Prosthodontic rehabilitation following aggressive infections and malignancies is challenging. Sur... more Prosthodontic rehabilitation following aggressive infections and malignancies is challenging. Surgical intervention may lead to anatomical defects resulting in oro-antral communication that causes difficulty in mastication, deglutition, impaired speech and facial disfigurement. The role of a Prosthodontist in maxillary defects is to restore these defects with the fabrication of obturators. However, the extent of the obturator into these large defects necessitate bulkier, heavy prosthesis which would likely be nonretentive and uncomfortable for the patient. Hollow bulb obturators are fabricated to reduce the weight of these prostheses. The method of fabrication of hollow bulb obturators with lost salt technique in the maxillary defect is described in this case report.

The study aims to evaluate the frequency of these occlusal schemes in Nepalese dental students. Materials and methods: A cross-sectional study was conducted among dental students in People’s Dental College and Hospital, Kathmandu, Nepal after the ethical approval from PDCH-IRC. Sixty four undergraduate students aged 21-30 years, were examined for the occlusal contact patterns in lateral mandibular movements using 12μm shim stock. The data were analyzed using SPSS software (version 21) Result: On 1mm laterotrusive position on mandible, 43 subjects (67%) showed group function, 15 subjects (24%) canine guided and 6 subjects (9%) non specified occlusion. The need for maintenance to keep pace with these changes necessitates a regular follow up. However, in a developing country like ours, it is not unusual to find patients either delaying or completely avoiding these follow ups citing various reasons. Often, a new prosthesis is recommended in these situations without considering the prospect of relining and rebasing. This clinical report presents a scenario where one of these maintenance therapies was undertaken to improve the fit and function of the complete denture.

Obturator is one of the modalities to facilitate closure of the resultant defect. This report presents a case where an acrylic plug obturator retained with clasp, was provided to the patient for the interim period following marsupialization on the mandibular left posterior region.

The presence of these displaceable denture-bearing tissues often poses difficulty during fabrication of complete dentures resulting in compromised support, retention and stability. Many impression techniques and materials have been proposed in various literatures to help overcome this difficulty. These techniques vary in philosophies but are often quite time-consuming to perform, and rely on materials not commonly in use in contemporary dental practice. The purpose of this paper is to describe an impression technique for flabby ridges, using conventional impression material.

This study aims to identify the sources of stress amongst final year undergraduate dental students in prosthodontics at People’s Dental College and Hospital, Nepal. Identifying these potential sources of stress may provide faculties and administrators an opportunity to delineate areas of concern and approach student effectively. Materials and methods: A modified version of the Dental Environment Stress (DES) questionnaire with 22 items was used to assess levels of stress. Results: The major stressors included worry of not completing quotas with a mean score of 3.63 followed by examinations, shortage of allocated clinical and laboratory time, fear of failing a courser the year, overloaded feeling due to huge syllabus, late ending day, responsibility of getting suitable patients, fear of being unable to catch up if behind and patients being late or not showing for their appointments. It demands an equally sized ocular prosthesis to replace lost tissue volume. A conventional solid acrylic ocular prosthesis may be deterrent to retention and esthetics because of its weight, which can be reduced by eliminating the mass of material within it. This article attempts to present a simplified and accurate technique to fabricate a hollow ocular prosthesis using silicone putty cavity form. It is removed prior to final processing and serves to ensure appropriate uniform thickness of acrylic resin and optimal weight reduction of the definitive prosthesis without compromising mobility, esthetics, and structural integrity.

Background: Impression making is one of the critical factors in determining success or failure of... more Background: Impression making is one of the critical factors in determining success or failure of complete denture therapy. The precise reproduction of edentulous foundation is essential for retention, stability, and support of the removable prosthesis. Evidence suggests that there is variability in materials and techniques regarding impressions making for complete dentures. There is little published information on studies conducted among Nepalese Prosthodontists. The purpose of this study was to evaluate current materials and methods for impressions for complete denture prosthodontics in Nepal. Materials & Methods: A questionnaire based survey was conducted among Nepalese Prosthodontists from May 2017 to December 2017. Ethical approval for the beginning of the study was obtained via Institutional Review Committee, Nepal Medical College. A self-administered anonymous questionnaire was distributed to the participants. Data were analyzed by using frequency distribution. Results: Majority of responses indicated use of irreversible hydrocolloid in stock metal tray for making the preliminary impression. The selective pressure was the predominantly used impression philosophy (78.3%). The most common material for the fabrication of custom trays was autopolymerizing acrylic resin (96.7%). All respondents border molded the custom tray prior to making the final impression. The final impression materials used were zinc oxide eugenol impression paste (73.3%), polyvinylsiloxane (11.7%), polyether (11.7%) and polysulphide (3.3%). Conclusion: Although there is variability in impression materials and techniques used by Prosthodontists in Nepal for the fabrication of complete dentures, the results showed interesting trends. Most practitioners followed the traditional techniques of complete denture impression procedures.

Accurate positioning of the iris is critical for the success of an ocular prosthesis. Precise dup... more Accurate positioning of the iris is critical for the success of an ocular prosthesis. Precise duplication of size and location of the iris is essential for the establishment of realism, symmetry, and a natural-appearing gaze. This article explores an alternative procedure for positioning the iris in a custom-made ocular prosthesis using a pupillary distance ruler (PD ruler). The orientation and mediolateral dimension of the iris of the natural eye was measured from the graduated scale on a PD ruler and transferred on to the sculpted scleral wax pattern. The iris button was positioned in the marked area, and the established orientation was evaluated for symmetry and a natural-appearing gaze. Accurate, predictable, and easy positioning of the iris with the transfer of these dimensions to the definitive prosthesis provided a good result in terms of patient esthetics and satisfaction.

Impression procedures for such defects can be time consuming and cumbersome, challenging the skills of even the most-experienced clinician. A well-oriented impression tray with proper extension and adequate contour is essential for successfully capturing the relevant details in impression. This article describes a method using the patient's existing obturator prosthesis to simplify and expedite the fabrication of a lightweight individualized impression tray directly on an irreversible hydrocolloid impression, thus avoiding the preliminary impression procedure for the patient.

A decreased denture-bearing area, increased interarch space, and lever action in such maxillofacial defects may cause problems with regard to retention, stability, and support of the prosthesis. Reducing the weight of the prosthesis helps to decrease the lever action and load on the residual alveolar ridge. This clinical report describes how a patient with a posterior segmental mandibulectomy was rehabilitated with a hollow cast partial dental prosthesis using a simplified technique with expanded polystyrene.

Before surgical intervention, feeding appliance is used to restore the palatal cleft and aid in feeding of an infant. The impression procedure during the fabrication of a feeding appliance is a crucial step and selection of impression tray and impression material is a challenging task. This article attempts to briefly describe an alternative technique for making impression in infants with cleft palate.
