Le bupropion a entame, avec le traitement de la dependance tabagique, une deuxieme carriere, apre... more Le bupropion a entame, avec le traitement de la dependance tabagique, une deuxieme carriere, apres celle d'un antidepresseur. Cette molecule inhibe la recapture de la noradrenaline et de la dopamine et antagonise les recepteurs nicotiniques. Deux etudes princeps d'efficacite, l'une effet-dose et l'autre comparative a un traitement nicotinique, ont montre l'efficacite du bupropion a la dose de 300 mg/j sur l'abstinence tabagique par rapport au placebo et a un traitement standardise par timbres nicotiniques. Ces etudes ont egalement montre un effet de reduction du syndrome de sevrage, des envies imperieuses de fumer et de la prise de poids. L'effet sur la dependance tabagique semble distinct de l'effet antidepresseur de la molecule. Le bupropion a un bon profil de tolerance, avec des effets secondaires mineurs, essentiellement l'insomnie et la secheresse de la bouche.

L'objectif de notre travail etait de construire et de valider une echelle de motivation a l&#... more L'objectif de notre travail etait de construire et de valider une echelle de motivation a l'arret du tabac. Un premier travail avait deja permis de selectionner sept questions parmi 25 questions explorant neuf domaines de la motivation, a partir d'une evaluation semi-structuree d'expert. La presente etude a concerne 261 fumeurs recrutes dans 30 centres de tabacologie, qui ont rempli le questionnaire de sept questions a deux reprises : lors de la prise de rendez-vous, puis de la consultation. Les investigateurs ont ete formes a evaluer la motivation sur une echelle analogique visuelle a partir d'un entretien structure. Un modele de regression multiple a permis d'obtenir un coefficient de ponderation pour chacune des sept questions. L'heterogeneite des coefficients nous a conduits a supprimer trois questions qui n'intervenaient que marginalement dans le score final. Plusieurs tentatives de ponderation ont ete realisees pour obtenir, par approximations successives, un score maximal de l'echelle commode a utiliser et une ponderation harmonieuse des items de chaque question. Finalement, l'echelle comporte quatre questions et un score maximal de 20. Une analyse de regression lineaire entre le score de l'echelle et la mesure de la motivation par l'investigateur sur l'echelle analogique visuelle a montre un coefficient de correlation de Spearman a 0,75 (p < 0,0001). L'application de la cotation retenue aux deux auto-questionnaires de motivation successivement remplis par les patients a permis de trouver une evolution d'intensite faible, mais significative (p = 0,04). Cette echelle parait donc sensible au changement. En conclusion, nous avons construit et valide une echelle d'auto-evaluation de la motivation a arreter de fumer en quatre questions: Q-MAT. La validite predictive du succes de cette echelle est en cours d'etude.

It... more The paper presents an environment for teaching elements of a computer system memory hierarchy. It is made up of a hierarchical memory system, a reference manual, a software package and a set of laboratory experiments. The hierarchical memory system is devised to cover the virtual memory and translation lookaside buffer, the cache memory, and the main memory. The reference manual provides all implementation details with the appropriate circuits drawings and detailed descriptions. For the devised hierarchical memory system a software package, which includes the graphical simulator with the accompanying tools, is developed. They allow one to carry out the simulation down to the register transfer level by executing a set of laboratory experiments

One hundred and twenty four patients with severe asthma requiring maintenance treatment with oral... more One hundred and twenty four patients with severe asthma requiring maintenance treatment with oral corticosteroids were included in a multicentre, double-blind, randomized study comparing the effects of inhaled beclomethasone dipropionate (BDP) (250 micrograms.puff-1), beginning with 1,000 micrograms daily, vs placebo (P). Pulmonary function was assessed and dosage of prednisone and BDP (or P) were adjusted every 15 days according to a clinical score. Our results showed, after 3 months: 1) A greater drop-out rate in the P group than in the BDP group (36 vs 6%, respectively, p less than 0.01); 2) A total weaning from prednisone in 76% of patients in the BDP group (mean BDP dosage = 1,270 +/- 340 micrograms.day-1, mean +/- SD), vs 34% in the P group (p less than 0.001). The mean daily dosage of prednisone was reduced from 17 +/- 7.5 mg to 3.1 +/- 7.4 mg in the BDP group vs 15.6 +/- 7.7 mg to 9.1 +/- 9.4 mg in the P group (p less than 0.001) without any relationship between the steroid-...

Although most patients with asthma improve with currently available drugs, there appears to be a ... more Although most patients with asthma improve with currently available drugs, there appears to be a subset of patients with asthma resistant to intensive treatment, including large doses of systemic corticosteroids. In 10 patients with asthma difficult to treat, a double-blind, placebo-controlled, crossover study was performed to evaluate whether long-term, continuous subcutaneous infusion of large doses of salbutamol, in addition to steroids, could improve pulmonary function, compared with intermittent nebulization of salbutamol. Four weeks of administration of large doses (greater than 14 mg/day) of beta 2-agonists were well tolerated, elicited a significant decrease of corticosteroid consumption (p less than 0.01), and were associated with an improvement in pulmonary function (p less than 0.01), compared with run in or the placebo administration period. Mean morning and evening peak expiratory flow rates were similar during all periods, and the variability between morning and evening peak expiratory flow rates was only slightly and nonsignificantly reduced during the period when salbutamol was administered subcutaneously. Both nebulized and subcutaneous salbutamol elicited similar metabolic and muscular side effects, but these untoward reactions never caused any patient to stop the treatment. Local infection at the injection site was observed in two of 10 patients with continuous subcutaneous infusion. Tachyphylaxis to beta 2-agonist, as measured by the reversibility of FEV1 to inhaled salbutamol 12 hours after the end of each period, did not occur. In conclusion, large doses of beta 2-agonist administered to patients with severe, chronic, and steroid-dependent asthma were able to improve respiratory function and to decrease corticosteroids requirements.

