Obesity is a major worldwide health problem because it is strongly associated with insulin resist... more Obesity is a major worldwide health problem because it is strongly associated with insulin resistance, type 2 diabetes, atherosclerosis, and ischemic heart disease—conditions that are associated with reduced life expectancy and impressive economic and societal consequences. The impact of obesity in the critically ill patient may vary by the type of critical illness, obesity severity (obesity distribution), and obesity associated comorbidities. Obese patients in the intensive care unit are at higher risk for acute cardiovascular complications (congestive heart failure, atrial fibrillation), pulmonary complications (respiratory failure), and renal complications (acute kidney injury). Obesity also represents a challenge in the management of invasive mechanical ventilation, hemodynamic monitoring, and pharmacokinetic dose adjustments. Sometimes this particular patient group may need tracheostomy. This surgical procedure is capable of saving lives. However, in obese patients, some care regarding the position to perform the procedure and cannula selection, for example, can minimize complications. Unfortunately, clinical investigations addressing the care of tracheostomy in obese patients are scare.

Tracheostomy is one of the most common procedures in intensive care units (ICUs), and bronchoscopy is crucial in the study of respiratory diseases. Among the different types of bronchoscopy, flexible bronchoscopy is the most widely used technique, although there are other types, such as rigid and ultrasound-guided bronchoscopy. There are diagnostic and therapeutic indications for performing bronchoscopy. One of the most common, within the ICU, is to aid percutaneous tracheostomy. Others indications include evaluation of complications after respiratory tract surgery, including tracheostomy; aspiration, collection, and tracheobronchial biopsies; aspiration of foreign bodies; evaluation of the adequacy of the tracheostomy cannula; part of the protocol for tracheostomy decannulation; and diagnosis of synchronic and metachronous lesions of the distal airways. To evaluate p63 expression in laryngeal squamous cell carcinoma and its prognostic significance. ... more To evaluate p63 expression in laryngeal squamous cell carcinoma and its prognostic significance. p63 expression was examined by immunohistochemistry and scored in 127 patients with laryngeal squamous cell carcinomas. Sixty-two cases had scored 3, sixty had scored 2, four had scored 1 and one case did not show any expression (48.8, 47.2, 3.1 and 0.8%, respectively). Overall survival was 73.9% at 24 months and 59.5% at 60 months. The disease-free survival was 77.2 and 75.1%, and the disease-specific survival was 79 and 67% at 24 and 60 months, respectively. Uni- and multivariate analysis identified that decreased immunoexpression of protein p63 was a statistically significant factor for the risk of recurrence and death by cancer. p63 expression was highly prevalent in laryngeal squamous cell carcinomas, and its underexpression was correlated with a worse prognosis.
