Karawang Regency is one of the center of rice production in West Java. The increasing development... more Karawang Regency is one of the center of rice production in West Java. The increasing development of infrastructure due to the proximity of this region leading to high conversion of paddy field to the other land use, which implies a decrease in the ability of rice production, which in turn decrease the contribution of the region to regional and national food security. Areas along the road infrastructure has the potency to suffer a more dynamic land use change compared to the hinterland areas. The purpose of this study was to identify changes in land use in last 11 years and the suitability of land for paddy field of the converted land along the highways and national roads, as well as implications for rice production. Paddy field delineation was done using IKONOS imagery from rhe work of the Ministry of Agriculture, while the other land use were identified using Landsat imagery. Land suitability analysis for rice field was done using the Automated Land Evaluation System (ALES), using data from the previous soil survey. The results of the analysis are interpreted in terms of the effect of road infrastructure on land use change and the potential reduction in rice production in the various classes of land suitability. The results show that in terms of the growth of settlements, there were effect of the road as far as 3 km from the road. In terms of paddy field conversion, the area on the edge of the road were converted less than the hinterland area. The converted land near the road mostly have land suitability classes of S3 (marginally suitable).

Pemetakan ini dapat dilakukan dengan menggunakan tehnik pengindaraan jauh, proses citra satelit, atau menelusuri dan merekam jalur dengan menggunakan alat penerima sinyal GPS. Kedua, alat penerima sinyal GPS digunakan untuk mengukur luas tanam dan luas panen. Untuk mendapatkan luas tanam dapat diukur areal tanamnya atau mengukur areal yang belum ditanami, sehingga berdasarkan data spasial luas baku sawah akan diperoleh luas tanam dengan cara mengurangkan selisihnya. Ketiga, diukur luas panen dengan cara merekam areal panen atau yang belum panen untuk medapatkan data luas panen. Data luas panen ini dapat diperoleh dengan mengurangkan luas tanam dengan luas belum panen. Dengan diperolehnya data luas tanam, data luas panen, maka dengan angka produktivitas yang diketahui kita akan mendapatkan nilai berapa besarnya produksi.

The objective of this research were to review the spatial pattern of LULC change and analyze the cause and to identify the anticipation strategy of paddy field conversion. The analysis uses the Landsat data of 1999, 2004, 2009, and 2014 which were interpreted by supervise technique. The interpretation result was compared to the existing LULC and was examined with KHAT statistic. This research was focused on the spatial pattern of LULC change, integrated with the Interpretative Structural Modeling (ISM) for reviewing the cause and the anticipation strategy of the conversion. The results revealed that the paddy field was converted into plantation, built area, and dryland agriculture. Planning analysis to increase rice production either through intensification of existing paddy fi... more Planning analysis to increase rice production either through intensification of existing paddy field area or extensification in potential land area was conducted in West Lombok Regency, West Nusa Tenggara Province, Indonesia. Existing paddy field was delineated using high-resolution data from IKONOS imagery of 2012. Land use and land cover outside existing paddy field were interpreted using SPOT-5 imagery of 2012. The Automated Land Evaluation System (ALES) was used for land suitability analysis for paddy. The results are interpreted in terms of the potential of paddy field intensification in existing paddy field area and the potential of extensification in land potentially used for paddy field. The result of analysis showed that in West Lombok Regency, there are still possible to do intensification and extensification of paddy field to increase rice production in order to improve regional food security.

Radiation therapy is the standard of care treatment for locally advanced cervical cancer in the U... more Radiation therapy is the standard of care treatment for locally advanced cervical cancer in the United States. In 1999 the addition of concomitant chemotherapy to radical radiotherapy became standard. The addition of cisplatin (CDDP) with or without 5-fluorouracil (5-FU) chemotherapy to radiation therapy was based on the near simultaneous reporting of five randomized, controlled clinical trials which all showed an improvement in survival with a magnitude of approximately 35%. The purpose of our study was to test the hypothesis that the addition of chemotherapy improved survival in our patients. We identified 291 patients treated with primary 'intent-to-cure' radiation therapy for locally advanced carcinoma of the cervix between 1985 and 2000. We analyzed patients using a stepwise Cox regression, including as possible predictors: clinical stage, age at diagnosis, use of concurrent chemotherapy with radiation and method of teletherapy delivery. We also examined survival as a function of CRT with a CDDP and/or 5-FU containing regimen using the Kaplan-Meier estimates of overall survival. The use of concurrent CDDP and/or 5-FU chemotherapy with radiation (CRT) was not associated with an increase in disease free survival (p=0.734) or overall survival (p=0.989). In this retrospective study there was no disease free or overall survival benefit from the addition of CDDP and/or 5-FU chemotherapy to radical radiotherapy for the treatment of locally advanced cervical carcinoma, although there was a trend favoring CRT.

Micturition syncope is a relatively uncommon cause of sudden and temporary loss of consciousness.... more Micturition syncope is a relatively uncommon cause of sudden and temporary loss of consciousness. Most prevalent in older males, this condition is extremely rare in women. Two pregnant women presented with recurrent syncope. The syncopal events were more common in the early morning and were precipitated by the presence of a distended bladder, urinary urgency, assuming a standing position after either being supine or sitting, or associated with travel motion. Extensive neurology and cardiology evaluations were negative, and the events were considered consistent with micturition syncope. Both patients responded favorably to conservative voiding behavior modification measures throughout the remainder of pregnancy. Micturition syncope during pregnancy may not be a rare occurrence. This potentially dangerous condition is amenable to voiding behavior modification measures.

