Some previous studies focused on the effects of adding sufentanil and/or midazolam to bupivacaine and compared each with using bupivacaine alone. However, there has been no study to assess the effects of using sufentanil and midazolam in combination with bupivacaine. The aim of this study was to evaluate and compare properties (time of achievement/recovery of sensory/motor blocks; and time to request opium), complications (nausea, vomiting, shivering and hypotension), and neonatal first minute Apgar score with and without the addition of midazolam (M) or sufentanil (S) to bupivacaine (B) through intrathecal injection for spinal anesthesia, after the cesarean section. In this double blind randomized clinical trial participants were randomly allocated to three equal groups: Group B (2.5 cc of bupivacaine 0.5% + 1 cc normal saline 0.9%), Group BM (2.5 cc of bupivacaine + 0.02 mg/kg midazolam) and Group BS (2.5 cc of bupivacaine 0.5% + 0.7 cc normal saline 0.9% + 1.5 µg of sufentanil, 0.3 cc). We used analysis of variance (ANOVA), post hoc test with Bonferroni adjustment, and chi-square test for statistical analysis; the analyses were performed using the SPSS-16 software. Given a significant level of 0.05, overall and pair-wise comparisons were made. Seventy-five females participated in the study with no significant age difference (mean ± standard deviation (SD): 28.60 ± 6.06, 28.12 ± 5.29 and 28.76 ± 3.97 year; P = 0.9). Except for "time to motor block recovery" (P = 0.057), the overall differences among the three groups was significant in terms of "time to sensory/motor block" (P < 0.001), "time to sensory block recovery" (P < 0.001), and "time to request opium" (P < 0.001). In all pair-wise comparisons there was no significant difference between the BM and BS group, except for "time to request opium", which was longer in the BS group (P < 0.001). The occurrence of nausea (P = 0.02), postoperative shivering (P = 0.01) and hypotension (P < 0.001) were significantly different between the groups, unlike vomiting, where the difference was not significant (P = 0.2). All neonates had an Apgar score of nine. The findings showed that adding sufentanil or midazolam to bupivacaine shortens the onset of spinal anesthesia and increases the time duration of anesthesia; however it does not change the motor block recovery time. Adding sufentanil delays the first request for narcotic analgesics while adding midazolam leads to a decrease in nausea and hypotension. Adding sufentanil or midazolam does not have any deleterious effect on infants' Apgar scores. However, increases shivering in patients.

Primary axillary hyperhidrosis (PAH) is a chronic idiopathic disorder causing major stress in pat... more Primary axillary hyperhidrosis (PAH) is a chronic idiopathic disorder causing major stress in patients. Among the common therapies for PAH, only surgical interventions have proven feasible as a permanent solution. The aim of this study was to evaluate the efficacy and safety of fractional microneedle radiofrequency (FMR) as an alternative permanent treatment for PAH with long-term follow-up. This was a single-blind, sham-controlled comparative study. Twenty-five patients with severe PAH were provided three treatments of FMR at 3-week intervals (the treatment group), and a control group was provided the sham treatment. Clinical efficacy was evaluated using the hyperhidrosis disease severity scale (HDSS) at baseline and the end of the study, as well as during the 1 year follow-up phase. HDSS demonstrated significant improvement after treatment in the treatment group compared to the sham control. The mean (±standard deviation) of HDSS in the group being treated with radiofrequency was 2.50 (±0.88) after 1 year follow-up, and that of the control group was 3.38 (±0.49; P < 0.001). Follow-up results show that there were 10 patients (41.6%) with no relapse and 11 patients (45.9%) with relapse after 1 year. There was a significant correlation between HDSS changes in relapse and body mass index (BMI) (P = 0.03). Treatment of PAH with FMR is a safe and noninvasive procedure with a positive therapeutic effect on HDSS. It is recommended, however, that sessions of FMR be repeated after 1 year, particularly in overweight patients with high BMIs. IRCT2013111915455N1. Level II-1.

Background: Stuttering is potentially a socio communicational problem, which has negative effects... more Background: Stuttering is potentially a socio communicational problem, which has negative effects on individuals' mental and emotional health and other aspects of life. Recognizing the quality of life (QOL) dimensions in adults who stutter can be useful in treatment planning and preventing later mental, emotional and social disorders. Objectives: This study aimed to investigate the QOL and its relationship with stuttering severity in adults who stutter. Patients and Methods: This analytic-descriptive study was performed on 78 adults (61 males and 17 females) who stuttered and 78 normal control cases in Mashhad City, Iran. Short-form 36 (SF-36) questionnaire of QOL was used to collect data and the percentage of syllables stuttered (%SS) was determined in a spontaneous speech sample as a frequency measurement tool. Mann-Whitney test was used to compare data between the groups and ANOVA test was also used to evaluate the correlation between stuttering severity and QOL dimensions. Results: This study showed a significant difference between the groups in mean scores of QOL, general and emotional health, social and physical functions, vitality and role limitations made by physical and emotional problems (P < 0.05). No significant difference was found in pain between the groups and there was no significant relation between %SS and dimensions of QOL (P > 0.05). Conclusions: Stuttering has negative effects on different dimensions of QOL among individuals. Thus, it is necessary to pay special attention to its prevention and treatment before it is developed into a completed and progressive condition.

