The aneurysm was recognized and ruptured during a surgical intervention for otitis. Successful endovascular treatment of the aneurysm was performed by occlusion of the parent vessel using detachable balloon and coils.

Lenfanjioleimyomatozis (LAM) çoğunlukla genç kadınları etkileyen, genellikle solunum yetmezliği v... more Lenfanjioleimyomatozis (LAM) çoğunlukla genç kadınları etkileyen, genellikle solunum yetmezliği ve ölüme ilerleyebilen bir tablodur. 33 yaşında postpartum periyodda kadın hasta ilerleyici nefes darlığı ve yüksek CA-125 seviyeleri ile başvurdu. Hastada bilateral şilopnömotoraks, şilöz asit mevcut idi ve açık akciğer biyopsisi ile LAM tanısı aldı. Hastanın takibi oldukça karmaşıktı ve şilöz asit ve plevral efüzyonlar tedaviye çok dirençliydi. Betadin plörodezisi şilotoraksların tedavisinde oldukça başarılı oldu. Literatürde yüksek CA-125 seviyeleri ile başvuran LAM vakası sunulmamıştır. Takipte CA-125 seviyelerinin yararlı bir parametre olabileceğini düşünmekteyiz.

Approximately 75-80% of myxomas originate from the left atrial septum; 10-20% are localized in the right atrium; and the rest of 6-8% are bi-atrial, or in the right or left ventricle. 1 Isolated left ventricular (LV) myxoma is extremely rare, accounting for 2.5-4% of all cases. 2 The most common treatment strategy is surgical extirpation under cardiopulmonary bypass, with an excellent prognosis. We present an extremely rare case of 2 LV myxomas originating from the mitral anterior and posterior leaflets into the LV cavity and obstructing the outflow tract (LVOT), and discuss the current literature.

Abdominal aortic surgery can cause ischemic/reperfusion (I/R) injury in not only the lower extremities, but also in the remote organs and tissues such as lungs, kidneys, heart, and liver during abdominal aortic surgery. It can result in mortality and morbidity because of the remote organ injury in early postoperative period. In this study, we investigate the effects of iloprost and vitamin C on the kidney remote organ damage after I/R following abdominal aortic surgery. Material and methods. Thirty-four adult male Wistar rats were used and divided into five groups. I/R was studied infrarenally in the abdominal aorta following a median laparotomy. The left kidney was excised immediately following the laparotomy in group I (n ‫؍‬ 6, normal group). Group II (n ‫؍‬ 6) was the sham group. Group III (n ‫؍‬ 6, control group) was subjected to 3 h of ischemia followed by an hour of reperfusion. Group IV (n ‫؍‬ 8) was given iloprost 20 ng/kg/min during I/R period before aortic-clamping. Group V (n ‫؍‬ 8) was given vitamin C 100 mg/kg during I/R period before aortic-clamping. Arterial blood samples were obtained to determine the levels of blood pH, pO 2 (mmHg), pCO2 (mmHg), HCO 3 (mmol/L), and plasma malondialdehyde (MDA, nmol/mL) at the end of reperfusion period in all groups. The left kidneys were used for remote measurements of tissue MDA (nmol/g.w.t) and scored by histopathological examination for acute inflammation. Results. While the arterial blood pO 2 and HCO 3 levels significantly increased, the plasma and renal parenchymal MDA levels significantly decreased in both group IV and group V when compared to group III (P < 0.05). Histopathological and acute inflammation scores statistically decreased in both group IV and V compared with group III (P < 0.05). Although MDA levels, histopathologic and acute inflammation scores in group V were lower than group IV, the differences were not statistically significant (P > 0.05). Conclusion. Both iloprost and vitamin C decreased remote organ damage on the kidney after I/R of lower extremities in the rat model. However, vitamin C is more effective than iloprost in preventing postoperative renal dysfunction.

