Spacer devices used by 62 asthmatic children were examined. Swabs taken from the inner surface of the reservoirs and face masks were cultured. Parents were interviewed regarding their spacer cleaning and disinfection routines. Bacterial contamination was noted in 22 reservoirs (35.5%) and 16 masks (25.8%). Pseudomonas aeruginosa was isolated from 21.0% of the reservoirs and 14.5% of the face masks, Klebsiella pneumoniae from 6.5% and 4.8%, and Staphylococcus aureus from 9.7% and 8.1%, respectively. Only 34 parents (54.8%) reported that they received cleaning and maintenance instructions from the medical staff at initiation of spacer use by their child, and only 38 (61.8%) cleaned the device after each use. Bacterial contamination is common in spacer devices. This study demonstrates that contamination rates are significantly lower when parents clean and actually dry (preferably with an air blower) spacer devices after each use. Spacer device maintenance should be emphasized in education programs for managing asthma.

Patients admitted for elective surgery (inguinal hernia, varicose veins and hemorrhoids) were stu... more Patients admitted for elective surgery (inguinal hernia, varicose veins and hemorrhoids) were studied in order to evaluate the impact of performing pre-admission testing (PAT) by utilizing an automated multiphasic health testing (AMHT) technology on the rate of repeating the pre-operative routine laboratory tests during hospitalization. A slightly lower but statistically significant rate of repeated tests was found among patients who performed PAT by AMHTS compared with those who performed the tests via the conventional ambulatory system. This result suggests that performing routine tests before hospitalization in a single authorized AMHTS facility is preferred since it saves the patient time and reduces the need for repeated tests.

The conditions which currently characterize the modern clinical setting which although emanating ... more The conditions which currently characterize the modern clinical setting which although emanating from unprecedented scientific progress, have paradoxically potentiated the likelihood that medicines may ironically harm those whom they were intended to heal. With this reality becoming ubiquitous throughout the Western world, new technologies and organizational cultures will need to be developed and implemented in all levels of the clinical setting to facilitate the safe use of drugs. Our recent experience in the Israeli primary care setting has demonstrated that the implementation of Electronic Patient Records (EPRs) systems in the managed care setting have provided expanded opportunities for drug utilization surveillance in the community setting. We therefore propose that electronic patient records (EPRs) should play an important role in efforts to improve drug safety in the community setting. Although EPRs data generated in the managed care setting most probably demonstrates the greatest potential to facilitate programs of this kind, it must be clarified that the validity of this data is highly dependant upon the quality of physician data-entry into the EPRs during patient visits. Thus, managed care organizations should dedicate resources to create an organizational culture that encourages complete and accurate information-recording during patient visits through the creation of a work environment conducive to this task.

A prospective, randomized, double-blind study was performed on 87 patients with ureterolithiasis,... more A prospective, randomized, double-blind study was performed on 87 patients with ureterolithiasis, assessing the effect of the essential oil preparation Rowatinex (Rowa Pharmaceuticals, Ireland) for the treatment of ureterolithiasis. Forty-three patients were treated with Rowatinex and 44 patients with placebo. Despite the larger average diameter of calculi in the group of patients treated with Rowatinex, the overall stone expulsion rate was significantly higher in the Rowatinex group as compared to placebo: 81% and 59%, respectively (0.025 > p > 0.01). This higher rate of stone expulsion is observed in patients with disappearance of pretreatment ureteral dilatation (when patients with expelled stones are excluded) (0.05 > p > 0.001). Seven patients in the Rowatinex group had mild to moderate gastrointestinal disturbances; no other significant side effects were noted during the treatment in either group. We conclude that early treatment with Rowatinex for patients with ureteral stones is indicated before other more aggressive measures are considered.