Strengthening training (ST) and whole body vibration training (WBV) alone may improve symptoms of... more Strengthening training (ST) and whole body vibration training (WBV) alone may improve symptoms of osteoarthritis of the knee. In this study, we investigated the effect of adding WBV training to quadriceps and hamstring muscles strengthening training on functional activity, pain, quality of life and muscle strength in patients with knee osteoarthritis. 28 volunteers were randomly allocated to two groups; 1) quadriceps and hamstring muscles strengthening training (ST group, 13 patients) and 2) quadriceps and hamstring muscles strengthening training along with WBV training (ST + WBV group, 15 patients). The treatment protocol for both groups involved 3 sessions per week for 8 weeks. All measurements were performed before and after intervention. The measurements included: pain by means of a visual analogue scale (VAS), quality of life by means of the WOMAC scale, functional activity by the 2 min walking test (2MWT), time up & go test (TUGT) and 50-foot walking test (50FWT) and the muscle peak torque (MPT), total work (TW) and muscle power (MP) as muscle performance of quadriceps and hamstring muscles by an Isokinetic Biodex machine. After intervention, the comparison of mean changes between two groups showed improvement in the WBV + ST group in terms of 2MWT, MPT, TW and MP variables (P < 0.05). However, no significant difference was found between the experimental groups in term of pain, quality of life, TUGT and 50FWT. These results suggest that adding whole body vibration training to strengthening training may provide better treatment effects for patients with knee osteoarthritis.

Background: Hip fracture is very common orthopaedic fracture in elderly. The present study invest... more Background: Hip fracture is very common orthopaedic fracture in elderly. The present study investigates the outcome after surgical treatment. Methods: The hip fractures of over 65-year old patients that had undergone surgery in a 5-year period were retrospectively studied in a hospital in Semnan, Iran. The information of 259 patients (120 male, 139 women) were collected including demographic data, medical history, history of surgery (type of anesthesia, technique of surgery), length of hospitalization before and after surgery, and death in hospital. Results: From all the operated cases, 174 (67.2%) received spinal, 68% (26.3), epidural and 17% (6.6) general anesthesia. 185 pateints (71.4%) had no complication and 74% (28.6) had at least one complication. 12 patients (n = 6.4) died while in hospital. 56 cases (21.6%) were walking with no problem; 32 (12.4%) required walking and 9 patients (3.5%) were not able to walk. ABSTRACT Dysmenorrhea or painful menstruation is one of the most common problems of women. Using ... more ABSTRACT Dysmenorrhea or painful menstruation is one of the most common problems of women. Using systematic review and meta‑analysis, this study aimed to determine the effect of vitamin E on ameliorating the intensity of pain of primary dysmenorrhea. Available databases comprising PubMed, Google Scholar, ISI, Science Direct, SID, Iran Medex, and Magiran were searched to find relevant papers. Clinical trials with the aim of the effect of vitamin E on pain reduction of primary dysmenorrhea were selected. Two independent reviewers assessed the quality of studies and extracted the necessary data. Three studies were eligible for meta‑analysis. The meta‑analysis of these studies showed that vitamin E (with or without mefenamic acid), could reduce the intensity of pain about 7% (95% CI: 2-30%). Based on these studies, vitamin E can reduce the intensity of primary dysmenorrhea.

This study was carried out to use multiple imputation (MI) in order to correct for the potential ... more This study was carried out to use multiple imputation (MI) in order to correct for the potential nonresponse bias in measurements related to variable fasting blood glucose (FBS) in non-communicable disease risk factors survey conducted in Iran in 2007. Five multiple imputation methods as bootstrap expectation maximization, multivariate normal regression, univariate linear regression, MI by chained equation, and predictive mean matching were applied to impute variable fasting blood sugar. To make FBS consistent with normality assumption natural logarithm (Ln) and Box-Cox (BC) transformations were used prior to imputation. Measurements from which we intended to remove nonresponse bias included mean of FBS and percentage of those with high FBS. For mean of FBS results didn&amp;amp;amp;amp;amp;amp;amp;#39;t considerably change after applying MI methods. Regarding the prevalence of high blood sugar all methods on original scale tended to increase the estimates except for predictive mean matching that along with all methods on BC or Ln transformed data didn&amp;amp;amp;amp;amp;amp;amp;#39;t change the results. FBS-related measurements didn&amp;amp;amp;amp;amp;amp;amp;#39;t change after applying different MI methods. It seems that nonresponse bias was not an important challenge regarding these measurements. However use of MI methods resulted in more efficient estimations. Further studies are encouraged on accuracy of MI methods in these settings.
