O objetivo deste estudo foi caracterizar a qualidade da atenção básica com base nas respostas de ... more O objetivo deste estudo foi caracterizar a qualidade da atenção básica com base nas respostas de profissionais e usuários, focando no tipo de médico – inserido ou não no Programa Mais Médicos (PMM). Realizou-se um estudo transversal aplicando o Questionário de Fortalecimento da Atenção Básica a 149 médicos e 795 usuários em unidades básicas de saúde de Pernambuco, Brasil, de fevereiro a novembro de 2018. Neste estudo, foi utilizado o conceito de provimento do PMM no qual foi previsto a destinação do profissional médico para área de vulnerabilidade social. Os médicos foram divididos em três subgrupos: brasileiros não PMM, brasileiros PMM e cubanos PMM. Foram gerados escores médios a partir da perspectiva dos profissionais e dos usuários por meio de análise bivariada e multivariada dos subgrupos dos médicos com variáveis sociodemográficas, educacionais e atributos da atenção primária, considerando-se o nível de significância de 5%. Médicos cubanos do PMM apresentaram os maiores escores médios, especialmente no atributo orientação familiar e comunitária (7,19), seguido pelo subgrupo dos brasileiros do programa (4,74). A análise multivariada mostrou significância para médicos cubanos, sexo feminino e profissionais com residência médica. Entre os usuários, observou-se maior associação com a qualidade da atenção básica no subgrupo dos médicos cubanos, com exceção da longitudinalidade. Os resultados indicam a efetividade dos médicos cubanos do PMM na atenção básica, ressaltando a necessidade de focar na centralidade do usuário. Os achados reforçam a importância de utilizar instrumentos de avaliação precisos e abrangentes na gestão da saúde pública.

This study summarizes the findings of a training needs and priority assessment completed in Haiti. Its objective is to describe the characteristics of nursing and allied professions providing first level maternal health care and identify training needs and priorities to inform planning of Human Resources for Health interventions. Methods. A cross-sectional survey was completed between October 2016 and March 2017 by the Pan American Health Organization/World Health Organization Haiti office in collaboration with national health authorities. Participants reached consensus to submit one finalized version of the survey. Data were collected on composition, capacities, and training needs and priorities of traditional birth attendants, community health workers, registered nurses, professional midwives, and auxiliary nurses. Results. Haiti relies heavily on community level workers including community health workers, auxiliary nurses, and traditional birth attendants. Traditional birth attendants attend the majority of Haiti's births, despite having low education levels and not being regulated by the Ministry of Public Health and Population. All professional categories prioritize preventive capacities such as timely identification of complications, while none are trained to manage postpartum hemorrhage, preeclampsia, or eclampsia. Management of obstetric emergencies is a training priority for Haiti but is not part of the scope of work of the nursing and allied health professions included in this study. Conclusions. Community level health workers are key in providing preventive care and referral of complicated pregnancies, but lack of access to providers qualified to treat obstetric complications remains a challenge to reducing maternal mortality.

There are several obstacles to access universal, comprehensive, good-quality health services in t... more There are several obstacles to access universal, comprehensive, good-quality health services in the Region of the Americas (1). Despite advances in economic and social development and the consequent strengthening of health systems in the Region, collaboration across health, education, and labor sectors needs to be better aligned to promote education that will prepare professionals to meet the health needs of the population and the countries (3-4). Interprofessional collaboration is a promising strategy for mitigating the workforce crisis and improving health care that, if carried out by interprofessional health teams, enables optimization of skills and holistic, high-quality, people-centered care (5-6). Effective collaboration among health team members requires health professional education based on the interprofessional education (IPE) approach. Objective. To describe and analyze the current nursing regulations across countries in the Region... more Objective. To describe and analyze the current nursing regulations across countries in the Region of the Americas. Methods. A country comparative analysis was carried out by gathering information from the ministries of health, nursing schools, councils, associations, and boards in 2018. The main categories evaluated were type of regulatory bodies, requirements for initial professional registration, and registration renewal. Results. All countries regulate the nursing profession through a regulatory body. Competency exams for initial registration are required in the United States, Canada, and most Caribbean countries. Registration renewal is required in 54.3% of the countries. Continuing education is required for professional registration renewal in the United States, Canada, and 53% of Caribbean countries. Labor hours are required in the United States and Canada. Conclusion. Regulations promote and protect professional integrity. The Pan American Health Organization/ World Health Organization recommends that countries make efforts to evaluate competency and training, consider the use of competency exams for initial registration, and add continuing education as requirements for registration renewal.

We have used the technique of dynamic capillaroscopy to study the time-course of photodynamic vas... more We have used the technique of dynamic capillaroscopy to study the time-course of photodynamic vascular occlusion in mice injected intrapentoneally with either of two photosensitizers; hematoporphyrin esters (HPE) or meso-tetrahydroxyphenyl chlorin (mTHPC). The peak of vascular occlusion induced by HPE coincided in time with peak serum levels of this photosensitizer (about 3 h after injection). However, there was also a second peak of occlusive activity at about 12 h after injection, at which time serum HPE was falling monotonically. In the case of mTHPC, no peak of occlusive activity was seen at 3 h after injection, even though the serum levels of this photosensitizer, like those of HPE, were highest around this time. Instead, a steady rise in photosensitizing activity was observed, peaking at 1 1 h. This decoupling between serum drug levels and vascular photosensitization-partial for HPE and complete for mTHPC-suggests that direct photosensitization of endothelial cells is unlikely wholly to explain the vascular collapse. Instead, there must be either another compartment that accumulates photosensitizer more slowly and in which photodynamic activity has an indirect effect on the blood capillaries or a slow metabolic transformation of mTHPC into a more active sensitizer.

Emotional detachment and cynicism are defense mechanisms, which can emerge as a consequence. The consolidation of a professional identity committed to patients' interests can become a challenge when medical students are not comfortable in their role as caregivers. Na Aprendizagem Baseada em Problemas (ABP), o tutor representa um papel crucial como facilitador do processo de aprendizagem. A maioria dos professores envolvidos na educação médica como tutores na ABP teve sua formação baseada em aulas teóricas, com pouquíssimos modelos profissionais de tutoria, gerando desconforto e dúvidas com relação ao seu novo papel. Com o objetivo de identificar as características que conferem maior efetividade ao tutor em ABP, fez-se uma revisão da literatura publicada nas bases de dados Pubmed, Embase, Lilacs, SciELO e Eric de 1999 a 2013. RESUMO A tutoria educacional, desenvolvida eminentemente em caráter individual, é considerada pad... more RESUMO A tutoria educacional, desenvolvida eminentemente em caráter individual, é considerada padrão ouro em educação quando comparada a outras estratégias de ensino. Na Aprendizagem Baseada em Problemas (ABP), o tutor representa um papel crucial como facilitador do processo de aprendizagem. A maioria dos professores envolvidos na educação médica como tutores na ABP teve sua formação baseada em aulas teóricas, com pouquíssimos modelos profissionais de tutoria, gerando desconforto e dúvidas com relação ao seu novo papel. Com o objetivo de identificar as características que conferem maior efetividade ao tutor em ABP, fez-se uma revisão da literatura publicada nas bases de dados Pubmed, Embase, Lilacs, SciELO e Eric de 1999 a 2013. Estão abrangidos na pesquisa estudos quantitativos e qualitativos que incluem instrumentos de avaliação de tutores em PBL e indicativos de sua efetividade.