Widely differing ranges for the preferred firstmating age for rats have been reported by differen... more Widely differing ranges for the preferred firstmating age for rats have been reported by different investigators. As well as being of physiological interest this information is important for optimum utilization of animal resources. Previously published results, however, do not provide sufficient information on mortality and the numbers of pups born and weaned. We studied 1623 pups born to 36 female Wistar rats. Dams were first mated at the age of 35 days or 70 days or 105 days. The results show that reproduction and weaning weights increase with an increased first-mating age of dams, although the pup mortality rate also increases. This study can be used when a programme for the production and supply of Wistar rats is planned.

Background: Primary care physicians' adherence to accepted asthma guidelines is necessary for the... more Background: Primary care physicians' adherence to accepted asthma guidelines is necessary for the proper care of asthma patients. Objectives: To investigate the compliance of primary care physicians with clinical guidelines for asthma treatment and their participation in related educational programs, and to evaluate the influence of their employment status. Methods: A questionnaire was administered to a random sample of 1,000 primary care practitioners (pediatricians and family physicians) in Israel. Results: The response rate was 64%. Of the physicians who participated, 473 (75%) had read and consulted the guidelines but only 192 (29%) had participated in an educational program on asthma management in the last 12 months. The younger the responding physician (fewer years in practice), the more likely his/her attendance in such a program (p < 0.0001). After consulting the guidelines 189 physicians (40%) had modified their treatment strategies. Significantly more self-employed than salaried physicians had read the guidelines and participated in educational programs; physicians who were both self-employed and salaried fell somewhere between these groups. This trend was not influenced by the number of years in practice. Conclusions: All primary care physicians should update their knowledge more often. The publication of guidelines on asthma must be followed by their proper dissemination and utilization. Our study suggests that major efforts should be directed at the population of employed physicians.

This number is similar to the normal litter size and normal number of teats in rats. In our study, the normal litter size was X = 12·5. Our results were obtained under favourable conditions without disturbing the animals and facilitating access to food and water for the mothers and pups.

Background: Current guidelines for the treatment of uncomplicated urinary tract infection (UTI) i... more Background: Current guidelines for the treatment of uncomplicated urinary tract infection (UTI) in women recommend empiric therapy with antibiotics for which local resistance rates do not exceed 10–20%. We hypothesized that resistance rates of Escherichia coli to fluoroquinolones may have surpassed this level in older women in the Israeli community setting. Objectives: To identify age groups of women in which fluoroquinolones may no longer be appropriate for empiric treatment of UTI. Methods: Resistance rates for ofloxacin were calculated for all cases of uncomplicated UTI diagnosed during the first 5 months of 2005 in a managed care organization (MCO) in Israel, in community-dwelling women aged 41–75 years. The women were without risk factors for fluoroquinolone resistance. Uncomplicated UTI was diagnosed with a urine culture positive for E. coli. The data set was stratified for age, using 5 year intervals, and stratum-specific resistance rates (% and 95% CI) were calculated. These data were analyzed to identify age groups in which resistance rates have surpassed 10%. Results: The data from 1291 urine cultures were included. The crude resistance rate to ofloxacin was 8.7% (95% CI 7.4 to 10.2). Resistance was lowest among the youngest (aged 41–50 y) women (3.2%; 95% CI 1.11 to 5.18), approached 10% in women aged 51–55 years (7.1%; 95% CI 3.4 to 10.9), and reached 19.86% (95% CI 13.2 to 26.5) among the oldest women (aged 56–75 y). Conclusions: Physicians who opt to treat UTI in postmenopausal women empirically should consider prescribing drugs other than fluoroquinolones. Concomitant longitudinal surveillance of both antibiotic utilization patterns and uropathogen resistance rates should become routine practice in this managed-care organization.

Strychiomel is a compound of three drugs, yohimbine, thioridazine (Mellaril), and strychnine sulfate, each of which has a specific effect. The results suggest that the compound drug may be beneficial in the majority of patients (66.7%) when impotence occurs without hormonal or severe vascular or psychologic problems. Other factors such as age or presence of other simultaneous diseases do not appear to be significant when treatment with this multiaction drug (Strychiomel) is utilized.

