with mass effect and midline shift to left. Magnetic resonance imaging (MRI)-brain [Figure 2] confirmed a predominantly cystic intra-axial right-frontal-SOL with enhancing solid component, perilesional edema, and mass effect. Postural hypotension, elevation of 24 h urinar y catecholamines [Table 1], and the findings on abdominal imaging were diagnostic of pheochromocytoma. The patient was adequately blocked with prazosin (starting with 1 mg daily, which was increased to 8 mg daily over a period of 5 days, all the while monitoring his blood pressure and heart rate). There was no need for beta-blocker. The patient was adequately hydrated and liberal salt intake was encouraged to prevent any episodes of hypotension. Thereafter, the patient underwent laparoscopic right adrenalectomy. Histopathology was consistent with pheochromocytoma. Three months back he was treated for oral and genital ulcers with oral steroids. His ulcers improved and shifted to indigenous medication. His clinical examination showed polyneuropathy. CSF study did not show albuminocytological dissociation. Nerve conduction study showed demyelinating polyneuropathy.
