Metal on Metal hip resurfacing (MoM HR) can be an effective operation for the young arthritic hip... more Metal on Metal hip resurfacing (MoM HR) can be an effective operation for the young arthritic hip population. However, errors in cup orientation have been associated with increased wear, circulating blood metal ions, and soft tissue abnormalities that can lead to premature failure of the bearing surface and subsequent revision surgery. While image free computer guidance has been shown to increase surgical accuracy in total hip arthroplasty, the role of image based technology in MoM HR is unclear. In this study, we compared the accuracy of cup orientation in MoM HR performed by either freehand technique or CT based navigation. Seventy five patients (81 hips) underwent either freehand (n=42) or navigation (n=39) surgery, both requiring a three dimensional (3D) CT surgical plan. Surgery was conducted by hip specialists blind to the method of cup implantation until the operation. Deviation in inclination and version from the planned orientation, as well as, number of cups within a 10° safe zone and 5° optimal zone of the target position was calculated using post operative 3D CT analysis. Error in inclination was significantly reduced with navigation compared to freehand technique (4° vs 6°, p=0.02). We could not detect a difference between the two groups for version error (5° vs 7°, p=0.06). There was a significantly greater number of hips within a 10° (87% vs 67%, p=0.04) and 5° (50% vs 20%, p=0.06) safe zone when navigated. Image based navigation can substantially improve accuracy in cup orientation. The results of our freehand group appear better than historic controls, suggesting the use of a 3D plan may help to reduce technical error and improve the learning curve in this technically demanding procedure. We advocate the use of image based navigation in MoM hip resurfacing arthroplasty.

The purpose of this study was to investigate the status and epidemiology of Intestinal Parasites ... more The purpose of this study was to investigate the status and epidemiology of Intestinal Parasites in Iran. The information was driven from an extensive Health Survey which was done by the Ministry of Health and Medical Education, deputy of Research Affairs in 1990-92. Sampling fraction was 1 per 1000 of individuals aged between 2 and 69, the sampling method was cluster sampling and each cluster consisted of 7 families. Formal-ether was the method of finding parasites which included: Oxior, Ascariasis, Giardiasis, Entamoeba-histolytica, Tinea, Strongyloidiasis, Ancylostoma, and Trichocephaliasis. The highest prevalence rate belonged to Giardiasis with 14.4% and the lowest one belonged to Tinea and Ancylostoma with 0.2%. The prevalence rate in rural area was significantly lower than urban area (p<0.0001).

Background: Pulmonary carcinoid tumor is a low-grade malignant neoplasm comprised of neuroendocri... more Background: Pulmonary carcinoid tumor is a low-grade malignant neoplasm comprised of neuroendocrine cells. The aim of this study was to determine the clinical features; radiological and bronchoscopic findings, as well as treatment in patients. The hospital records of 21 patients with pulmonary carciniod tumor, who admitted to NRITLD between 1995 and 2001, were evaluated. Results: 19 patients had typical and 2 had atypical carcinoid tumor. Male to female ratio was 1.1. The mean age ± SD was 39.33±16.31.Cough (80.9%) was the most common presenting symptom. 16 patients had abnormal chest radiography. The diagnosis was made using bronchoscopy and biopsy. Most of the tumors arose in main bronchi. Surgery with or without endobronchial laser therapy was the treatment of our patients. Lobectomy (47.3%) was the most common procedure. surgery with or without laser therapy has a favorable outcome in pulmonary carcinoid tumors.

Introduction: Skin graft is the standard therapeutic technique in patients with deep ulcers, but ... more Introduction: Skin graft is the standard therapeutic technique in patients with deep ulcers, but like every surgical procedure, it may present some complications. Although several modern dressings are available to enhance comfort of donor site, the use of techniques that accelerate wound healing may enhance patient's satisfaction. Low level laser therapy (LLLT) has been used in several medical fields, especially for wound healing, but it may take several months for large ulcers treated with laser to heal completely. Methods: Nine patients with bilateral similar grade 3 burn ulcers in both hands or both feet were selected as candidates for split-thickness skin graft (STSG). One side was selected for laser irradiation and the other side as control, randomly. Laser was irradiated every day for 7 days with red 655 nm light, 150 mW, 2 J/cm 2 at the bed of the ulcer and with infra-red 808 nm light, 200 mW for the margins. Results: The rate of wound dehiscence after skin graft surgery was significantly lower in laser treated group in comparison to control group which received only classic dressing (P = 0.019). The results showed LLLT to be a safe effective method which improves graft survival and wound healing process and decreases the rate of wound dehiscence in patients with deep burn ulcers.

Tobacco smoke is known for releasing metals in indoor air of waterpipe/cigarette cafe ´s. However... more Tobacco smoke is known for releasing metals in indoor air of waterpipe/cigarette cafe ´s. However, the worker exposure to metals, and its association with oxidative stress in these cafe ´s are still unclear. To this end, 54 workers and 38 customers from waterpipe/cigarette cafe ´s (the exposed group), 30 workers from non-smoking cafe ´s (the control group 1 (CG_1)) and 32 individuals from the general population (the control group 2 (CG_2)) were selected and toenails samples were then taken from them. Our findings revealed a significant difference in terms of toenail-bounded metal levels between the exposure and control groups (CG_1 and CG_2) (Mann-Whitney U test, P value \ 0.05). This study has also indicated that ''type of tobacco'' could be considered as a predictor for toenail-bounded heavy metals. Furthermore, our research's results suggest that toenail-bounded heavy metals are positively and significantly correlated with urinary levels of 8-hydroxy-2 0deoxyguanosine (8-OHdG, as a biomarker for the degradation of deoxyribonucleic acid (DNA) oxidative stress). Therefore, it can be concluded that workers of waterpipe/cigarette cafe ´s are at high risks of adverse health of DNA oxidative degradation.