Koroner bypass cerrahisi (CABG) sırasında safen ven grefti hazırlanırken gerek cerrahi manüplasyo... more Koroner bypass cerrahisi (CABG) sırasında safen ven grefti hazırlanırken gerek cerrahi manüplasyonlara bağlı, gerekse organ banyosunda iskemik ortamda bekletilmeye bağlı intimal hasar oluşmaktadır. İn-situ teknik ile damar endotelinin iskemiden korunacağı ve intimal hasarın azalacağı öngörüsü ile prospektif bir çalışma planlandı. Gereç ve Yöntemler: CABG yapılacak ardışık 40 hastadan hazırlanan safen ven grefti çalışmaya alındı. Yirmişer hastadan oluşan iki grup oluşturuldu. Grup-1'de geleneksel "açık cilt kesiği tekniğiyle" safen ven grefti hazırlandı ve papaverinli organ banyosunda 45 dk. bekletildi. Grup-2'de in-situ teknikle safen ven grefti hazırlandı ve etrafına papaverinli spanç sarılarak 45 dk. bekletildi. Histopatolojik değerlendirme için, greftlerin distal uç kısmından 1 cm'lik safen parçası alındı. Doku örnekleri hemotoksilin-eosin ile boyanarak ışık mikroskopisinde incelendi ve histopatolojik olarak skorlandı. İstatistiksel değerlendirmede ki-kare testi kullanıldı. Bulgular: Her iki grupta da safen ven örneklerinden elde edilen patolojik hasar median skorları 2 bulundu ve istatistiksel fark saptanmadı (p = 0.987). Her iki grupta da hafif-orta derecede intimal desquamasyon saptandı. Sonuç: İn-situ teknikle safen ven grefti hazırlama geleneksel açık cilt kesiği tekniğiyle hazırlamaya intimal hasar bakımından üstün değildir.

Herein we present an interesting patient who initially presented with sacroiliitis and was ultimately diagnosed with stage IV lung cancer. She was initially misdiagnosed as axial spondyloarthritis. We aim to emphasize red flags in the differential diagnosis of sacroiliitis. Hypercalcemia and progressive, unbearable low back pain were supportive factors for underlying malignancy in our case.

Materials and Methods: We used the ERCC-1 antibody to measure the level of expression of ERCC-1 protein by immunohistochemical analysis from 351 patients and ERCC-1 gene copy number was evaluated by fluorescence in situ hybridization (FISH) in tumors from 81 patients. One hour after artificial PE (n = 6) and sham (n = 6) models were created, Technegas SPECT/CT ventilation and 68Ga-MAA PET/CT perfusion scan (V/Q scan) were performed. Ventilation imaging was performed first on all cases. Technegas SPECT/CT and 68Ga-MAA PET/CT images were evaluated by a nuclear medicine physician who recorded the presence, number, and location of PE on a per-lobe basis. The sensitivity, specificity, and accuracy of Technegas SPECT/CT and 68Ga-MAA PET/CT for detecting PE were calculated using a histopathological evaluation as a reference standard. T he salivary glands consist of three major glands as parotid, submandibular and sublingual gland... more T he salivary glands consist of three major glands as parotid, submandibular and sublingual glands and hundreds of minor glands, which are diffusely located in the beginning part of the respiratory system. Tumors of the salivary glands are rarely encountered. However, they appear with different histopathological characters. Different biological behaviors can be observed even in neoplasms defined by the same name. In the latest classification made by the World Health Organization (WHO) in 2017, salivary gland tumors were classified into more than 30 benign and malignant histologic subtypes [1]. This wide histopathological spectrum and significant differences in prognosis between them distinguish salivary gland tumors from other organ neoplasms. Salivary gland tumors account for approximately 3-4% of all head and neck tumors [2]. Their inci-ABSTRACT OBJECTIVE: To determine the demographic characteristics of benign and malignant salivary gland tumors in our department in the last 15 years. METHODS: In this study, the files of the patients who underwent salivary gland surgery between 2004 and 2019 in our department were analyzed retrospectively. A total of 366 patients whose diagnoses of salivary gland tumor histopathologically confirmed were included in this study. The demographic data of the patients and tumor localizations were recorded, and their relationships with histopathological results were examined. RESULTS: Of the 366 tumors, 292 (80%) of them was originated from the parotid gland, 52 (14%) from the submandibular gland, and 22 (6%) from the minor salivary gland. The male: female ratio was found as 1.2:1, and the mean age of the patients was 50.37 years. A total of 259 (70.8%) tumors were benign, and 107 (29.2%) were malignant. The most frequent benign tumor was pleomorphic adenoma, with 121 cases (46.7%). While pleomorphic adenomas constituted the majority of the benign tumors of the submandibular gland and minor salivary glands, Warthin tumor was detected more than pleomorphic adenoma in the parotid gland. The most common malignant tumor was mucoepidermoid carcinoma, with 26 cases (24.3%). CONCLUSION: The distribution of the salivary gland tumors detected in our department was found to be substantially similar to other worldwide series. Unlike the literature, we concluded that salivary gland tumors were seen slightly more in males, minor salivary gland tumors were relatively rare and Warthin tumors were more frequently detected in our region.

