This divergence clouds the process of envisioning a future for the library, but, if properly defined, it is a source of clarity, which sheds light on strategic opportunities.

Retinoids, both naturally occurring and synthetic, are analogues of vitamin A. They represent a m... more Retinoids, both naturally occurring and synthetic, are analogues of vitamin A. They represent a major advance in the treatment of acne and psoriasis, as well as rarer conditions such as the ichthyoses. The more frequent and widespread use of these drugs requires that the dermatologist be aware of the wide spectrum of adverse events that may be encountered with their use. Some are serious and irreversible, such as teratogenicity, necessitating great care if these drugs are given to women of childbearing potential. Other effects may be less severe, dose dependent and reversible. Although not devastating to the patient, appropriate advice may reduce morbidity.

The objective of this research was to examine the contribution of a moderately high fat (MHF) die... more The objective of this research was to examine the contribution of a moderately high fat (MHF) diet to the development of salt-sensitive hypertension in obese Zucker rats. Lean and obese Zucker rats were fed either a MHF diet or a diet of standard rat chow (control diet) for 10 weeks. From week 4 through week 10, the drinking water was supplemented with 1% NaCl. Blood pressure was measured weekly, and urinary excretion of nitric oxide metabolites (NO(x)) was determined at weeks 4 and 10. At week 10, renal nitric oxide synthase (NOS) activity was assessed in kidney homogenates. Blood pressures of obese, but not lean, rats on the MHF fat diet were significantly increased by salt-supplementation, whereas blood pressures of rats on the control diet were not appreciably affected. NO(x) excretion was increased in response to salt-supplementation in rats on the control diet, with the effect being particularly dramatic in obese rats. After salt-supplementation, NO(x) excretion by rats on the MHF diet was lower than rats on the control diet. In obese rats on the MHF diet, this decrease in NO production was accompanied by a reduction in renal NOS activity. These results indicate that obese rats are more inclined than lean rats to develop diet-induced hypertension in response to a moderately high fat, salt-supplemented diet. Furthermore, they suggest that MHF diet-induced defects in NO production may promote the salt-sensitivity of blood pressure in obese Zucker rats, which appear to require more NO to maintain blood pressure during a salt challenge.

Reported here is an evaluation of a new immunochromatographic strip, the PATH Falciparum Malaria IC Strip, which is impregnated with an immobilized IgM monoclonal antibody that binds to the HRP-II antigen of Plasmodium falciparum. In contrast to other commercially available kits marketed for the rapid diagnosis of falciparum malaria, this kit should be affordable in the malaria-endemic world. Using microscopy and polymerase chain reaction (PCR)-based methods as reference standards, we compared two versions of the PATH test for the detection of P. falciparum infection in 200 febrile travellers. The ParaSight-F test and the ICT Malaria Pf test are commercially available kits marketed for the diagnosis of Plasmodium falciparum malaria. Both tests are antigen-capture assays based on the detection of P. falciparum histidine-rich protein 2 in peripheral blood. Using microscopy and a polymerase chain reaction (PCR)-based method as reference standards, we performed a 'blinded' comparison of these assays for the detection of P. falciparum infection in 200 febrile travellers returning from malaria-endemic areas. As determined by PCR and microscopy, 148 travellers had malaria and, of these patients, 54.7% (81/148) were infected with P. vivax only, 31.1% (46/148) with P. falciparum only, 9.5% (14/148) with P. ovale, 0.7% (1/148) with P. malariae, and 4.1% (6/148) had mixed infections.
