Reports on the postoperative outcomes of sleeve gastrectomy (SG) have only been from small, singl... more Reports on the postoperative outcomes of sleeve gastrectomy (SG) have only been from small, single-center series and meta-analyses of studies with variable SG management. The objective of this study was to evaluate post-SG outcomes in a specialized bariatric surgery center with a routinely performed standardized procedure. The postoperative complication rate, operating times, and postoperative data were evaluated from all patients undergoing a primary SG between November 2004 and February 2012. Results were analyzed for 3 separate surgical periods, which differed with perioperative management. Of 600 patients (mean age: 41.8±11.3; mean body mass index [BMI]: 47.2±16 kg/m²; 80% were women who underwent primary SG), 26.8% had a BMI≥50 kg/m². The mean operating time was 84 minutes. The rate of conversion was 1%. There were no postoperative deaths. Sa correction peut nécessiter un traitement chirurgical en cas d'échec d'une prise en charge médicale adaptée. Une grossesse obtenue dans les suites d'un traitement chirurgical par pose d'anneau gastrique doit être programmée et surveillée comme une grossesse à haut risque, en effet celle-ci peut se compliquer. Mesh infections (MI) are the major complication of prosthetic material. The aim of this study was to assess the efficacy of partial removal of mesh (PRM) therapy in the treatment of MI. From January 2000 to April 2010, from a prospective database, we retrospectively selected patients who underwent surgery for MI. We studied the epidemiological data (sex, age, obesity, diabetes, smoking), the operating time of the initial intervention, the presence of intestinal injuries during the first intervention, the average interval between initial surgical procedure and MI, the location of the hernia, the average size of the hernia, type of mesh used, the position of the mesh, type of surgery performed, the number through interventions required to achieve a cure, the cumulative duration of hospital stay and hernia recurrence rates. Repair of the abdominal wall is the last stage of abdominal surgery; pariet complications, partic... more Repair of the abdominal wall is the last stage of abdominal surgery; pariet complications, particularly infection, can have serious impact on operative results. While abdominal wound infections are not universally preventable, they are often predictable; the aphorism of Jean Rives (Stoppa, 1999 [1]) summarizes this sequence: "Infection is the mother of postoperative incisional hernia and infection of the incisional hernia repair is the grand-daughter". Repair of the abdominal wall in a potentially septic milieu requires the solution of an equation involving four variables: mechanism of sepsis, its severity, the surgical approach, and choice of prosthetic material. These interdependent variables potentiate each other, requiring adaptations of surgical strategy that cannot be absolutely determined pre-operatively, even with collegial consultation.

These may be discovered when reoperating on the bladder, prostate or iliac vessels. The difficulties are related to the extensive fibrosis involving the cave of Retzius and/or the space of Bogros. The authors report their intraoperative and anatomical findings. They propose the following solutions: 1. When cleavage of the cave of Retzius is impossible (in bladder or prostate surgery): a retropubic subperiosteal plane is developed, either isolated or combined with a transperitoneal approach. 2. When cleavage of the space of Bogros is impossible (in surgery on the iliac vessels): a transperitoneal approach is used. ABSTRACT La gastrectomia longitudinale (sleeve gastrectomy degli anglosassoni) è una tecnica di c... more ABSTRACT La gastrectomia longitudinale (sleeve gastrectomy degli anglosassoni) è una tecnica di chirurgia restrittiva che si è recentemente diffusa in Francia. Il benessere postoperatorio dei pazienti e i risultati a medio termine spiegano l’entusiasmo che attualmente circonda questa tecnica. Nel 2008, la Haute autorité de santé ha definito la gastrectomia longitudinale un’alternativa alla gastroplastica con anello e al bypass gastrico nei pazienti che soffrono di obesità patologica, in particolare in caso di rifiuto di impianto di un corpo estraneo. Si tratta di una tecnica riproducibile, facilmente insegnabile e con dei postumi operatori semplici nove volte su dieci. L’emorragia postoperatoria e la fistola gastrica costituiscono le due complicanze potenzialmente gravi, ma sono rare. Noi descriviamo le indicazioni, la tecnica operatoria, i risultati e i postumi di questo intervento.