Objective: To compare the characteristics of a group of pregnant women who smoked until delivery ... more Objective: To compare the characteristics of a group of pregnant women who smoked until delivery with women who gave up smoking during pregnancy. Study design: Questionnaire-based, descriptive study of 979 pregnant women in four regions of France. The variables analysed included the characteristics of the mother and neonate at delivery, the smoking habits of the mother before and during pregnancy, the perception of risk linked to smoking, and the reasons for giving up smoking. Results: Eighteen percent of women smoked until delivery. Fifty-five percent of women gave up smoking during pregnancy, usually in the first trimester. Women who failed to give up smoking were more disadvantaged psychosocially and were more likely to live alone. There was a higher level of dependency among women who failed to give up smoking and a lower perception of risk to the foetus. Among the women who had tried to reduce their tobacco consumption without success, 6% stated that they had been motivated by medical information compared to 28% of women who succeeded in stopping smoking. Conclusions: Despite the risks associated with smoking the number of pregnant women who smoke until delivery remains high. Knowledge of the psychosocial profile and degree of dependency of these patients is an important step to managing this problem.

In a multicenter, randomized, double-blind study, inhaled beclomethasone dipropionate (BDP) 1,500... more In a multicenter, randomized, double-blind study, inhaled beclomethasone dipropionate (BDP) 1,500 micrograms/day was compared to placebo in 43 chronic asthmatic patients uncontrolled by inhaled salbutamol and oral theophylline. During the prestudy period, a test of maximal steroid reversibility with oral prednisolone 0.5 mg/kg/day for 14 days was performed. The therapeutic response was measured over an 8-wk period as the ability to maintain the clinical improvement and the optimal pulmonary function induced by prednisolone. During the study, severe asthma exacerbation occurred in one (5%) of the 21 patients who received BDP and in 15 (78%) of the 22 patients who received placebo (p less than 0.001). In patients who received BDP, FEV1 and peak expiratory flow (PEF) remained above the optimal postprednisolone value, with a trend to improvement during the 8-wk study period. In patients who received placebo, FEV1 and PEF decreased and remained below the optimal value. We conclude that, in chronic asthma, inhaled BDP 1,500 micrograms/day maintains the optimal pulmonary function in addition to the clinical benefit induced by a short course of oral corticosteroids.

Objective: To compare the characteristics of a group of pregnant women who smoked until delivery ... more Objective: To compare the characteristics of a group of pregnant women who smoked until delivery with women who gave up smoking during pregnancy. Study design: Questionnaire-based, descriptive study of 979 pregnant women in four regions of France. The variables analysed included the characteristics of the mother and neonate at delivery, the smoking habits of the mother before and during pregnancy, the perception of risk linked to smoking, and the reasons for giving up smoking. Results: Eighteen percent of women smoked until delivery. Fifty-five percent of women gave up smoking during pregnancy, usually in the first trimester. Women who failed to give up smoking were more disadvantaged psychosocially and were more likely to live alone. There was a higher level of dependency among women who failed to give up smoking and a lower perception of risk to the foetus. Among the women who had tried to reduce their tobacco consumption without success, 6% stated that they had been motivated by medical information compared to 28% of women who succeeded in stopping smoking. Conclusions: Despite the risks associated with smoking the number of pregnant women who smoke until delivery remains high. Knowledge of the psychosocial profile and degree of dependency of these patients is an important step to managing this problem.

In a multicenter, randomized, double-blind study, inhaled beclomethasone dipropionate (BDP) 1,500... more In a multicenter, randomized, double-blind study, inhaled beclomethasone dipropionate (BDP) 1,500 micrograms/day was compared to placebo in 43 chronic asthmatic patients uncontrolled by inhaled salbutamol and oral theophylline. During the prestudy period, a test of maximal steroid reversibility with oral prednisolone 0.5 mg/kg/day for 14 days was performed. The therapeutic response was measured over an 8-wk period as the ability to maintain the clinical improvement and the optimal pulmonary function induced by prednisolone. During the study, severe asthma exacerbation occurred in one (5%) of the 21 patients who received BDP and in 15 (78%) of the 22 patients who received placebo (p less than 0.001). In patients who received BDP, FEV1 and peak expiratory flow (PEF) remained above the optimal postprednisolone value, with a trend to improvement during the 8-wk study period. In patients who received placebo, FEV1 and PEF decreased and remained below the optimal value. We conclude that, in chronic asthma, inhaled BDP 1,500 micrograms/day maintains the optimal pulmonary function in addition to the clinical benefit induced by a short course of oral corticosteroids.