Gigantomastia is a rare complication of pregnancy usually associated with benign conditions, incl... more Gigantomastia is a rare complication of pregnancy usually associated with benign conditions, including end-organ hypersensitivity to normal hormone levels, penicillamine therapy, mirror syndrome, and benign or glandular fibroadenomas. A young woman presented at 30 weeks of gestation with chest pain and gigantomastia. Echocardiography performed because of the patient's tachypnea and tachycardia disclosed pulmonary hypertension. After spontaneous delivery, core needle breast and axillary lymph node biopsies and computerized tomography imaging were performed, and stage IV diffuse large B cell lymphoma with infiltration of the breasts was confirmed. The patient responded to systemic chemotherapy with resolution of the gigantomastia and pulmonary artery hypertension. This case demonstrates that systemic malignancies such as diffuse large B cell lymphoma should be considered in the differential diagnosis of gigantomastia during pregnancy. In addition, malignancy-related pulmonary hypertension during pregnancy may be reversible after chemotherapy, as reported in nonpregnant patients.

Cervical pregnancy is an uncommon ectopic pregnancy that accounts for approximately &... more Cervical pregnancy is an uncommon ectopic pregnancy that accounts for approximately <1% of extrauterine gestations. This condition is associated with an extremely high risk of massive hemorrhage and previously often required hysterectomy. Current early ultrasonographic diagnosis and medical management in conjunction with other conservative measures, which include uterine artery embolization and intracervical balloon tamponade, have enabled conservation of the uterus. A young nulliparous patient ultrasonographically diagnosed with a cervical pregnancy and early fetal demise at 11 and 4/7 weeks gestation was managed with high-dose methotrexate and folinic acid rescue treatment. On the second day after treatment was initiated she spontaneously passed an intact gestational sac accompanied by minimal hemorrhage. Treatment was continued, with decreasing serum beta subunit of human chorionic gonadotropin levels and subsiding hemorrhage. Subsequent surgical measures were not required. This case suggests that complete abortion of a cervical pregnancy not necessitating surgical measures can occur.

The cavum septum pellucidum is a fluid-filled cavity located between the membranes of the septi p... more The cavum septum pellucidum is a fluid-filled cavity located between the membranes of the septi pellucidi, which should always be visualized sonographically in normal fetuses between 18 and 37 weeks gestation and in approximately 50% of term infants. The cavum vergae, a cavity within the septum pellucidum, is located posterior to an arbitrary vertical plane formed by the columns of the fornix. We present a case in which markedly dilated cavum septum pellucidum et vergae noted at 29 weeks gestation was associated with midgut malrotation and volvulus, and review the literature pertaining to this unusual prenatal ultrasonographic diagnosis.

Gigantomastia is a rare complication of pregnancy usually associated with benign conditions, incl... more Gigantomastia is a rare complication of pregnancy usually associated with benign conditions, including end-organ hypersensitivity to normal hormone levels, penicillamine therapy, mirror syndrome, and benign or glandular fibroadenomas. A young woman presented at 30 weeks of gestation with chest pain and gigantomastia. Echocardiography performed because of the patient's tachypnea and tachycardia disclosed pulmonary hypertension. After spontaneous delivery, core needle breast and axillary lymph node biopsies and computerized tomography imaging were performed, and stage IV diffuse large B cell lymphoma with infiltration of the breasts was confirmed. The patient responded to systemic chemotherapy with resolution of the gigantomastia and pulmonary artery hypertension. This case demonstrates that systemic malignancies such as diffuse large B cell lymphoma should be considered in the differential diagnosis of gigantomastia during pregnancy. In addition, malignancy-related pulmonary hypertension during pregnancy may be reversible after chemotherapy, as reported in nonpregnant patients.

This condition is associated with an extremely high risk of massive hemorrhage and previously often required hysterectomy. Current early ultrasonographic diagnosis and medical management in conjunction with other conservative measures, which include uterine artery embolization and intracervical balloon tamponade, have enabled conservation of the uterus. A young nulliparous patient ultrasonographically diagnosed with a cervical pregnancy and early fetal demise at 11 and 4/7 weeks gestation was managed with high-dose methotrexate and folinic acid rescue treatment. On the second day after treatment was initiated she spontaneously passed an intact gestational sac accompanied by minimal hemorrhage. Treatment was continued, with decreasing serum beta subunit of human chorionic gonadotropin levels and subsiding hemorrhage. Subsequent surgical measures were not required. This case suggests that complete abortion of a cervical pregnancy not necessitating surgical measures can occur.

The cavum septum pellucidum is a fluid-filled cavity located between the membranes of the septi p... more The cavum septum pellucidum is a fluid-filled cavity located between the membranes of the septi pellucidi, which should always be visualized sonographically in normal fetuses between 18 and 37 weeks gestation and in approximately 50% of term infants. The cavum vergae, a cavity within the septum pellucidum, is located posterior to an arbitrary vertical plane formed by the columns of the fornix. We present a case in which markedly dilated cavum septum pellucidum et vergae noted at 29 weeks gestation was associated with midgut malrotation and volvulus, and review the literature pertaining to this unusual prenatal ultrasonographic diagnosis.
