Nosocomial pneumonia caused by multidrug-resistant (MDR) Pseudomonas aeruginosa is becoming incre... more Nosocomial pneumonia caused by multidrug-resistant (MDR) Pseudomonas aeruginosa is becoming increasingly prevalent throughout the world. The use of polymyxins to treat these infections has greatly increased. We analysed 74 patients with nosocomial pneumonia caused by MDR P. aeruginosa who were treated with polymyxin B. A favourable outcome was observed in 35 patients (47.3%). A case-control study was performed to assess the variables associated with an unfavourable outcome. The presence of acute respiratory distress syndrome (odds ratio (OR) = 11.29, 95% confidence interval (CI) 2.64-48.22; P = 0.001) and septic shock (OR = 4.81, 95% CI 1.42-16.25; P = 0.01) were independently associated with an unfavourable outcome in patients with nosocomial pneumonia due to MDR P. aeruginosa. Our study demonstrated that polymyxin B is a reliable antimicrobial drug, but only as salvage therapy, for nosocomial pneumonia caused by MDR P. aeruginosa.

The guidelines are found in a dynamic model and its optimality conditions. The instrumental is used to model a supply chain that adopts the ECR. The purpose is to determine the impacts on the financial management and on the structure of the retail market that arise with the ECR adoption. It's shown up the inter-relations among the several aspects that must be considered in the implementation of a supply partnership.

Objectives: The aim of this study was to evaluate the risk factors and attributable mortality ass... more Objectives: The aim of this study was to evaluate the risk factors and attributable mortality associated with imipenem-resistant Pseudomonas aeruginosa (IRPA) infections in a medical-surgical intensive care unit (ICU). Methods: A retrospective case-control study was carried out at a 16-bed medical-surgical ICU in a 780bed, university-affiliated hospital. All patients admitted from January 1, 2003, to December 31, 2004, who had nosocomial infection caused by IRPA, were included in the study. Results: Imipenem-resistant P. aeruginosa was recovered from 63 patients during the study period. One hundred eighty-two controls were matched with cases by period of admission, age, and time at risk. Urinary tract (34.9%) and respiratory tract (22.2%) were the main sources of IRPA isolation. In multivariate analysis, a previous stay in the ICU (odds ratio, 3.54; 95% confidence interval [CI], 1.29-9.73; P = .03) was the only independent risk factor for IRPA infection. The in-hospital mortality rate among case patients was 49% (31 of 63) compared with 33% (61 of 182) for control patients (odds ratio, 1.92; 95% CI, 1.07-3.44; P = .02). Thus, we had an attributable mortality of 16% (95% CI, 9.74%-22.3%; P = .03). Conclusions: Our study suggests that IRPA infections are strongly related to previous ICU stay, and that IRPA infections significantly increase mortality in those critical patients.

Ocorre em 4 a 13% dos pacientes com polipose adenomatosa familiar (FAP). Apesar de histologicamente benignos, os TD têm comportamento maligno, sendo localmente invasivos e com elevada recorrência após ressecção. Os autores relatam um caso de tumor desmóide tratado cirurgicamente no Hospital Governador Israel Pinheiro -IPSEMG e fazem revisão da literatura sobre o tratamento. Purpose: to make a differential diagnosis in regard to breast carcinoma and to evaluate diagnosti... more Purpose: to make a differential diagnosis in regard to breast carcinoma and to evaluate diagnostic and clinical methods in the treatment of breast tuberculosis and the follow-up after adequate treatment. Patients and Methods: three patients with breast tuberculosis were observed from March 2001 to March 2002; the first two were hospitalized at our Mastology Department and the third patient was treated at a private clinic. The clinical signs and symptoms, laboratory findings, response to therapy and follow-up were evaluated. Results: the average age of the patients was 40.6 years. The most frequent signs and symptoms were pain and breast tumor. In two patients the presumptive diagnosis was based on the clinical findings, on the histological findings (granulomatous inflammatory process), and on the therapeutic response to tuberculostatic drugs. Only one patient had a microbiological diagnosis, as Koch's bacillus was identified in a sample of her breast tissue. Treatment with a triple tuberculostatic regimen, including rifampin, isoniazid and pyrazinamide, led to the regression of the lesions. Conclusion: primary breast tuberculosis, a rare occurrence which may present clinically as a breast nodule and radiologically as carcinoma, should be taken into account when making the differential diagnosis of patients presenting with mammary mass.

The proposed process is comprised of four phases supported by the DoD Std 2167 A. It suggests the use of UML notation during the model elaboration. In order to verify the process, it was applied on an actual project named Amazon Integration and Cooperation for Modernization of Hydrological Monitoring, under the development of the Brazilian Aeronautics Institute of Technology (Instituto Tecnológico de Aeronáutica -ITA). The main contributions of this paper are the proposed system use cases identification process and the validation procedure performed, comparing the obtained results with those results from a traditional approach.
