This current examines future family‑related plans adolescents might have on the verge of exiting high school and its relation to their self‑confidence, family, peer group and teacher support. An online survey was adminis‑ trated to 3,524 XII th graders. Results suggest that adolescents with high levels of support from their peers are more likely to plan moving with the partner, getting married or having a child in the near future. In this study we aimed to investigate experiences and challenges faced by Romanian professionals working in psychosocial oncology carein order to identify the stage of development of this area seven years after Romania joined the European Union.To meet this goal, we have structured our study into seven sections. Thus, in the second section, which follows the introductory part, we define the field of psychosocial care in oncology and briefly describe the evolution of this field both on international and national level. The third section briefly overviews standards of practice and ethical principles in psychosocial oncology care, as they were adapted from the American Association of Oncology Social Workers (AOSW). Next, we present the first empirical study we conducted among Romanian professionals in psychosocial oncology care. Section four, which presents the methodology used in the research, is followed by detailed description of preliminary results of the study. In this regard, the fifth section presents results structured by main dimensions, variablesincluded in the research:socio-demographic and professional characteristics;aspects of job satisfaction; opinions and attitudes about work and cancer patients’ distress; awareness of the state of psychosocial oncology development in Romania; and currentprofessional needs.Section six reflects relevant implications for professional ethics in psychosocial oncology care, which leads to directions for future action to improve services in psychosocial cancer care in Romania, in the seventh, last section.Translation from power to empowerment is an unsolved professional need in cancer care in Romania, as patients are still approached by professionals with over-protective and avoidant attitudes, which make them vulnarable to victimhood. Cancer patients in Romania are not yet considered active partners in helping, supportive relationships.

This paper aims to identify and explore psychosocial burdens experienced by cancer patients using a mixed-method research design with mainly qualitative output.The introductory part of this paper is followed by presentation of a theoretical framework comprising definitions used in qualitative research: triangulation, observation, purposive sampling, thematic analysis, collation, theory-based analysis (grounded theory analysis), theoretical saturation and phenomenological approach. In the section dedicated to methodology we described the sampling process, criteria for selecting participants in the study, data collection instruments and interview transcription protocols. The next section covers the data analysis process. Analysis, processing and interpretation of raw data resulting from qualitative research on cancer patients were based on 83 interviews included in the MAXQDA database.In this mixed-method design, an essentialist-realistic, inductive and evaluative approach data analysis was used, focused on theme-based analysis, tending towards grounded theory.The most extended section of this paper presents qualitative and quantitative data, results from the mixed-method analysis. Thus, there were common themes identified and described to provide a comprehensive picture of the meaning and essence of study participants' experiences. Complexity of the dynamics of interaction between different groups of patients and investigated subjects produced additional results, too. This paper ends with the conclusion section, including limitations and future directions of research.

ABSTRACT

Nowadays, cancer is affecting society at large, which makes the development of mental health care an important factor in oncology. This aspect affects Romania in a specific way, since the country is still struggling with deficiencies. The major aim of this paper was to investigate the possible positive effect of an intervention tailored to offer emotional and social support for cancer outpatients, improving mental health outside the hospital. The initial assessment was carried out with eight persons, while seven persons received psychosocial intervention. The intervention consisted of eight sessions, with practices of expressive writing and mindfulness-based activities. With the help of distress screening instruments, we monitored the values of psychosocial variables, thus learning about the psychosocial condition of cancer outpatients, values of emotional distress and the need for help, as well as changes in distress and in the number of perceived problems during the intervention. Original assumptions have been partly proven in the course of individual evaluations: five cases out of the seven yielded decreased or stagnant results, while the increase in certain indicators might be explained by an aggravated disease state, lack of social support, and negative life events which occurred during intervention. During the pre-test phase, we did not find significant differences with any of the variables between the two groups, while the post-test indicated significant differences between the two groups in terms of anxiety and depression. Consequently, social and emotional support was provided, need for help was properly met, and social isolation decreased – in the light of all these, this intervention may be considered in further studies to refine the positive effect within terms of mental health protection this specific population.

The narrowing opportunities, the uncertainties in Romanian society and the economical depression impede on adolescents' chances and increase the pressures on them to meet social requirements. Purpose of Study: Our study analyzes: (1) how adolescents understand the process of becoming an adult in Romania, and what criteria they have for it; (2) explore how a combination of psychological factors can interact with school adaptation to set the stage for the transition to adulthood. Research Methods: Sample consisted of 874 adolescents. The outcomes were measured using a questionnaire based on Arnett's survey and SSP , which was completed and adapted in Romania by the research team. Findings: Results indicate that 1) Romanian adolescents at the end of high-school consider autonomy on the top of their rankings to define adulthood, in similar ways as their fellows in the US; in preparation of adulthood, most important criteria include: financial and personal independence from parents. On the other hand, there are many differences in the rankings of these groups, mostly in the perceptions of future roles; (2) individual cognitions, school adaptation and educational attainment, hope in the future and overall quality of life are important for the transition period. Conclusions: The specific stage of emergence from adolescence to young adulthood requires a reexamination of its factors, taking into consideration the interaction of social cognitions and optimism, about future career, relationships, finances.

