The long-term outcome of patients with 'IgA' nephropathy needs to be studied incl... more The long-term outcome of patients with 'IgA' nephropathy needs to be studied include the clinical spectrum and outcome. We studied 50 patients with biopsy-proven 'IgA' nephropathy (IF 2+) who had urinary abnormalities at the time of presentation, we attempt to optimize the clinical spectrum of the patients with 'IgA' nephropathy and analyze the short-term outcome of the patients including progression to 'ESRD'. The mean age of the patient was 45.63±3.56 years (IQR 32-48 years) odds ratio was 4.86 years.The majority of the patients were in the third decade with male preponderance p=0.023 statistically significant with age group of the population. Renal biopsy showed focal and diffuse mesangial proliferative glomerulonephritis in (22%) and (8%) respectively. Chronic glomerulosclerosis was seen in (58%). Crecentric GN in (28%). IFTA >50% was seen in 44% of patients. 'IgAN' is a very common primary glomerulopathy conventionally described as a slowly progressive disease eventually leading to 'ESRD' in 30–40% patients. However, it manifests more aggressively in Indian patients with a 10-year renal survival of only 35%, which is lower than the other studies.

Objectives: To study the problem of nonadherence to prescription drugs in elderly patients attend... more Objectives: To study the problem of nonadherence to prescription drugs in elderly patients attending medical OPD.To study the proportion of elderly of patients with uncontrolled hypertension and Diabetes among the patients attending the medical OPD.To estimate the problem of nonadherence in the proportion of elderly patients with uncontrolled Diabetes and hypertension. Methods:218 patients aged over 65 years were selected by computer randomisation from the pool of patients attending the medical OPD.They underwent comprehensive geriatric evaluation with special emphasis on drug adherence and the control of diabetes and hypertension was checked and compared with international standards. The data was analysed using SPSS software and the adherence rate in the uncontrolled diabetes, uncontrolled hypertension and uncontrolled diabetes and hypertension was compared with the general geriatric population. Results: 131 out of the 218 were male patients and the remaining were femalepatients.63patients out of 218 patients had uncontrolled hypertension (28.9%).42patients out of 218patients had uncontrolled blood sugars (19.2%).30patients out of 218patients were nonadherent to their medications.13.7%).18patients out of the 63 poorly controlled hypertensives were nonadherent to their medications( 11.34%).14 patients out of the 42 of poorly controlled diabetics were non adherent to their medications ( 33.33%).17 patients were having poorly controlled diabetes and hypertension and 8 of them were nonadherent to their medications(44%)On analysis of the data a significant proportion of the patients with uncontrolled diabetes, hypertension and uncontrolled diabetes and hypertension were nonadherent to the prescribed medications (11.34%, 33.33% and 44% respectively). Conclusion: Nonadherence to prescription drugs is prevalent in elderly patients attending medical OPD.The problem of nonadherence is significantly more in the proportion of patients with poorly controlled hypertension and Diabetes.

Using survival status at T months as the primary endpoint in a phase II trial with interim looks ... more Using survival status at T months as the primary endpoint in a phase II trial with interim looks is sometimes challenging. The primary issue is censoring at the time of the interim analysis – not all subjects are followed for T months. The simple proportion of all subjects surviving the required time is a biased estimate of the survival probability if some have incomplete follow-up. However, suspending accrual to allow all the subjects to be followed for the required length of time is impractical. Case and Morgan (2003) developed a two-stage design for evaluating survival probabilities with a planned interim analysis without interrupting the trial. In this report, the authors review the design, and describe development of analysis tools for calculating the test statistic for the interim analysis. Both a SAS macro and R-code were developed. The SAS macro is convenient if SAS is used as the analysis platform for the trial. The R-code is also easy to use if R is the preferred analysis tool. Development of both tools allowed for cross-verification of the programs. A cancer trial example is used to demonstrate the procedure and discuss practical issues related to rapid accrual. The SAS macro is available upon request.

AIM AND OBJECTIVES : The aim and objectives of the study is to know the etiological profile and i... more AIM AND OBJECTIVES : The aim and objectives of the study is to know the etiological profile and infection rate of ventilator associated pneumonia(VAP) in ICU over one year period. MATERIALS AND METHODS : A hospital based study was carried out in the ICU in ESICMC & PGIMSR Bangalore. This study was conducted from October 2011 to August 2012. The study was carried out amongst inpatients admitted to ICU and who were on ventilator.VAP was defined according to CDC criteria &endotracheal aspirates were collected from eligible patients on ventilator. Identification of pathogens were performed as per guidelines. The data was analysed for determining VAP infection rate. RESULTS: A total of 173 patients were on ventilator in ICU. Among them 57 developed ventilator Associated pneumonia. VAP infection rate was 23%. Acinetobacter baumannii& Pseudomonas aeruginosa were commonest in VAP.
