Fabry disease (FD) is an X-linked hereditary defect of glycosphingolipid storage caused by mutati... more Fabry disease (FD) is an X-linked hereditary defect of glycosphingolipid storage caused by mutations in the gene encoding the lysosomal hydrolase a-galactosidase A (GLA, a-gal A). To date, over 400 mutations causing amino acid substitutions have been described. Most of these mutations are related to the classical Fabry phenotype. Generally in lysosomal storage disorders a reliable genotype/phenotype correlation is difficult to achieve, especially in FD with its X-linked mode of inheritance. In order to predict the metabolic consequence of a given mutation, we combined in vitro enzyme activity with in vivo biomarker data. Furthermore, we used the pharmacological chaperone (PC) 1-deoxygalactonojirimycin (DGJ) as a tool to analyse the influence of individual mutations on subcellular organelle-trafficking and stability. We analysed a significant number of mutations and correlated the obtained properties to the clinical manifestation related to the mutation in order to improve our knowledge of the identity of functional relevant amino acids. Additionally, we illustrate the consequences of different mutations on plasma lyso-globotriaosylsphingosine (lyso-Gb3) accumulation in the patients' plasma, a biomarker proven to reflect the impaired substrate clearance caused by specific mutations. The established system enables us to provide information for the clinical relevance of PC therapy for a given mutant. Finally, in order to generate reliable predictions of mutant GLA defects we compared the different data sets to reveal the most coherent system to reflect the clinical situation.

Weiter-und Fortbildung Suizidale Krisen Zusammenfassung Die Intervention suizidaler Krisen in der... more Weiter-und Fortbildung Suizidale Krisen Zusammenfassung Die Intervention suizidaler Krisen in der Notfallmedizin widmet sich einer der schwierigsten seelischen Krisen.Da dieses Thema im Rettungsdienst eher vernachlässigt ist, zugleich aber, trotz erwiesen geringem Kenntnisstand, die Bedeutung psychiatrischer Notfälle hoch ist, wird ein Überblick zu psychiatrischen Krisen, Symptombildung und Suizidalität gegeben.Entstehungsbedingungen von Suizidalität, Risikofaktoren für Suizide und Hilfen zur Risikoeinschätzung durch Notärzte folgen.Als nützliche Modelle werden das "Präsuizidale Syndrom" und ein Modell des Ablaufes suizidaler Krisen dargelegt.Zur Therapie werden allgemeine und weiterführende Regeln dargelegt und vier Schritte des Umgangs mit Suizidalen beschrieben. Hinweise zur Belastungsverarbeitung für betroffene Notärzte bilden den Abschluß. Nicht so sehr die Spezifität der Krise, sondern der Schweregrad ist entscheidend ᭤ Unterscheidung nach prädominierenden Symptomen ᭤ Unspezifische Symptome

Besides pharmacological and interventional treatments a variety of non-medical therapeutic option... more Besides pharmacological and interventional treatments a variety of non-medical therapeutic options exist for migraine, which has largely been derived from behavioral therapy. Areas covered: For our update we collected available studies via PubMed searches. This review highlights that already consulting of the patient is able to reduce the frequency of migraine attacks. Relaxation techniques, especially progressive muscle relaxation, and various types of biofeedback are effective, as is the implementation of cognitive behavioral therapy. However, recent reviews also point to some existing inconsistencies and methodological limitations. The advent of modern information technology based approaches (e.g. online therapy, smartphone applications) further advanced the arsenal of behavioral treatment regimes. The combination of behavioral treatment options and the combination with pharmacotherapy lead to additive effects. In modern multidisciplinary treatment approaches, behavioral therapy is an indispensable component. Expert commentary: Behavioral treatment in prophylaxis of migraine is as effective as pharmacological treatment with additional effects when pharmacological and behavioral treatment is applied in combination. Novel treatment approaches using online technology and electronic devices offer interesting options that will spread more in the future.

Juli 2005 in London Originalien Am Vormittag des 7. Juli 2005 wurde London durch 4 terroristische Bombenattentate in U-Bahnen und einem Bus erschüttert. 56 Personen starben, mehr als 700 wurden z. T. schwer verletzt [23]. Diese Anschläge kamen unvermittelt, trafen die Londoner Rettungskräfte aber nicht unvorbereitet. In ersten fachlichen Reaktionen auf die Anschläge wurde die professionelle und koordinierte Versorgung der Opfer durch die medizinischen Einsatzkräfte beschrieben (z. B. [13, 15, 21]).

Satisfaction with sexual activity i.e. sexual satisfaction and the importance of sexuality and af... more Satisfaction with sexual activity i.e. sexual satisfaction and the importance of sexuality and affection were analysed using data from the German "Interdisciplinary Longitudinal Study of Adult Development" (ILSE). At three measurement points, 1993-1995, 1997-1998, and 2004-2006 i.e. subjects' ages of 63, 67, and 74 years, participants' reports about their affection and sexual activity were collected. The sample of completed records used for this study consisted of 194 urban non-institutionalised participants, 68% male, all living with partners. Median levels of sexual satisfaction were reported, fluctuating between the measurement points of ages 63 to 74. Between baseline, first and second follow-up no differences were found in levels of sexual satisfaction, though at measurement points age 63 and 67 women were more satisfied than men. Emergency medical personnel (EMP) are repeatedly exposed to traumatic and stressful events with p... more Emergency medical personnel (EMP) are repeatedly exposed to traumatic and stressful events with possible consequences on their mental wellbeing. Out of the group of EMP, we chose German Emergency Physicians (EP), because they represent a distinct group within the German pre-hospital emergency services. In this group, we studied the prevalence rates of posttraumatic stress disorder (PTSD), burnout and depression. We specifically focussed on the role of personality and other factors of vulnerability. Four hundred eighty-seven German EPs answered questionnaires with scales assessing probable PTSD, burnout, depression, and personality factors. Additionally, we asked for biographic, occupational and mental health information. More than 90% of the participants reported at least one traumatic event. Finance by taxes, standards set by frameworks and statutory bodies to monitor nationwide the outcome, are virtually unknown on the Continent. For the patients there are considerable differences in how they are referred to psychotherapy and receive their treatment based on guaranteed standards. Health professionals identified with the NHS have to cope with a plethora of government directives.