Design: A randomized, double-blind design was used. Patients and Methods: Primary pediatric community clinics. Two hundred children aged 5 to 18 years with ear pain and clinical ear drum findings of acute otitis media were treated with naturopathic ear drops or oral amoxicillin. Ear pain was assessed over three days using a visual analog scale. The primary outcome measure was the reduction in mean score for pain, the secondary outcome measure was the presence of middle ear effusion on tympanometry during a 14-week follow-up. Results: Although a reduction in pain score was observed in both groups, it was significantly greater in the children treated with the naturopathic eardrops (p<0.0001). There was no statistically significant difference between the groups in the clinical resolution of acute otitis media or the presence of middle ear effusion (5% in both). Conclusions: The naturopathic eardrops solution used in the present study is at least as effective as amoxicillin in the treatment of middle ear effusion, and is significantly better for the management of ear pain associated with acute otitis media.

Resistance was lowest among the youngest (aged 41–50 y) women (3.2%; 95% CI 1.11 to 5.18), approached 10% in women aged 51–55 years (7.1%; 95% CI 3.4 to 10.9), and reached 19.86% (95% CI 13.2 to 26.5) among the oldest women (aged 56–75 y). Conclusions: Physicians who opt to treat UTI in postmenopausal women empirically should consider prescribing drugs other than fluoroquinolones. Concomitant longitudinal surveillance of both antibiotic utilization patterns and uropathogen resistance rates should become routine practice in this managed-care organization.

Spacer devices used by 62 asthmatic children were examined. Swabs taken from the inner surface of the reservoirs and face masks were cultured. Parents were interviewed regarding their spacer cleaning and disinfection routines. Bacterial contamination was noted in 22 reservoirs (35.5%) and 16 masks (25.8%). Pseudomonas aeruginosa was isolated from 21.0% of the reservoirs and 14.5% of the face masks, Klebsiella pneumoniae from 6.5% and 4.8%, and Staphylococcus aureus from 9.7% and 8.1%, respectively. Only 34 parents (54.8%) reported that they received cleaning and maintenance instructions from the medical staff at initiation of spacer use by their child, and only 38 (61.8%) cleaned the device after each use. Bacterial contamination is common in spacer devices. This study demonstrates that contamination rates are significantly lower when parents clean and actually dry (preferably with an air blower) spacer devices after each use. Spacer device maintenance should be emphasized in education programs for managing asthma.

Strychiomel is a compound of three drugs, yohimbine, thioridazine (Mellaril), and strychnine sulfate, each of which has a specific effect. The results suggest that the compound drug may be beneficial in the majority of patients (66.7%) when impotence occurs without hormonal or severe vascular or psychologic problems. Other factors such as age or presence of other simultaneous diseases do not appear to be significant when treatment with this multiaction drug (Strychiomel) is utilized.

Objective: To compare the efficacy of naturopathic eardrops with amoxicillin in the management of... more Objective: To compare the efficacy of naturopathic eardrops with amoxicillin in the management of ear pain associated with acute otitis media. Design: A randomized, double-blind design was used. Patients and Methods: Primary pediatric community clinics. Two hundred children aged 5 to 18 years with ear pain and clinical ear drum findings of acute otitis media were treated with naturopathic ear drops or oral amoxicillin. Ear pain was assessed over three days using a visual analog scale. The primary outcome measure was the reduction in mean score for pain, the secondary outcome measure was the presence of middle ear effusion on tympanometry during a 14-week follow-up. Results: Although a reduction in pain score was observed in both groups, it was significantly greater in the children treated with the naturopathic eardrops (p<0.0001). There was no statistically significant difference between the groups in the clinical resolution of acute otitis media or the presence of middle ear effusion (5% in both). Conclusions: The naturopathic eardrops solution used in the present study is at least as effective as amoxicillin in the treatment of middle ear effusion, and is significantly better for the management of ear pain associated with acute otitis media.
