Among the advantages of perforator flaps one can name the sparing of the muscle function, the ability to utilize tissue as needed, optimized healing period and better aesthetic results. Utilization of perforator flaps has therefore been popularized in the past 10 years with various classification and nomenclature complexities. This article aims to review the terminology, anatomy and classifications, and nomenclatures of perforator flaps with a review of the most common perforator flaps, their surgical techniques, and relevant literature.

Methods: Human cartilage-derived cells (hCDCs) were isolated from the cartilage tissue of a 57-year-old woman diagnosed with gonarthrosis. Tissue samples were cultured in Dulbecco’s modified Eagle’s medium (DMEM) and RPMI-1640. The cells’ chondrogenic activities were observed. After serial passagings, cells were divided into 4 groups at the end of the 6th week. On the 14th day, proliferated cells were examined using an inverted microscope with x4, x10, x20 and x40 magnification and microphotographs were taken. Living cell quantity was determined on the first and 14th days using MTS-ELISA cell proliferation assay. Results: DMEM (without adding ITS premix solution) and RPMI-1640 containin...

The study included 44 patients (53 feet) with plantar fasciitis. The mean symptom duration was 7.2+/-5.9 weeks (range 1 to 24 weeks). Calcaneal spurs were detected in 12 feet. All the patients received classic conservative treatment and all were recommended to use a night splint that kept the ankle in 5-degree of dorsiflexion for eight weeks. Twenty-five patients (14 females, 11 males; 31 feet) did not accept to use a night splint, whereas 19 patients (12 females, 7 males; 22 feet) did. Evaluations were made with the AOFAS ankle-hindfoot rating scale and a visual analog scale (VAS) before and after two months of treatment. The mean follow-up periods were 33.8 months (range 12 to 54 months) and 32.7 months (range 13 to 53 months) for those who completed treatment with and without the use of a night splint, respectively. Although there were no significant differences between...

They are, however, not uncommon among workers in industry using paint, automotive grease, solvents and diesel oil. The prognostic factors are the type, amount and temperature of the material and the pressure of injection. The results of 14 surgically treated high-pressure injection injuries of the hand with a minimum of two years follow-up are reported. Ten cases required soft tissue debridement and foreign body removal. Six required reconstructive microsurgical procedures and one underwent digital tip amputation, which followed major and devastating soft tissue problems. In the present study, we describe the mechanism of the soft tissue damage and the principles of treatment. This study confirms the fact that high-pressure injection injury to the hand is a significant problem, which can easily lead to serious sequelae and, even, amputation.

The patient was furthermore treated with a continous lumbal epidural block for one week with fentanyl, bupivacaine, epinephrine and clonidine. After one week the epidural was tapered and discontinued. The patient was discharged from hospital after 8 days with pregabalin 300mg daily. During the first week the sympathetic response with sudophoresis gradually disappeared. Thin-fibre diagnosis 2 weeks post-injury showed normal efferent fibres but affected thermosensibility on the afferent fibres of the L5-dermatome. The patient was almost without any pain at discharge and reduced the pregabalin over a two months period. The patient is now fully recovered without any neurologic sequelae.

We describe a 18-year-old male patient with a benign chondroid syringoma that occurred at an unusual site, leading to compression symptoms of the ulnar nerve causing symptoms at the hypothenar region of the right hand. In this study new configurations of plate/bone constructs with varied screw angulations were tested. The conventional screw configurations a parallel set of screws aligned vertical to interface were tested against two new configurations, called the divergent configuration and the convergent configuration. A total of four construct systems were prepared for tests. The samples used in the experiments are listed. Sample A, named conventional construct, used a set of parallel screws drilled vertical to the interface plane. Sample B, named convergent construct, used screw holes drilled such that their axes converged. Samples C and D, named divergent construct, used screw holes with diverging axes. The difference between Samples C and D is in the angle of alignment between their screw axes and the normal to the interfacial plane, the f...

Devolopmental insuffuciency of bony structures yields asymetric ball and socket formation, which usually associated with trophic changes and instability. Total hip joint replacement has successfull results in patients with hip dysplasia in adulthood. The major goal of the treatment is rebuilding of a new stable artificial joint with painless range of motion. There is a need of a meticulous preoperative planning assessing the anatomic severity of the pathology, which is consisted of involved bony and soft tissue components. Limb length discrepency, quality of bone stock, muscle atrophies, weakness and contractures should be evaluated before surgery. There are several classifications for assesment of acetabuar dysplasia. METHODS Data from 25 patients (24 females, 1 male) who underwent surgical treatment with growing rods for scoliosis between 1997 and 2007 were evaluated retrospectively. Dual growing rods were used in 16 patients and single growing rods in 9. Patients' average age was 7.38 ± 3.8 years at the initial surgery. Cobb angle, T1-S1 length, and instrumentation length were measured radiographically. Respiratory functions were evaluated at the final follow-up. RESULTS Patients received an average of 4.2 lengthening treatments over an average period of 44.9 months. Cobb angles improved from 56.7° to 25.1° after final fusion. T1-S1 length increased from 27.2 ± 3.4 to 34.9 ± 3.6 cm after the initial surgery and 38.6 ± 3.7 cm post final fusion. Average growth was 1 ± 0.4 cm per year. Mean values of respirat...

It is a minimally invasive surgery that prevents blood loss and results in minimal damage to the thoracic wall, better cosmetic appearance and shorter hospitalization time compared to conventional thoracotomy. Technical difficulty, a steep learning curve, longer operation times and applicability in a limited patient group with a certain type of curve are the disadvantages of this procedure.
