To search for HTLV-1 DNA in skin biopsies of patients with CTCL. Material and Methods: A retrospe... more To search for HTLV-1 DNA in skin biopsies of patients with CTCL. Material and Methods: A retrospective study was conducted using 25 biopsies of patients with CTCL. DNA was extracted from lymphoid tissue by microdissection. A nested PCR was conducted to detect HTLV-1 genome using primers for the tax region. As negative controls, four cases of superficial perivascular dermatitis were chosen. As positive controls, five cases of T-cell leukemia/lymphoma (ATCL) were studied. Results: A positive reaction was found in 3 of 25 cases. These biopsies corresponded to a case of Mycosis Fungoides, a case of CD30 (-) T cell lymphoma and a case of lymphomatoid papulosis. Search was negative in the four cases of superficial perivascular dermatitis and positive in four cases of adult T-cell leukemia/lymphoma (ATCL). Conclusions: HTLV-1 DNA search in tissues is a useful tool recommended to study T-cell lymphomas. HTLV-1 infection only occurs in sporadic cases but may contribute to tumor aggressiveness and prognosis.

Studies on trends in sun-related behaviors have been temporally and geographically limited. Methods Individuals from an international consortium of melanoma-prone families (GenoMEL) were retrospectively asked about sunscreen use, sun exposure (time spent outside), sunburns, and sunbed use at several timepoints over their lifetime. Generalized linear mixed models were used to examine the association between these outcomes and birth cohort defined by decade spans, after adjusting for covariates. Results A total of 2407 participants from 547 families across 17 centers were analyzed. Sunscreen use increased across subsequent birth cohorts, and although the likelihood of sunburns increased until the 1950s birth cohort, it decreased thereafter. new systemic therapies for melanoma Melanoma represents a disease with high tendency to metastasi... more new systemic therapies for melanoma Melanoma represents a disease with high tendency to metastasize locally, regionally to the lymph nodes, and systemically to lungs, liver, brain and bone. The surgical treatment has been the cornerstone to treat melanoma patients, with excellent results in early stage, but with poor effectiveness in advance disease with systemic metastases. Traditional chemotherapy with cytotoxic agents has shown little impact on stage IV patients, which has motivated new investigation and innovation in this field. During last 4 years, FDA has approved new therapies for the treatment of metastatic or unresectable melanoma, mostly based in immunotherapy and signal transduction inhibitors, with very promising results. The surgeons treating these patients must be updated about these new therapies, which represent a new treatment opportunity for our patients.

El carcinoma espinocelular (CEC) es una neoplasia epitelial maligna. La mayor parte se concentra ... more El carcinoma espinocelular (CEC) es una neoplasia epitelial maligna. La mayor parte se concentra en 4 áreas: cáncer de piel no melanoma, de cabeza y cuello, esofágico y pulmonar. El riesgo de metástasis de CEC es de 0,3-3,7%. El CEC vulvar representa aproximadamente un 3-5% de los cánceres ginecológicos. Caso clínico: Mujer de 86 años con prurito genital de larga data. Evaluada en varias oportunidades, siendo tratada como Herpes genital con valaciclovir, y Liquen Escleroso y Atrófico (LEA) con corticoides tópicos y tacrolimus con mala respuesta. Consultó por intenso prurito y nuevas lesiones vulvares. Al examen físico, destacaban 2 nódulos ulcerados en región peri-clitorídea izquierda e introito. La biopsia confirmó CEC bien diferenciado infiltrante. El TAC de abdomen y pelvis descartó metástasis. Se realizó radioterapia por 7 semanas. Por persistencia de la lesión, ingresó a cuidados paliativos. Dos años después la paciente está en buenas condiciones. Discusión: El CEC representa el 95% de las neoplasias vulvares. Existen 2 tipos: CEC en mujeres jóvenes, asociado a infección por virus papiloma humano de alto riesgo y CEC en mujeres mayores en relación a LEA. El 45-61% de los CEC de vulva se asocian a LEA preexistente, por lo que se recomienda el seguimiento de pacientes portadoras de LEA cada 6 meses. Conclusión: Es importante realizar biopsias de lesiones vulvares con mala respuesta a tratamiento, sobre todo si se asocia a LEA.

