Introduction: Giant cell tumor of the larynx is an uncommon entity with only 42 reported cases in... more Introduction: Giant cell tumor of the larynx is an uncommon entity with only 42 reported cases in the literature, of which 32 involved the thyroid cartilage. These tumors most commonly occur in the epiphysis of the long bones of female patients in the third decade. Case Report: We present a 57-year-old man with hoarseness and a growing anterior neck mass for the past 5 months. Endoscopic examination revealed a submucosal swelling of the left supraglottic area. CT and MRI of the larynx showed a large, expansive mass at the left thyroid cartilage. The anterior neck mass was biopsied and histopathological analysis showed mononuclear cells and homogeneously distributed multinucleated giant cells. A giant cell tumor of the larynx was suspected. The patient underwent a supracricoid subtotal laryngectomy. Conclusion: Giant cell tumor of the larynx is an uncommon entity, with very few cases reported in the literature. These tumors should be included in the differential diagnosis of a patient with hoarseness and anterior neck swelling. There is no consensus regarding the management, although surgical excision is the most frequently employed treatment with excellent outcomes.

Early detection and appropriate treatment of hearing loss are essential to minimise the consequen... more Early detection and appropriate treatment of hearing loss are essential to minimise the consequences of hearing loss. In addition to conventional audiometry (125-8,000Hz), extended high-frequency audiometry (9,000-20,000Hz) is available. This type of audiometry may be useful in early diagnosis of hearing loss in certain conditions, such as the ototoxic effect of cisplatin-based treatment, noise exposure or oral misunderstanding, especially in noisy environments. Eleven examples are shown in which extended high-frequency audiometry has been useful in early detection of hearing loss, despite the subject having a normal conventional audiometry. The goal of the present paper was to highlight the importance of the extended high-frequency audiometry examination for it to become a standard tool in routine audiological examinations.

In the literature various methods are described to reduce bleeding in endoscopic sinus surgery. S... more In the literature various methods are described to reduce bleeding in endoscopic sinus surgery. Scientific evidence and results were gathered and analysed to determine the effectiveness of the various methods used. A total of 20 articles fulfilled the inclusion criteria. Two retrospective articles studied the differences between local and general anaesthesia. Three articles analysed the use of local methods to control bleeding. The majority of the articles analysed the use of different systemic drugs to control intraoperative bleeding. Certain procedures, such as the reverse Trendelenburg position, the use of high doses of epinephrine, the infiltration of phenylephrine and lidocaine into the pterygopalatine fossa, the preoperative use of prednisone, and the control of the heart rate (with dexmedetomidine or remifentanil), appear to reduce the intraoperative blood loss and/or improve the visualisation of the surgical field. The aim of the present study was to provide thresholds data as a function of age for an otologica... more The aim of the present study was to provide thresholds data as a function of age for an otologically normal population in Spain, compared to the current ISO 7029 (2000) standard. A prospective study in an otologically screened population. Data was collected from 1175 otologically-normal persons aged between 5 and 90 years. Inclusion criteria involved those listed in ISO 389-1 (1998) and the 8253-1 (2010). Suitability for inclusion was evaluated through interview, based on a questionnaire, and physical examination. The hearing thresholds decreased slightly from 125 to 2000 Hz. From 2000 Hz onwards the thresholds increased; this increase being more pronounced with increasing frequency and age. No statistically significant sex differences were found. The hearing threshold levels in the present study were higher (poorer) than the ones provided by ISO 7029 (2000). ABSTRACT Objective: To determine common reference equivalent threshold sound pressure levels (RET... more ABSTRACT Objective: To determine common reference equivalent threshold sound pressure levels (RETSPL) for the earphones used in the extended high-frequency (EHF) range, as different earphones are commercially available, but there are not RETSPLs for each model. Design: Hearing threshold sound pressure levels were measured up to 20 kHz for the Sennheiser HDA 200 audiometric earphone, and were compared to the ISO 389-5 (2006) norm and other investigations using that earphone and different ones. Study sample: A total of 223 otologically-normal subjects (aged 5-25 years old) participated in the hearing determination. Results: The results are in good agreement with previous studies of hearing thresholds using the same and other earphones. Conclusions: The results of the present investigation are relevant for the international standard for the calibration of audiometric equipment in the 8 to 16 kHz frequency range, ISO 389-5. The data may be used for a future update of the RETSPL for circumaural and insert audiometric earphones.