Histopathological and acute inflammation scores statistically decreased in both group IV and V compared with group III (P < 0.05). Although MDA levels, histopathologic and acute inflammation scores in group V were lower than group IV, the differences were not statistically significant (P > 0.05). Conclusion. Both iloprost and vitamin C decreased remote organ damage on the kidney after I/R of lower extremities in the rat model. However, vitamin C is more effective than iloprost in preventing postoperative renal dysfunction.

İn-situ teknik ile damar endotelinin iskemiden korunacağı ve intimal hasarın azalacağı öngörüsü ile prospektif bir çalışma planlandı. Gereç ve Yöntemler: CABG yapılacak ardışık 40 hastadan hazırlanan safen ven grefti çalışmaya alındı. Yirmişer hastadan oluşan iki grup oluşturuldu. Grup-1'de geleneksel "açık cilt kesiği tekniğiyle" safen ven grefti hazırlandı ve papaverinli organ banyosunda 45 dk. bekletildi. Grup-2'de in-situ teknikle safen ven grefti hazırlandı ve etrafına papaverinli spanç sarılarak 45 dk. bekletildi. Histopatolojik değerlendirme için, greftlerin distal uç kısmından 1 cm'lik safen parçası alındı. Doku örnekleri hemotoksilin-eosin ile boyanarak ışık mikroskopisinde incelendi ve histopatolojik olarak skorlandı. İstatistiksel değerlendirmede ki-kare testi kullanıldı. Bulgular: Her iki grupta da safen ven örneklerinden elde edilen patolojik hasar median skorları 2 bulundu ve istatistiksel fark saptanmadı (p = 0.987). Her iki grupta da hafif-orta derecede intimal desquamasyon saptandı. Sonuç: İn-situ teknikle safen ven grefti hazırlama geleneksel açık cilt kesiği tekniğiyle hazırlamaya intimal hasar bakımından üstün değildir.

Herein we present an interesting patient who initially presented with sacroiliitis and was ultimately diagnosed with stage IV lung cancer. She was initially misdiagnosed as axial spondyloarthritis. We aim to emphasize red flags in the differential diagnosis of sacroiliitis. Hypercalcemia and progressive, unbearable low back pain were supportive factors for underlying malignancy in our case.

One hour after artificial PE (n = 6) and sham (n = 6) models were created, Technegas SPECT/CT ventilation and 68Ga-MAA PET/CT perfusion scan (V/Q scan) were performed. Ventilation imaging was performed first on all cases. Technegas SPECT/CT and 68Ga-MAA PET/CT images were evaluated by a nuclear medicine physician who recorded the presence, number, and location of PE on a per-lobe basis. The sensitivity, specificity, and accuracy of Technegas SPECT/CT and 68Ga-MAA PET/CT for detecting PE were calculated using a histopathological evaluation as a reference standard. Tumors of the salivary glands are rarely encountered. However, they appear with different histopathological characters. Different biological behaviors can be observed even in neoplasms defined by the same name. In the latest classification made by the World Health Organization (WHO) in 2017, salivary gland tumors were classified into more than 30 benign and malignant histologic subtypes [1]. This wide histopathological spectrum and significant differences in prognosis between them distinguish salivary gland tumors from other organ neoplasms. Salivary gland tumors account for approximately 3-4% of all head and neck tumors [2]. Their inci-ABSTRACT OBJECTIVE: To determine the demographic characteristics of benign and malignant salivary gland tumors in our department in the last 15 years. METHODS: In this study, the files of the patients who underwent salivary gland surgery between 2004 and 2019 in our department were analyzed retrospectively. A total of 366 patients whose diagnoses of salivary gland tumor histopathologically confirmed were included in this study. The demographic data of the patients and tumor localizations were recorded, and their relationships with histopathological results were examined. RESULTS: Of the 366 tumors, 292 (80%) of them was originated from the parotid gland, 52 (14%) from the submandibular gland, and 22 (6%) from the minor salivary gland. The male: female ratio was found as 1.2:1, and the mean age of the patients was 50.37 years. A total of 259 (70.8%) tumors were benign, and 107 (29.2%) were malignant. The most frequent benign tumor was pleomorphic adenoma, with 121 cases (46.7%). While pleomorphic adenomas constituted the majority of the benign tumors of the submandibular gland and minor salivary glands, Warthin tumor was detected more than pleomorphic adenoma in the parotid gland. The most common malignant tumor was mucoepidermoid carcinoma, with 26 cases (24.3%). CONCLUSION: The distribution of the salivary gland tumors detected in our department was found to be substantially similar to other worldwide series. Unlike the literature, we concluded that salivary gland tumors were seen slightly more in males, minor salivary gland tumors were relatively rare and Warthin tumors were more frequently detected in our region.
