We implanted stents in three patients who had traumatic abdominal aortic dissections, complicated... more We implanted stents in three patients who had traumatic abdominal aortic dissections, complicated by right limb ischemia in one case. The circulating false channel extended to the left iliac artery in one case and to both iliac arteries in the last case. Diagnosis and radiological follow-up included ultrasound, computed tomography, and arteriography. Two patients were treated with Wallstents, one with a Palmaz stent. The occlusion of the false channel was obtained in all patients without any significant residual stenosis. No early or late complication was noted in any of the patients. The longest follow-up was 2 years.

Bronchial artery dilatation and aneurysm formation is a potential complication of local inflammat... more Bronchial artery dilatation and aneurysm formation is a potential complication of local inflammation, especially in bronchiectasis. When the bronchial artery has an ectopic origin from the inferior segment of the aortic arch, aneurysms may mimick aortic aneurysms. Despite this particular location, endovascular treatment is possible. We report two such aneurysms that were successfully embolized with steel coils.

Breast MRI in invasive lobular carcinoma: diagnosis and staging J Radiol 2005;86:1027-34 Purpose.... more Breast MRI in invasive lobular carcinoma: diagnosis and staging J Radiol 2005;86:1027-34 Purpose. To assess the use of breast MRI for the diagnosis and staging of invasive lobular carcinoma and its impact on surgical management. Materials and methods. Retrospective study of 35 invasive lobular carcinoma, histologically diagnosed in 34 patients who underwent clinical exam, mammography, ultrasonography and magnetic resonance imaging. Results. Enhancement at MRI was seen for all 35 cancers. It was focal for 24 patients, regional for10 and diffuse for 1. The kinetic of the enhancement was characteristic of malignancy for 33 patients. For 11 patients the MRI staging was positive, finding 8 news cancers. For 3 patients MRI lead to biopsy of benign lesions but improved the surgical management in the 8 cases of new cancers: wider excision for 3 cases, planned breast conservation converted to mastectomy in 3 cases and excision of contralateral lesion in 2 cases. Conclusion. Breast MRI is useful in diagnosis, staging and surgical management of invasive lobular breast carcinoma. Objectif. Évaluer les performances de l'IRM mammaire pour le diagnostic et le bilan d'extension dans les carcinomes lobulaires infiltrants ainsi que l'impact thérapeutique en découlant.. Matériels et méthodes. Étude rétrospective de 35 dossiers consécutifs de patientes âgées de 38 à 76 ans ayant toutes un carcinome lobulaire infiltrant. Chaque dossier comportait une évaluation clinique au moment du diagnostic, une mammographie, une échographie mammaire et une IRM mammaire. Résultats. Les 35 cancers étudiés présentaient tous un rehaussement à l'IRM. Ce rehaussement était nodulaire ou focal pour 24 patientes, en plage pour 10 patientes et diffus pour une patiente. La cinétique de rehaussement tumoral était suspecte de malignité (précoce, rapide, intense) chez 33 patientes. Pour 11 patientes le bilan d'extension était positif à l'IRM retrouvant au final 8 cancers. L'IRM avait induit 3 cas de biopsies pour des lésions bénignes mais permit d'optimiser le geste chirurgical pour les 8 cas de cancers en élargissant le geste conservateur dans 3 cas, en transformant une indication de chirurgie conservatrice en mastectomie dans 3 cas et en permettant une biopsieexerèse d'une lésion néoplasique controlatérale dans 2 cas. Conclusion. L'IRM mammaire est performante dans le diagnostic positif des carcinomes lobulaires infiltrants ainsi que dans le bilan d'extension et a donc un impact thérapeutique majeur.

Although large bowel obstruction (LBO) is four to five times less common than small bowel obstruc... more Although large bowel obstruction (LBO) is four to five times less common than small bowel obstruction (SBO), it is a frequent problem and accounts for approximately 2-4% of surgical admissions for acute abdominal conditions. Because of significant limitations in the clinical and initial radiologic evaluations, with the development of computed tomography (CT) for examination of the acute abdomen, the role of CT in the diagnosis and assessment of bowel obstruction has expanded. This role has been stressed mainly for the diagnosis and management of SBOs . As with SBO, the ideal method for evaluating patients with suspected LBO should be relatively noninvasive and capable of answering the following questions defined by Mondor et al.