To determine the relationship between vascular loops in the internal auditory canal and otologic ... more To determine the relationship between vascular loops in the internal auditory canal and otologic symptoms. Methods: We performed a retrospective study, with a systematic review of the literature and analyzed the magnetic resonances imaging (MRI) of 91 patients attended in the ENT department since April to June 2013, in order to correlate radiological findings with otologic symptoms. Results: Vascular loops were seen in the internal auditory canal of patients without clinical symptoms; however an association was found between the presence of vascular compression of the eighth nerve displayed on MRI and the presence of sensorineural hearing loss and tinnitus. No association was found between vertigo and vascular loops. Conclusion: The presence of vascular loops in the internal auditory canal may be an incidental finding in MRIs of patients without clinical symptoms. However, in patients with tinnitus or sensorineural hearing loss that is unexplained by other clinical pathologies, these may be correlated with the existence of vascular loops which compress the eighth cranial nerve or contact the bone wall of the internal auditory canal. Further studies involving a larger number of patients are required to accurately evaluate the association between these symptoms and vascular loops.

Cystic lesions of the posterior triangle are a pathologic entity whose diagnosis is made in the f... more Cystic lesions of the posterior triangle are a pathologic entity whose diagnosis is made in the first two years of life. Its presentation in adulthood is an incidental finding and the differential diagnosis includes cystic lymphangioma, lymphatic metastasis of thyroid cancer and branchial cyst. Often with the finding of a cervical lump, FNA is made before diagnostic imaging is performed, however, this procedure is not always advisable. We reviewed the cases of patients who came last year to our department with a cystic mass in this location and correlating the imaging findings with pathologic specimen. We show characteristic findings of these lesions in order to make an early diagnosis and thus to get the approach and treatment appropriate of adult patients with a cystic lesion in the posterior cervical triangle.

Conclusions: The results of the present investigation are relevant for the international standard for the calibration of audiometric equipment in the 8 to 16 kHz frequency range, ISO 389-5. The data may be used for a future update of the RETSPL for circumaural and insert audiometric earphones.

Methods: We performed a retrospective study, with a systematic review of the literature and analyzed the magnetic resonances imaging (MRI) of 91 patients attended in the ENT department since April to June 2013, in order to correlate radiological findings with otologic symptoms. Results: Vascular loops were seen in the internal auditory canal of patients without clinical symptoms; however an association was found between the presence of vascular compression of the eighth nerve displayed on MRI and the presence of sensorineural hearing loss and tinnitus. No association was found between vertigo and vascular loops. Conclusion: The presence of vascular loops in the internal auditory canal may be an incidental finding in MRIs of patients without clinical symptoms. However, in patients with tinnitus or sensorineural hearing loss that is unexplained by other clinical pathologies, these may be correlated with the existence of vascular loops which compress the eighth cranial nerve or contact the bone wall of the internal auditory canal. Further studies involving a larger number of patients are required to accurately evaluate the association between these symptoms and vascular loops.

Its presentation in adulthood is an incidental finding and the differential diagnosis includes cystic lymphangioma, lymphatic metastasis of thyroid cancer and branchial cyst. Often with the finding of a cervical lump, FNA is made before diagnostic imaging is performed, however, this procedure is not always advisable. We reviewed the cases of patients who came last year to our department with a cystic mass in this location and correlating the imaging findings with pathologic specimen. We show characteristic findings of these lesions in order to make an early diagnosis and thus to get the approach and treatment appropriate of adult patients with a cystic lesion in the posterior cervical triangle.
