Background: We published the Canadian 2003 International Consensus Algorithm for the Diagnosis, T... more Background: We published the Canadian 2003 International Consensus Algorithm for the Diagnosis, Therapy, and Management of Hereditary Angioedema (HAE; C1 inhibitor [C1-INH] deficiency) and updated this as Hereditary angioedema: a current state-of-the-art review: Canadian Hungarian 2007 International Consensus Algorithm for the Diagnosis, Therapy, and Management of Hereditary Angioedema. Objective: To update the International Consensus Algorithm for the Diagnosis, Therapy and Management of Hereditary Angioedema (circa 2010). Methods: The Canadian Hereditary Angioedema Network (CHAEN)/Réseau Canadien d'angioédème héréditaire (RCAH) http://www.haecanada.com and cosponsors University of Calgary and the Canadian Society of Allergy and Clinical Immunology (with an unrestricted educational grant from CSL Behring) held our third Conference May 15th to 16th, 2010 in Toronto Canada to update our consensus approach. The Consensus document was reviewed at the meeting and then circulated for review. Results: This manuscript is the 2010 International Consensus Algorithm for the Diagnosis, Therapy and Management of Hereditary Angioedema that resulted from that conference.

Vancouver BC, and Toronto, ONT To help address whether peanut-sensitive travelers are exposed to ... more Vancouver BC, and Toronto, ONT To help address whether peanut-sensitive travelers are exposed to peanut aeroallergans during airline flights on which peanuts are served, we obtained filter units (23" x 23" x 6") from the ventilation systems of two commercial airliners at the time of their annual replacement at~er approximately 5000 flight hours. The filter papers were torn into pieces, extrwcted hi0 w:v in 1.0M NaC1-20mM phosphate buffer (pH 7.4) overnight at 4°C, dialyzed against 100raM NH4HCO3, and lyophilized. A new, non-expesed filter paper was extracted in the same fashion as a negative control. Non-dialyzable solids from each of the three extracts were dissolved in phosphate-buffered saline (100 mg/ml) for testing.

With a goal of improving operational numerical weather prediction 1 (NWP), the Developmental Test... more With a goal of improving operational numerical weather prediction 1 (NWP), the Developmental Testbed Center (DTC) has been working with the operational 2 centers, including NCEP, NOAA, NASA, the Air Force, etc., to support numerical 3 models/systems and their research, perform objective testing and evaluation of NWP 4 methods, and facilitate research to operations (R2O) transitions. This article introduces 5 the first attempt of the DTC in the data assimilation area to help achieve this goal. Since 6 2009, the DTC, NCEP's Environmental Modeling Center (EMC), and other GSI 7 developers have made significant progress in transitioning the operational Gridpoint 8 Statistical Interpolation (GSI) data assimilation system into a community-based code 9 management framework. Currently, GSI is provided to the public with user support and is 10 open for contributions from both internal developers as well as the broader research 11 community, following the same code transition procedures. This article introduces 12 measures and steps taken during this community GSI effort followed by discussions of 13 encountered challenges and issues. The purpose of this article is to promote contributions 14 from the research community to operational data assimilation capabilities and, 15 furthermore, to seek potential solutions to stimulate such a transition and eventually, 16 improve the NWP capabilities in the United States.

Natural killer (NK) cell activity was quantitated using 51Cr release from the human erythroleukem... more Natural killer (NK) cell activity was quantitated using 51Cr release from the human erythroleukemia cell line K562 in 39 heterosexual males, 60 asymptomatic homosexuals, 39 patients with persistent generalized lymphadenopathy (PGL), and 16 patients with acquired immunodeficiency syndrome (AIDS). PGL and AIDS patients showed a slight decrease in NK cell activity compared to control groups. Absolute numbers of Leu 11a-positive cells were decreased in PGL and AIDS patients, and this decrease correlated with a decrease in absolute number of both the T4+ and T8+ cell subsets. Autologous plasma inhibited NK cell activity in 48% of asymptomatic homosexuals, 63% of PGL patients, and 63% of AIDS patients, but in none of the heterosexual controls. NK cell responses in fetal calf serum, normal human plasma, or autologous plasma showed no correlation with absolute numbers of T4+ cells, or with T4/T8 ratio. Associates of the Palm Beaches, North Palm Beach, FL #University of Toronto, West Toronto, Canada... more Associates of the Palm Beaches, North Palm Beach, FL #University of Toronto, West Toronto, Canada ABayer Corporation, West Haven, CT Intravenous IgG-Chromatography, 10% (IVIG-C) is manufactured by employing a novel purification process that includes anlonexchange chromatography and an integrated caprylate-based viral inactivation step. A randomized, double-blind, parallel group study of 172 patients with primary immune deficiency (PID) was conducted at 25 sites in the USA and Canada to demonstrate the therapeutic equivalence of IVIG-C to Gamimune N 10%, an intravenous IgG product manufactured using solvent/detergent inactivation (1VIG-S/D). The primary endpoint was the incidence of validated sinopulmonary infections (i.e., acute sinusitis or pneumonia). Baseline characteristics [sex, race, age, weight, dosing interval, initial dose, IgG trough level, and presence of bronchiectasis] were comparable between treatment groups. Fewer patients receiving IVIG-C had at least one validated infection than those receiving IVIG-S/D (9 of 73, 12% vs. 17 of 73, 23%). The annual rate of validated infections was also lower for the 1VIG-C group (0.18 vs. 0.43, p=0.023), although the annual rate of any infection did not differ significantly (2.76 vs. 3.26, p--0.287). The incidence of drug-related adverse events (AEs) per infusion was <2% and was comparable between the products. Fifteen patients receiving 1VIG-C experienced 18 serious AEs and 21 patients receiving 1VIG-S/D experienced 34 serious AEs. Most of these AEs were not considered by investigators to be related to study drug. Vh-al safety monitoring of antigen and PCR markers for HCV, HBV, HIV and Parvovirus B 19 showed no viral transmissions occm'red on study. The current study, the largest pivotal trial ever conducted for licensure of an 1VIG product for use in PID, demonstrated IG1V-C to be at least as effective and safe as IVIG-S/D for IgG replacement therapy.