Psychotherapy and psycho-social interventions in oncological rehabilitation, based on confirmed r... more Psychotherapy and psycho-social interventions in oncological rehabilitation, based on confirmed results of several controlled studies designed and conducted in a bio-psycho-social research framework, have been proved to reduce the psycho-social vulnerability of cancer patients and to enhance health related quality of life in this patient population. It is also supposed that psychological intervention increases survival outcomes of cancer patients through immune-modulation. Findings of psycho-neuro-immunological research have underlined that the human neurological and immunological system, mainly the NK, CD4 and CD8 cell activity and cytotoxicity, are mediators and modulators in the progress of malignant tumors. Positive outcomes of psycho-social rehabilitation in oncology reinforce not only its effectiveness in reducing stress and anxiety, strengthening active coping mechanisms and immune functions, and improving quality of life but also its cost-effectiveness in view of relatively low investment and the high, multiplied health related benefits.

Romania still has to adapt and develop psychosocial assessment protocols which would allow standa... more Romania still has to adapt and develop psychosocial assessment protocols which would allow standardized screening for the sixth vital sign in cancer patients, namely distress and quality of life. The present study screens for the psychosocial and communication needs of cancer outpatients in palliative care and in rehabilitation. These data are also compared with those obtained from a sample of cancer inpatients. Subjects for this study were recruited from cancer care services from the Transylvania region in Romania. A total of 203 persons with cancer were included in the screening. Of this number, 68 were in the hospitalized group, 71 were from the rehabilitation group, and 64 were palliative care cases. FACT-G 4.0 and BDI screening instruments were used in these clinical samples. Statistical analysis performed was: associational and comparative statistics, one-way analysis of variance. Statistically and clinically significant results were found in relation to symptoms of depression, physical and functional well-being, and overall quality of life. However, with regard to psychosocial well-being, there were no clinically relevant or significant differences among cancer patients under hospital treatment, or in rehabilitation and palliative care programs. More than 40 % of the cancer patients included in our pilot screening were not open to talk about their illness and their related needs. Cancer is an unmet psychosocial challenge to patients and to the specialized services, regardless of care type. The urgency of psychosocial screening in the Romanian cancer care system is revealed. Based on these findings, there is a need for psychosocial screening and related supportive care services that need to be integrated into Romania's cancer care programs. A recommendation is to pilot psychosocial and supportive care model within the four regional cancer care institutions.

A tremendous growth occurred in the reported drug use and abuse in Romania from 1995 to 1999. Lack of concern by government and little policy attention contributed to the surprising delay of drug policy and drug prevention system development. General public stigmatize drug users and drug consumption is considered a matter of personal fault and responsibility. There is some but not sufficient research and evaluation on drug use, abuse problem. Approach. Drug use, abuse and prevention are discussed from research-based, user-focused and prevention system development perspectives. Prevalence and trends of drug use, abuse in the past decade (1995–2005) are summarized. Prevention issues are discussed based on research data from adolescents, parents and teachers. The Romanian primary drug prevention system has been evaluated based on our experiences in drug use prevention activities carried out in schools and recreational environments. Key Findings. Public and scientific perspectives on drug consumption in Romania, between 1995 and 1999, were dominated by an idealistic, non-realistic perception. Since 1995, drug use among adolescents increased almost four times in less than 4 years. The first law against drug traffic and consumption was issued only in 2000. Now primary drug prevention strategies are in action, but in general they are lacking standard evaluation procedures. Implications/Conclusion. Conclusions are drafted for new perspectives in prevention activities. More long-term, user-focused, demand-centred prevention activities should be carried out in more and more diversified settings and evaluation should be thoroughly considered.[Dégi CL. This paper aims to summarize the basic dilemmas, questions from the beginning period and points out some useful professional and personal experiences acquired during the last 4 years of psycho-oncology work, but also raises new perspectives in psycho-oncology research and intervention based on study, evaluation and integration of personal and family narratives.