It is the commonest chronic porphyria. Two types of this disease have been reported up to now: acquired (Type 1, 80%) and inherited (Type 2, 20%) an autosomal dominant pattern with low clinical penetrance. Both types are associated with haemochromatosis, alcohol abuse, estrogens, iron overload, hepatitis C virus infection, and halogenated aromatic hydrocarbons causing deficiency of the uroporphyrinogen decarboxylase enzyme in the liver. In this case report we described a 23-year-old woman with increased hair growth on the face and neck, who visited an outpatient dermatology clinic for laser hair removal due to excessive hair growth on the face and neck during the last eight years (Figures 1, 2). Report of one case Sweet's syndrome, also known as acute febrile neutrophilic dermatosis, is characterized by fever, neutrophilia, erythematous and tender skin lesions that typically show a diffuse infi ltrate of neutrophils in the upper dermis. This disorder has been associated with myeloproliferative syndromes. We report the case of a 53-year-old woman with an acute myeloid leukemia, presenting a Sweet´s syndrome. A worsening of cutaneous lesions injuries was observed when granulocyte colony stimulating factor was added to treatment.

Taxanes are a class of chemotherapeutic agents with common associated dermatologic adverse events... more Taxanes are a class of chemotherapeutic agents with common associated dermatologic adverse events, such as skin hyperpigmentation, hand-foot skin syndrome, paronychia and onycholysis. Taxane-induced scleroderma is rare. Few cases with skin findings resembling systemic sclerosis, have been reported after the administration of these agents. We report two cases with stage IV breast cancer, aged 66 and 71 years, who developed sclerodermic skin lesions in their extremities after starting treatment with placlitaxel and nabplaclitaxel respectively.

We report a 21 years old male presenting with translucent skin revealing the subcutaneous venous pattern. He had a thin face, large-appearing eyes, thin lips, thin nose, joint hypermotility and history of hip dysplasia. A vascular Ehlers Danlos Syndrome was suspected. However, the genetic study to confirm the diagnosis was not done.

Report of one case Cutaneous tuberculosis represents 1-1.5% of extrapulmonary tuberculosis, including a variety of clinical conditions. Scrofuloderma and lupus vulgaris are the most common forms. We report a 49-year-old woman who sought medical attention through tele-dermatology concerning a cervical nodule associated with suppuration and cutaneous involvement. The diagnoses of scrofuloderma and pulmonary tuberculosis were confirmed, and during her evolution she presented a coinfection with SARS-CoV-2. The possible associations between tuberculosis and COVID-19 were reviewed.

BackgroundPrimary cutaneous lymphomas (PCLs) and pseudolymphomas presenting as single pink‐red no... more BackgroundPrimary cutaneous lymphomas (PCLs) and pseudolymphomas presenting as single pink‐red nodules/tumors are highly unspecific and include a wide differential diagnosis.ObjectiveTo describe the dermoscopic characteristics of PCL/pseudolymphoma.MethodsIn this retrospective, case‐control study, we evaluated the dermoscopic features of patients with solitary PCL/pseudolymphoma tumors and compared them to a control group of non‐lymphomatous, nonpigmented, solitary tumors (e.g., basal cell carcinoma, amelanotic melanoma, etc).ResultsWe included 14 patients with PCL/pseudolymphomas and 35 controls. T‐cell and B‐cell lymphoma proportions were 28.6% (n = 4) and 71.4% (n = 10), respectively. Although its importance in malignant melanoma is debated, vitamin D does appear to suppress the growth of the non-melanoma skin tumours basal cell carcinoma and squamous cell carcinoma. This study from Santiago, Chile, and Boston, USA, looked at whether the vitamin D receptor in melanoma cells was lost in comparison with mole cells. Both are formed from cells called melanocytes; the difference is that the melanocytes are malignant in melanoma, benign in moles. Melanoma samples were taken from 54 patients and compared with 55 samples of moles from patients with no history of melanoma or abnormal ("dysplastic") moles. The melanoma samples were looked at down the microscope and classified according to standard measures such as thickness, depth of invasion into the skin, ulceration, etc. They were specially stained to show up the vitamin D receptor. The intensity of staining, and the numbers of cells that showed vitamin D receptor in both the nucleus and cytoplasm, were assessed. There was significant loss of the vitamin D receptor in the cytoplasm, though not the nucleus, of melanoma cells in comparison to mole cells. This was associated with the thickness and staging of the melanoma in the skin: the thicker the tumour, the more vitamin D receptor was lost. Similar findings have been reported in carcinomas of the prostate, stomach and pancreas. The degree of loss of vitamin D receptor may help decide which patients might benefit from taking vitamin D.

