A series o f six patient management problems, with clinical case histories from the authors' own ... more A series o f six patient management problems, with clinical case histories from the authors' own practices, was presented in a Norwegian medical journal (Utpostenl during 1981 and the spring o f 1982. The readers were asked to respond to a number o f statements which related to each case history, using a 5-point rating scale. The answers were prepared from return forms mailed anonymously to the authors, leaving the doctors with copies of their own answers.

[Dissemination of research--a neglected obligation]

In contrast to research and teaching duties, the obligation to disseminate research results to th... more In contrast to research and teaching duties, the obligation to disseminate research results to the general public is poorly described and not lived up to in academic medicine. At the Norwegian University of Science and Technology's Faculty of Medicine, an interdisciplinary group met with academics to discuss and identify means for improvements. The academics reported positive attitudes to popularising and communicating their findings. Dissemination of research as a working area was, however, characterised as fragmented, incidental and too much dependent on individual enthusiasm. The most important initiatives in order to achieve better dissemination were said to be the following: more and better training in popularization and dealing with the media; development of an infrastructure for dissemination at a decentralised institutional level (defined responsibility, routines etc.); and encouragement of good presentations by making dissemination of research meritorious.

Korrespondanse: johan.h.bjorngaard@ntnu.no SAMMENDRAG En relativ stor andel av befolkningen i yrk... more Korrespondanse: johan.h.bjorngaard@ntnu.no SAMMENDRAG En relativ stor andel av befolkningen i yrkesaktiv alder i de nordiske landene mottar uførepensjon, og i Norge er det en klar tendens til økning. Parallelt med dette har interessen for forskning rundt uførepensjon som fenomen økt. Vi har i denne studien beskrevet den epidemiologiske forskningen om uførepensjon i Norden. I en systematisk litteraturgjennomgang fant vi 118 aktuelle artikler. Alle de nordiske landene er godt representert. Kohortstudier med uførepensjon som endepunkt utgjorde hovedvekten av materialet, hvor 67 var populasjonsbaserte mens 29 tok utgangspunkt i selekterte pasientpopulasjoner. I alt seks kohortstudier benyttet uførepensjon som eksponering. Vi fant videre syv kasus-kontrollstudier og ni forsøk. Det er betydelig dokumentasjon på at ulike sykdommer og helseplager er assosiert med økt risiko for uførhet, i tillegg til at sosioøkonomiske og arbeidsrelaterte forhold er av betydning. Gjennomgangen viser at den epidemiologiske forskningen for det meste har rettet blikket mot individuelle årsaker til uførepensjonering. Selv om denne forskningen er viktig, kan den neppe forklare økningen vi har sett i det siste tiåret. Den videre forskningen bør utforske naermere hvordan individuell sårbarhet ender i utstøtelse fra arbeidslivet. Med tanke på den omfattende forskningen om årsaker på dette feltet, bør man også i større grad gjennomføre forsøk for å bedre utsatte gruppers arbeidsdeltakelse. % av ratenivå i 1999! År! [Physician and expert--a conflict situation? General practitioners work with disability pension's errands in a time of restraint policy]

To study how general practitioners perceive their role and conduct their work as certifying physi... more To study how general practitioners perceive their role and conduct their work as certifying physicians for patients applying for disability pension, with emphasis on effects of the revision of the law in 1991 which narrowed the medical criteria for eligibility. Personal in-depth interviews, tape recorded for qualitative analysis. 24 general practitioners in the city of Trondheim and rural communities in the county of Møre og Romsdal. Attitudes and behaviour varied considerably: On one side "the patient's advocate" strived for what he considered to be the best overall result for his patient--as opposed to "the consultant", who regarded himself as neutral and loyal to the policy makers. Narrowing the medical eligibility criteria for disability pension was perceived as reducing the doctor's influence on the outcome of an application. Many "consultants" welcomed the revision. Some "advocates" sought to counteract the loss of influence on the patient's situation through various coping strategies related to the medical certificate. Others intensified the search for alternative solutions to the patient's problems. A third strategy implied leaving the "advocate's" involvement in favour of a more disengaged role in order to make ideals meet with reality. Many certifying physicians seem to have been under considerable pressure, owing to the conflicting interests of patients and authorities, and we ask whether, under such conditions, the typical doctor tends to adopt a more "bureaucratic" and less "hippocratic" role.

In response to a recent article in Nord Med (No 11/91) this paper raises a critique of the reanal... more In response to a recent article in Nord Med (No 11/91) this paper raises a critique of the reanalysis of two Nordic studies on the effects of unemployment on health: The model based on varying relationships between vocational ability and job requirement does not invalidate earlier findings. However, the interpretation of measurements of health in individual based follow-up studies warrants a discussion, as well as the external validity of their findings. Unemployment has been shown to affect health related behaviour, like consultation rates, referrals, sick leave and disability pension, in well controlled studies. These effects are of importance to the individuals and the health- and social services, regardless of their relation to disease in a biomedical sense.

Narrowing the medical eligibility criteria for disability pension was perceived as reducing the doctor's influence on the outcome of an application. Many "consultants" welcomed the revision. Some "advocates" sought to counteract the loss of influence on the patient's situation through various coping strategies related to the medical certificate. Others intensified the search for alternative solutions to the patient's problems. A third strategy implied leaving the "advocate's" involvement in favour of a more disengaged role in order to make ideals meet with reality. Many certifying physicians seem to have been under considerable pressure, owing to the conflicting interests of patients and authorities, and we ask whether, under such conditions, the typical doctor tends to adopt a more "bureaucratic" and less "hippocratic" role.

In response to a recent article in Nord Med (No 11/91) this paper raises a critique of the reanal... more In response to a recent article in Nord Med (No 11/91) this paper raises a critique of the reanalysis of two Nordic studies on the effects of unemployment on health: The model based on varying relationships between vocational ability and job requirement does not invalidate earlier findings. However, the interpretation of measurements of health in individual based follow-up studies warrants a discussion, as well as the external validity of their findings. Unemployment has been shown to affect health related behaviour, like consultation rates, referrals, sick leave and disability pension, in well controlled studies. These effects are of importance to the individuals and the health- and social services, regardless of their relation to disease in a biomedical sense.