One of the most important unsolved social issues in Romania is the situation of HIV infected chil... more One of the most important unsolved social issues in Romania is the situation of HIV infected children. Romania has a large group of children living with HIV and AIDS, over 7,000, according to the latest epidemiological data. All this children/ teenagers who are now turning to young adults were infected between 1987 and 1991 by unscreened blood transfusions and blood products, not properly sterilized needles, injections, surgical interventions etc. We would like to add that this is a unique, exceptional situation as they are the largest such group in Europe. Troublesome questions arise regarding participation or exclusion of young people living with HIV infection (YPLHIV) in Romania: how much discrimination, victimization are YPLHIV experiencing?; what is their level of education -do they have access to appropriate training and employment?; is Romania prepared to further guarantee to YPLHIV specific social protection; and how do YPLHIV cope with adulthood perspectives on individual, family and social level?. These questions are answered based on latest HIV/AIDS related data and they are exemplified with our personal and professional experiences from working with HIV infected children and their affected families.

In this regard, the fifth section presents results structured by main dimensions, variablesincluded in the research:socio-demographic and professional characteristics;aspects of job satisfaction; opinions and attitudes about work and cancer patients’ distress; awareness of the state of psychosocial oncology development in Romania; and currentprofessional needs.Section six reflects relevant implications for professional ethics in psychosocial oncology care, which leads to directions for future action to improve services in psychosocial cancer care in Romania, in the seventh, last section.Translation from power to empowerment is an unsolved professional need in cancer care in Romania, as patients are still approached by professionals with over-protective and avoidant attitudes, which make them vulnarable to victimhood. Cancer patients in Romania are not yet considered active partners in helping, supportive relationships.

This paper aims to identify and explore psychosocial burdens experienced by cancer patients using a mixed-method research design with mainly qualitative output.The introductory part of this paper is followed by presentation of a theoretical framework comprising definitions used in qualitative research: triangulation, observation, purposive sampling, thematic analysis, collation, theory-based analysis (grounded theory analysis), theoretical saturation and phenomenological approach. In the section dedicated to methodology we described the sampling process, criteria for selecting participants in the study, data collection instruments and interview transcription protocols. The next section covers the data analysis process. Analysis, processing and interpretation of raw data resulting from qualitative research on cancer patients were based on 83 interviews included in the MAXQDA database.In this mixed-method design, an essentialist-realistic, inductive and evaluative approach data analysis was used, focused on theme-based analysis, tending towards grounded theory.The most extended section of this paper presents qualitative and quantitative data, results from the mixed-method analysis. Thus, there were common themes identified and described to provide a comprehensive picture of the meaning and essence of study participants' experiences. Complexity of the dynamics of interaction between different groups of patients and investigated subjects produced additional results, too. This paper ends with the conclusion section, including limitations and future directions of research.

ABSTRACT

Nowadays, cancer is affecting society at large, which makes the development of mental health care an important factor in oncology. This aspect affects Romania in a specific way, since the country is still struggling with deficiencies. The major aim of this paper was to investigate the possible positive effect of an intervention tailored to offer emotional and social support for cancer outpatients, improving mental health outside the hospital. The initial assessment was carried out with eight persons, while seven persons received psychosocial intervention. The intervention consisted of eight sessions, with practices of expressive writing and mindfulness-based activities. With the help of distress screening instruments, we monitored the values of psychosocial variables, thus learning about the psychosocial condition of cancer outpatients, values of emotional distress and the need for help, as well as changes in distress and in the number of perceived problems during the intervention. Original assumptions have been partly proven in the course of individual evaluations: five cases out of the seven yielded decreased or stagnant results, while the increase in certain indicators might be explained by an aggravated disease state, lack of social support, and negative life events which occurred during intervention. During the pre-test phase, we did not find significant differences with any of the variables between the two groups, while the post-test indicated significant differences between the two groups in terms of anxiety and depression. Consequently, social and emotional support was provided, need for help was properly met, and social isolation decreased – in the light of all these, this intervention may be considered in further studies to refine the positive effect within terms of mental health protection this specific population.

The narrowing opportunities, the uncertainties in Romanian society and the economical depression impede on adolescents' chances and increase the pressures on them to meet social requirements. Purpose of Study: Our study analyzes: (1) how adolescents understand the process of becoming an adult in Romania, and what criteria they have for it; (2) explore how a combination of psychological factors can interact with school adaptation to set the stage for the transition to adulthood. Research Methods: Sample consisted of 874 adolescents. The outcomes were measured using a questionnaire based on Arnett's survey and SSP , which was completed and adapted in Romania by the research team. Findings: Results indicate that 1) Romanian adolescents at the end of high-school consider autonomy on the top of their rankings to define adulthood, in similar ways as their fellows in the US; in preparation of adulthood, most important criteria include: financial and personal independence from parents. On the other hand, there are many differences in the rankings of these groups, mostly in the perceptions of future roles; (2) individual cognitions, school adaptation and educational attainment, hope in the future and overall quality of life are important for the transition period. Conclusions: The specific stage of emergence from adolescence to young adulthood requires a reexamination of its factors, taking into consideration the interaction of social cognitions and optimism, about future career, relationships, finances.