Rosai-Dorfman disease is a rare histiocytic disorder shown to have gene mutations that activate t... more Rosai-Dorfman disease is a rare histiocytic disorder shown to have gene mutations that activate the MAPK/ERK pathway in at least one-third of cases. Most patients with Rosai-Dorfman disease present with bulky lymphadenopathy or extranodal disease, but rarely Rosai-Dorfman disease is detected concomitantly with lymphoma in the same biopsy specimen. The underlying molecular mechanisms of focal Rosai-Dorfman disease occurring in the setting of lymphoma have not been investigated. We report 12 cases of Rosai-Dorfman disease and lymphoma involving the same anatomic site. There were five men and seven women (age, 23 to 77 years) who underwent lymph node (n = 11) or skin (n = 1) biopsy; the lymphomas included nodular lymphocyte predominant Hodgkin lymphoma (n = 6), classical Hodgkin lymphoma (n = 4), small lymphocytic lymphoma (n = 1) and extranodal marginal zone lymphoma (n = 1). Vitamin D (VD) is a secosteroid hormone that is mainly synthesized in the skin upon exposure to U... more Vitamin D (VD) is a secosteroid hormone that is mainly synthesized in the skin upon exposure to UVB radiation. VD is widely known for its role in calcium metabolism; however, multiple endocrine, paracrine and autocrine functions of VD have been described, including a prominent role on carcinogenesis. In recent years, multiple associations between VD deficiency and different types of cancer have been described, supported by evidence of anti-proliferative, anti-angiogenic, pro-apoptotic, cell-differentiating and anti-invasive effects of this hormone. An immunomodulatory role of VD associated to cancer microenvironment has also been suggested. Regarding skin cancer, it has been shown that VD inhibits tumor development in basal cell carcinoma, squamous cell carcinoma, and melanoma in vitro. Some studies have suggested that lower VD levels may be a risk factor for skin cancer, while others have shown the opposite; there is also preliminary evidence on the role of VD supplementation for the prevention of melanoma in vivo. In this review, we explore the mechanisms of VD effects on carcinogenesis and the available scientific evidence of the interplay between VD and the genesis of both non-melanoma and melanoma skin cancer.

However, our study has a limitation in that any change in smoking status after registration has not been reflected. In addition, association does not demonstrate cause and effect; therefore, further investigation is needed.

Findings were consistent with intravascular large B-cell lymphoma (IVLBCL). With no systemic commitment. Treatment was performed with 6 chemotherapy sessions (rituximab, vincristine, cyclophosphamide, doxorubicin, R-CHOP prednisone), showing regression of lesions.

We report a case of an 83 years old patient, with chronical renal insufficiency in hemodialysis, ... more We report a case of an 83 years old patient, with chronical renal insufficiency in hemodialysis, who has had from the last year a bilobulated and ulcerated tumor on the lower lip. The lesion was excised and the biopsy showed epithelial tumor cells nest with atypia, high mitotic index and necrotic areas. Inmunohistochemical studies were positive for specific neuroenolasa and neurofilaments, and it was consistent with the diagnosis of Merkel Cell Carcinoma. The patient refused other therapies and he died one year after of his basal disease. We present a bibliographic review of this infrequently pathology, with only 13 cases reported ant the medical literature.

