This paper presents a definition and framework for a novel type of adaptive data network: the cog... more This paper presents a definition and framework for a novel type of adaptive data network: the cognitive network. In a cognitive network, the collection of elements that make up the network observes network conditions and then, using prior knowledge gained from previous interactions with the network, plans, decides and acts on this information. Cognitive networks are different from other "intelligent" communication technologies because these actions are taken with respect to the end-to-end goals of a data flow. In addition to the cognitive aspects of the network, a specification language is needed to translate the user's end-to-end goals into a form understandable by the cognitive process. The cognitive network also depends on a software adaptable network that has both an external interface accessible to the cognitive network and network status sensors. These devices are used to provide control and feedback. The paper concludes by presenting a simple case study to illustrate a cognitive network and its framework

A case-control study of risk factors for carcinomas of the tongue, gum, floor, and other specifie... more A case-control study of risk factors for carcinomas of the tongue, gum, floor, and other specified parts of the mouth was conducted in 3 metropolitan areas in Brazil: São Paulo (South-east), Curitiba (South), and Goiãnia (Central-West). We analyzed information on demographics, occupational history, environmental exposures, tobacco smoking and alcohol drinking habits, as well as diet, oral and other health characteristics obtained from interviews with 232 cases and 464 hospital non-cancer controls matched for 5-year age-group, sex, hospital catchment area and trimester of admission. Tobacco and alcohol consumption were the strongest risk factors irrespective of the anatomical site. The adjusted relative risks (RR) for ever vs. never smokers were: 6.3, 13.9, and 7.0, for industrial-brand cigarettes, pipe, and hand-rolled cigarettes, respectively. A strong correlation was seen between number of pack-years and risk. The RR for the heaviest vs. the lowest consumption categories (>100 vs. <1 pack-years) was 14.8. Risk levels generally decreased to those of never smokers after 10 years had elapsed since stopping smoking. The risk associated with alcohol was mostly evident for wine (cancer of the tongue) and “cachaça” (all sites), a hard liquor distilled from sugar cane. Other important risk factors were drinking “chimarrão” (a type of maté), use of a wood stove for cooking, and frequent consumption of charcoal-grilled meat and manioc. Oral hygiene characteristics represented correlates of disease risk. A significant protective effect was observed for consumption of carotene-rich vegetables and citric fruits, but not for green vegetables in general.

Meanwhile, such designs are especially ill suited for important commercial applications, such as on-line transaction processing (OLTP), which suffer from large memory stall times and exhibit little instruction-level parallelism. Given that commercial applications constitute by far the most important market for high-performance servers, the above trends emphasize the need to consider alternative processor designs that specifically target such workloads. The abundance of explicit thread-level parallelism in commercial workloads, along with advances in semiconductor integration density, identify chip multiprocessing (CMP) as potentially the most promising approach for designing processors targeted at commercial servers.

Commercial applications such as databases and Web servers constitute the largest and fastest-grow... more Commercial applications such as databases and Web servers constitute the largest and fastest-growing segment of the market for multiprocessor servers. Ongoing innovations in disk subsystems, along with the ever increasing gap between processor and memory speeds, have elevated memory system design as the critical performance factor for such workloads. However, most current server designs have been optimized to perform well on scientific and engineering workloads, potentially leading to design decisions that are non-ideal for commercial applications. The above problem is exacerbated by the lack of information on the performance requirements of commercial workloads, the lack of available applications for widespread study, and the fact that most representative applications are too large and complex to serve as suitable benchmarks for evaluating trade-offs in the design of processors and servers. This paper presents a detailed performance study of three important classes of commercial workloads: online transaction processing (OLTP), decision support systems (DSS), and Web index search. We use the Oracle commercial database engine for our OLTP and DSS workloads, and the AltaVista search engine for our Web index search workload. This study characterizes the memory system behavior of these workloads through a large number of architectural experiments on Alpha multiprocessors augmented with full system simulations to determine the impact of architectural trends. We also identify a set of simplifications that make these workloads more amenable to monitoring and simulation without affecting representative memory system behavior. We observe that systems optimized for OLTP versus DSS and index search workloads may lead to diverging designs, specifically in the size and speed requirements for off-chip caches.

Coronary-stent implantation is frequently performed for treatment of acute myocardial infarction.... more Coronary-stent implantation is frequently performed for treatment of acute myocardial infarction. However, few studies have compared stent implantation with primary angioplasty alone. We designed a multicenter study to compare primary angioplasty with angioplasty accompanied by implantation of a heparin-coated Palmaz-Schatz stent. Patients with acute myocardial infarction underwent emergency catheterization and angioplasty. Those with vessels suitable for stenting were randomly assigned to undergo angioplasty with stenting (452 patients) or angioplasty alone (448 patients). The mean (+/-SD) minimal luminal diameter was larger after stenting than after angioplasty alone (2.56+/-0.44 mm vs. 2.12+/-0.45 mm, P&amp;amp;amp;amp;lt;0.001), although fewer patients assigned to stenting had grade 3 blood flow (according to the classification of the Thrombolysis in Myocardial Infarction trial) (89.4 percent, vs. 92.7 percent in the angioplasty group; P=0.10). After six months, fewer patients in the stent group than in the angioplasty group had angina (11.3 percent vs. 16.9 percent, P=0.02) or needed target-vessel revascularization because of ischemia (7.7 percent vs. 17.0 percent, P&amp;amp;amp;amp;lt;0.001). In addition, the combined primary end point of death, reinfarction, disabling stroke, or target-vessel revascularization because of ischemia occurred in fewer patients in the stent group than in the angioplasty group (12.6 percent vs. 20.1 percent, P&amp;amp;amp;amp;lt;0.01). The decrease in the combined end point was due entirely to the decreased need for target-vessel revascularization. The six-month mortality rates were 4.2 percent in the stent group and 2.7 percent in the angioplasty group (P=0.27). Angiographic follow-up at 6.5 months demonstrated a lower incidence of restenosis in the stent group than in the angioplasty group (20.3 percent vs. 33.5 percent, P&amp;amp;amp;amp;lt;0.001). In patients with acute myocardial infarction, routine implantation of a stent has clinical benefits beyond those of primary coronary angioplasty alone.
