The hypothalamic-pituitary-gonadal (HPG) axis has an important role in the normal process of puberty. Disruption in the HPG axis causes hormonal disturbances, one of which is hypogonadotropic hypogonadism, which is associated with delayed puberty. Hormonal disorders can also cause various problems besides the reproductive system, such as muscle weakness, lack of energy, and an increased risk of bone fractures. We report the case of a male patient who complained of predominant muscle weakness, did not show signs and symptoms of puberty, and had an idiopathic hypogonadotropic hypogonadism who unresponsive to human chorionic gonadotropin (hCG) therapy. We decided to administered Testosterone Undecanoate (TU) therapy to overcome the problem of delayed puberty and complaints of muscle weakness experienced by the patient. Testosterone stimulate erection process physiologically. The study showed that black pepper extract increased sexual performance and testosterone level in animal research. The purpose of this study was to know the erotogenic effect, testosterone serum and concentration of brain androgen receptor of ethanolic extract of black pepper fruits. 25 adult male Wistar rats were divided into 5 groups: Na-CMC orally, TU/testosterone undecanoate (Andriol® Testocaps™) 4 mg/kg of body weight (BW) orally, the three groups were given of ethanolic extract of P. nigrum 3.33 mg, 6.66 mg and 13.32 mg/kg BW respectively for 55 days. Penile rat erection were observed by Total Penile Reflex (TPR). The measurement of testosterone serum and brain androgen receptor concentration used was ELISA method. Erection capability in the TU, P. nigrum 3.33 mg, 6.66 mg and 13.32 mg groups were increased until the 5 th week observation and decreased after that. Na-CMC group erection capability increased gradually from 1 st week until 8 th week observation. Testosterone level in the Na-CMC and P. nigrum 6.66 mg decrease continually in the 4 th and 8 th week measurement. In TU group, P. nigrum 3.33 mg and 13.32 mg groups of testosterone level were increased in the 4 th week and decreased in the 8 th week. Brain androgen receptor concentration of the TU, P. nigrum 3.33 mg, 6.66 mg and 13.32 mg groups were lower than Na-CMC group. Extract of black pepper fruit had erotogenic effect on the increase of testosterone level of male Wistar rats.

Erectile dysfunction is one of the male sexual problems that occur globally and is reported to affect men's quality of life. Herbal plants have been widely used for disease treatment, including the problem of erectile dysfunction. This paper aims to review the molecular potential of various plants in the physiology of erection and to treat erectile dysfunction. The literature search was carried out through the Pubmed and Google Scholar databases regarding the molecular mechanisms of herbal plants and their potential involvement in the physiology of erection and overcoming erectile dysfunction. This paper focuses on six herbal plants: Panax ginseng, Ginkgo biloba, Epimedium, Black pepper, Tribulus terrestris, and Eurycoma longifolia. The six herbal plants have involvement in the erection process and have molecular potential in the treatment of erectile problems

Background: Leukocytospermia is defined as the presence of leukocytes in semen at the concentrati... more Background: Leukocytospermia is defined as the presence of leukocytes in semen at the concentration of ≥ 1 x 10 6 /mL according to World Health Organization (WHO) manual procedure. Leukocytes are the source of reactive oxygen species (ROS), which may lead to oxidative stress conditions and sperm damage. This study aims to evaluate the association between leukocytospermia, poor semen quality, and infertility at Sadewa Maternal and Child Hospital, Yogyakarta, Indonesia. Methods: This study was based on secondary data of semen analyses of males attending IVF clinics at Sadewa Maternal and Child Hospital, Yogyakarta, Indonesia, from May 2015 to November 2020. Variables assessed in this study were semen volume, sperm concentration, progressive motility, and Total Motile Sperm Count (TMSC). Data were analyzed using SPSS version 23 for Windows. Results: The median sperm concentration of the leukocytospermia group was higher than that of the nonleukocytospermia group. There is a relationship between the number of leukocytes in semen and TMSC, with the higher TMSC median in the leukocytospermia group. Conclusion: This is the first study to report a significant association between leukocytospermia and total motile sperm count (TMSC). The association is not expected with leukocytospermia patients who have better TMSC value.

Pubertas merupakan serangkaian proses perkembangan fisik dan maturasi seksual sehingga dapat tercapai kapasitas reproduksi. Aksis hipotalamuspituitari-gonadal (HPG) memiliki peran penting dalam keberlangsungan proses normal pubertas. Gangguan pada kerja aksis HPG menyebabkan gangguan hormonal salah satunya hipogonadotropik hipogonadisme yang berkaitan dengan pubertas tertunda. Gangguan hormonal juga dapat menyebabkan berbagai keluhan di luar sistem reproduksi, seperti kelemahan otot, kekurangan energi, dan peningkatan risiko fraktur tulang. Kami melaporkan kasus pasien pria dengan keluhan kelemahan otot yang dominan, tidak menunjukkan kemunculan tanda dan gejala pubertas, dan mengalami kondisi hipogonadotropik hipogonadisme idiopatik yang tidak berespon terhadap terapi human chorionic gonadotropin (hCG). Terapi Testosteron Undecanoate (TU) diberikan untuk mengatasi masalah keterlambatan pubertas dan keluhan kelemahan otot yang dialami pasien. Terapi TU telah menunjukkan efek menguntungkan pada pasien ini dengan meningkatkan kadar testosteron, meningkatkan panjang penis, dan menghilangkan keluhan kelemahan otot.