Materials and methods: It was a qualitative study, and data were collected using in-depth interviews with 20 people (10 men and 10 women) with cultural and religious specialties (clergy). In total, twenty interviews were conducted (mean duration = 45 min) using a semi-structured guide consisting of open-ended questions. All recordings were transcribed verbatim in Persian. All items were extracted based on the participants' responses and related literature. Methods: Ancillary study of an international, multicenter, prospective cohort study (LUNG SAFE study[1]). Patients meeting ARDS criteria (Berlin definition) on day 1 or 2 of acute hypoxemic respiratory failure onset were included in the study and categorized as having "common" risk factors or not. Results: Among the 2813 patients presenting ARDS in the first 48 h, 266 patients (9.4 %) had no ARDS risk factor identified at admission. Table shows the final ARDS risk factor identified in patients with or without initial risk factor identified. The patients with no risk factor were older, had more frequent previously known chronic diseases and presented with less severe SOFA (8.7 ± 3.9 vs 9.5 ± 4.1, p < 0.001) and non-pulmonary (5.4 ± 3.9 vs 6.3 ± 4.1, p < 0.001) SOFA scores. ICU mortality was lower in ARDS patients with no risk factor than in others (28.6 % vs 34.9 %, p = 0.047), but in-hospital mortality was not (35.7 % vs 39.8 %, p = 0.20). The lack of ARDS risk factor was not associated with hospital mortality (adjusted OR = 0.86 [0.65-1.13], p = 0.29). In the subgroup of patients with no ARDS risk factor, age, SOFA, concomitant heart failure, and administration of steroids within 72 hours of ARDS onset were associated with hospital mortality (Table ). Conclusions: Almost ten percent of patients with ARDS had no risk factor identified and exhibit a different clinical phenotype than others. Future research aimed at studying management strategies in this subgroup of patients is warranted.

OBJETIVO: Estimar a prevalência do tabagismo e sua associação com o uso de outras drogas entre es... more OBJETIVO: Estimar a prevalência do tabagismo e sua associação com o uso de outras drogas entre escolares, do ensino fundamental e médio, do Distrito Federal (DF). MÉTODOS: Estudo epidemiológico, tendo como população de referência escolares do DF. Nossa amostra consistiu de 2.661 alunos com idades entre 9 e 19 anos de todas as séries do ensino fundamental II e do ensino médio que responderam a um questionário padrão. Os resultados foram analisados por gênero e tipo de rede escolar. RESULTADOS: A prevalência do tabagismo entre escolares do DF foi de 10,5%, sendo observada uma associação entre o uso do cigarro e o uso de álcool e outras drogas. CONCLUSÕES: O tabagismo é uma porta de entrada para o uso de outras drogas.

In this study, an electromagnetic tracking device was used to measure the three-dimensional scapula motion of nine patients with Bayley—Walker (B—W) reverse anatomy joint replacements. The data were then compared to scapula kinematics of 12 normal subjects with no known shoulder pathology. The scapula kinematics for each subject was determined and a regression analysis to find the mean scapula lateral rotation (SLR) for B—W and normal subjects was performed. The regression model which showed the highest change compared to normal was identified and was used in the biomechanical shoulder model to predict the glenohumeral joint contact force. Introduction: Concurrent use of tobacco products (hookah and cigarette) could increase the risks ... more Introduction: Concurrent use of tobacco products (hookah and cigarette) could increase the risks and complications associated with smoking and nicotine addiction. The interaction between these two simultaneous intakes can cause adverse effects on smokers. Precise information about dosage and side effects of concurrent use of both tobacco products has been less focused by experts. In this study, the concomitant use of hookah and cigarette and their related factors were examined. Materials and Methods: In this cross sectional study, different geographic areas were selected and random cluster sampling was done on people over 15 years old. The questionnaire of Global Adult Tobacco Survey (GYTS) was completed and analyzed. Results: From 1830 participants, 76 (4.2%) consumed both hookah and cigarette, 243 cases (13.3%) consumed hookah alone, and 120 (6/6%) smoked cigarette only. Also, about 86.8% of concurrent users were male, and 31.2% were female (p Conclusion: The percentage of hookah smokers shows increasing use in the community. Dual smokers were older than hookah smokers and more frequently were male. Dual smokers started hookah earlier than cigarette. This could support the theory that use of hookah is grounds for starting cigarette smoking. It seems that experts and policy makers of tobacco control should take into account this type of smoking.

This report described a 2-year-old boy who was presented with severe respiratory distress and str... more This report described a 2-year-old boy who was presented with severe respiratory distress and stridor. Bronchoscopy and CT revealed a mass in the left anterolateral tracheal wall and histopathology showed a tracheal inflammatory myofibroblastic tumor. Initial removal by rigid bronchoscopy resulted in prompt recurrence of the tumor. Therefore, he underwent tracheal surgical resection. A bronchoscopy at 12 months after surgery did not show any recurrence sign.

This report described a 2-year-old boy who was presented with severe respiratory distress and str... more This report described a 2-year-old boy who was presented with severe respiratory distress and stridor. Bronchoscopy and CT revealed a mass in the left anterolateral tracheal wall and histopathology showed a tracheal inflammatory myofibroblastic tumor. Initial removal by rigid bronchoscopy resulted in prompt recurrence of the tumor. Therefore, he underwent tracheal surgical resection. A bronchoscopy at 12 months after surgery did not show any recurrence sign.