BackgroundPrimary cutaneous lymphomas (PCLs) and pseudolymphomas presenting as single pink‐red no... more BackgroundPrimary cutaneous lymphomas (PCLs) and pseudolymphomas presenting as single pink‐red nodules/tumors are highly unspecific and include a wide differential diagnosis.ObjectiveTo describe the dermoscopic characteristics of PCL/pseudolymphoma.MethodsIn this retrospective, case‐control study, we evaluated the dermoscopic features of patients with solitary PCL/pseudolymphoma tumors and compared them to a control group of non‐lymphomatous, nonpigmented, solitary tumors (e.g., basal cell carcinoma, amelanotic melanoma, etc).ResultsWe included 14 patients with PCL/pseudolymphomas and 35 controls. T‐cell and B‐cell lymphoma proportions were 28.6% (n = 4) and 71.4% (n = 10), respectively. Compared to controls, most lymphomas presented dermoscopically with orange color (71.4% vs. 14.2%, P < 0.001), follicular plugs (85% vs. 2.8%, P < 0.001), and as organized lesions (85% vs. 31.4%, P = 0.001). Although its importance in malignant melanoma is debated, vitamin D does appear to suppress the growth of the non-melanoma skin tumours basal cell carcinoma and squamous cell carcinoma. This study from Santiago, Chile, and Boston, USA, looked at whether the vitamin D receptor in melanoma cells was lost in comparison with mole cells. Both are formed from cells called melanocytes; the difference is that the melanocytes are malignant in melanoma, benign in moles. Melanoma samples were taken from 54 patients and compared with 55 samples of moles from patients with no history of melanoma or abnormal ("dysplastic") moles. The melanoma samples were looked at down the microscope and classified according to standard measures such as thickness, depth of invasion into the skin, ulceration, etc. They were specially stained to show up the vitamin D receptor. The intensity of staining, and the numbers of cells that showed vitamin D receptor in both the nucleus and cytoplasm, were assessed. There was significant loss of the vitamin D receptor in the cytoplasm, though not the nucleus, of melanoma cells in comparison to mole cells. This was associated with the thickness and staging of the melanoma in the skin: the thicker the tumour, the more vitamin D receptor was lost. Similar findings have been reported in carcinomas of the prostate, stomach and pancreas. The degree of loss of vitamin D receptor may help decide which patients might benefit from taking vitamin D.

Rosai-Dorfman disease is a rare histiocytic disorder shown to have gene mutations that activate t... more Rosai-Dorfman disease is a rare histiocytic disorder shown to have gene mutations that activate the MAPK/ERK pathway in at least one-third of cases. Most patients with Rosai-Dorfman disease present with bulky lymphadenopathy or extranodal disease, but rarely Rosai-Dorfman disease is detected concomitantly with lymphoma in the same biopsy specimen. The underlying molecular mechanisms of focal Rosai-Dorfman disease occurring in the setting of lymphoma have not been investigated. We report 12 cases of Rosai-Dorfman disease and lymphoma involving the same anatomic site. There were five men and seven women (age, 23 to 77 years) who underwent lymph node (n = 11) or skin (n = 1) biopsy; the lymphomas included nodular lymphocyte predominant Hodgkin lymphoma (n = 6), classical Hodgkin lymphoma (n = 4), small lymphocytic lymphoma (n = 1) and extranodal marginal zone lymphoma (n = 1). Vitamin D (VD) is a secosteroid hormone that is mainly synthesized in the skin upon exposure to U... more Vitamin D (VD) is a secosteroid hormone that is mainly synthesized in the skin upon exposure to UVB radiation. VD is widely known for its role in calcium metabolism; however, multiple endocrine, paracrine and autocrine functions of VD have been described, including a prominent role on carcinogenesis. In recent years, multiple associations between VD deficiency and different types of cancer have been described, supported by evidence of anti-proliferative, anti-angiogenic, pro-apoptotic, cell-differentiating and anti-invasive effects of this hormone. An immunomodulatory role of VD associated to cancer microenvironment has also been suggested. Regarding skin cancer, it has been shown that VD inhibits tumor development in basal cell carcinoma, squamous cell carcinoma, and melanoma in vitro. Some studies have suggested that lower VD levels may be a risk factor for skin cancer, while others have shown the opposite; there is also preliminary evidence on the role of VD supplementation for the prevention of melanoma in vivo. In this review, we explore the mechanisms of VD effects on carcinogenesis and the available scientific evidence of the interplay between VD and the genesis of both non-melanoma and melanoma skin cancer.

However, our study has a limitation in that any change in smoking status after registration has not been reflected. In addition, association does not demonstrate cause and effect; therefore, further investigation is needed.

Findings were consistent with intravascular large B-cell lymphoma (IVLBCL). With no systemic commitment. Treatment was performed with 6 chemotherapy sessions (rituximab, vincristine, cyclophosphamide, doxorubicin, R-CHOP prednisone), showing regression of lesions.

We report a case of an 83 years old patient, with chronical renal insufficiency in hemodialysis, ... more We report a case of an 83 years old patient, with chronical renal insufficiency in hemodialysis, who has had from the last year a bilobulated and ulcerated tumor on the lower lip. The lesion was excised and the biopsy showed epithelial tumor cells nest with atypia, high mitotic index and necrotic areas. Inmunohistochemical studies were positive for specific neuroenolasa and neurofilaments, and it was consistent with the diagnosis of Merkel Cell Carcinoma. The patient refused other therapies and he died one year after of his basal disease. We present a bibliographic review of this infrequently pathology, with only 13 cases reported ant the medical literature.