Based on our results, we propose the routine use of self-administered questionnaires for better identification of psychosocial needs.

Psychotherapy and psycho-social interventions in oncological rehabilitation, based on confirmed r... more Psychotherapy and psycho-social interventions in oncological rehabilitation, based on confirmed results of several controlled studies designed and conducted in a bio-psycho-social research framework, have been proved to reduce the psycho-social vulnerability of cancer patients and to enhance health related quality of life in this patient population. It is also supposed that psychological intervention increases survival outcomes of cancer patients through immune-modulation. Findings of psycho-neuro-immunological research have underlined that the human neurological and immunological system, mainly the NK, CD4 and CD8 cell activity and cytotoxicity, are mediators and modulators in the progress of malignant tumors. Positive outcomes of psycho-social rehabilitation in oncology reinforce not only its effectiveness in reducing stress and anxiety, strengthening active coping mechanisms and immune functions, and improving quality of life but also its cost-effectiveness in view of relatively low investment and the high, multiplied health related benefits.

Romania still has to adapt and develop psychosocial assessment protocols which would allow standa... more Romania still has to adapt and develop psychosocial assessment protocols which would allow standardized screening for the sixth vital sign in cancer patients, namely distress and quality of life. The present study screens for the psychosocial and communication needs of cancer outpatients in palliative care and in rehabilitation. These data are also compared with those obtained from a sample of cancer inpatients. Subjects for this study were recruited from cancer care services from the Transylvania region in Romania. A total of 203 persons with cancer were included in the screening. Of this number, 68 were in the hospitalized group, 71 were from the rehabilitation group, and 64 were palliative care cases. FACT-G 4.0 and BDI screening instruments were used in these clinical samples. Statistical analysis performed was: associational and comparative statistics, one-way analysis of variance. Statistically and clinically significant results were found in relation to symptoms of depression, physical and functional well-being, and overall quality of life. However, with regard to psychosocial well-being, there were no clinically relevant or significant differences among cancer patients under hospital treatment, or in rehabilitation and palliative care programs. More than 40 % of the cancer patients included in our pilot screening were not open to talk about their illness and their related needs. Cancer is an unmet psychosocial challenge to patients and to the specialized services, regardless of care type. The urgency of psychosocial screening in the Romanian cancer care system is revealed. Based on these findings, there is a need for psychosocial screening and related supportive care services that need to be integrated into Romania's cancer care programs. A recommendation is to pilot psychosocial and supportive care model within the four regional cancer care institutions.

A tremendous growth occurred in the reported drug use and abuse in Romania from 1995 to 1999. Lack of concern by government and little policy attention contributed to the surprising delay of drug policy and drug prevention system development. General public stigmatize drug users and drug consumption is considered a matter of personal fault and responsibility. There is some but not sufficient research and evaluation on drug use, abuse problem. Approach. Drug use, abuse and prevention are discussed from research-based, user-focused and prevention system development perspectives. Prevalence and trends of drug use, abuse in the past decade (1995–2005) are summarized. Prevention issues are discussed based on research data from adolescents, parents and teachers. The Romanian primary drug prevention system has been evaluated based on our experiences in drug use prevention activities carried out in schools and recreational environments. Key Findings. Public and scientific perspectives on drug consumption in Romania, between 1995 and 1999, were dominated by an idealistic, non-realistic perception. Since 1995, drug use among adolescents increased almost four times in less than 4 years. The first law against drug traffic and consumption was issued only in 2000. Now primary drug prevention strategies are in action, but in general they are lacking standard evaluation procedures. Implications/Conclusion. Conclusions are drafted for new perspectives in prevention activities. More long-term, user-focused, demand-centred prevention activities should be carried out in more and more diversified settings and evaluation should be thoroughly considered.[Dégi CL. One of the most important unsolved social issues in Romania is the situation of HIV infected chil... more One of the most important unsolved social issues in Romania is the situation of HIV infected children. Romania has a large group of children living with HIV and AIDS, over 7,000, according to the latest epidemiological data. All this children/ teenagers who are now turning to young adults were infected between 1987 and 1991 by unscreened blood transfusions and blood products, not properly sterilized needles, injections, surgical interventions etc. We would like to add that this is a unique, exceptional situation as they are the largest such group in Europe. Troublesome questions arise regarding participation or exclusion of young people living with HIV infection (YPLHIV) in Romania: how much discrimination, victimization are YPLHIV experiencing?; what is their level of education -do they have access to appropriate training and employment?; is Romania prepared to further guarantee to YPLHIV specific social protection; and how do YPLHIV cope with adulthood perspectives on individual, family and social level?. These questions are answered based on latest HIV/AIDS related data and they are exemplified with our personal and professional experiences from working with HIV infected children and their affected families.
