Gejala nyeri dada khas tertentu karena keadaan iskemik akibat dari insufisiensi koroner akibat pe... more Gejala nyeri dada khas tertentu karena keadaan iskemik akibat dari insufisiensi koroner akibat penyumbatan oleh trombus arteri koroner disebut Sindroma Koroner Akut (SKA)1 yang dibedakan dalam tiga keadaan yaitu 1. Angina pektoris tidak stabil; 2. Infark miokard tanpa peningkatan gelombang ST; 3. Infark miokard dengan peningkatan gelombang ST.1 Cardiac Troponin (cTn) salah satu petanda iskemia miokardiak.6 Tautan klinis sangat penting untuk penafsiran nilai cTn, karena tidak setiap peningkatan cTn adalah infark miokard akut (IMA). T... more Brain injury becomes worldwide public health issue since it may cause disability and mortality. The diagnosis of brain injury is made based on clinical neurology examination, and CT scan or MRI. Serum S100 protein examination in mild and moderate brain injury patients is needed to detect and evaluate the presence of post traumatic brain injury. This research subject is healthy people and patients with mild and moderate brain injury, based on their GCS grade, clinical neurologic examination, and CT scan. On these patients, the blood for S100 protein examination is taken at admission, 6 and 24 hours post trauma, and last day of hospitalization. Examination of a serum S100 protein is conducted using ECLIA method. There is significant difference (p = 0,001) in mild or moderate brain injury patients in all serum S100 protein. Background : Prediabetic patients are considered as prone to cardiometabolic risk because they ca... more Background : Prediabetic patients are considered as prone to cardiometabolic risk because they can progress to diabetes mellitus (DM) and cardiovascular disease (CVD). Among lipid lipoprotein risk factors, sd-LDL is the most atherogenic lipoprotein. Triglyceride/HDL-cholesterol (Tg/HDL-C) ratio is considered parallel with sd-LDL level. Nevertheless, there is no data about sd-LDL level and Tg/HDL-C ratio in prediabetic and normoglycemic subgroups of first-degree relatives from patients with type 2 diabetes mellitus (T2DM) in Indonesia. The aim of this study was to prove that prediabetic subgroup have higher sd-LDL level compared to normoglycemic subgroup and Tg/HDL-C ratio is correlated well with sd-LDL level. Materials and methods : This cross sectional study is consisted of 96 subjects of first-degree relatives from patients with T2DM in Cipto Mangunkusumo General Hospital, Jakarta, which was divided to prediabetic and normoglycemic subgroups of 48 subjects each. Prediabetes was diagnosed according to ADA 2003 revised criteria. sd-LDL was measured as sd-LDL-cholesterol (sd-LDL-C) quantitatively by direct homogenous enzymatic method. Triglyceride was measured by enzymatic method and HDL-C was measured by direct homogenous enzymatic method. Results : Both sd-LDL-C level and Tg/HDL-C ratio were significantly higher in prediabetic subgroup compared to normoglycemic subgroup [30.5 (14.4 13 72.0) mg/dL vs 19.0 (10.1 13 34.0) mg/dL); p<0.001 and 2.70 (0.77 13 6.63) vs 1.49 (0.86 13 3.16); p<0.001]. There was significantly strong correlation between sd-LDL-C level and Tg/HDL-C ratio [r=0.806, p<0.001]. Conclusions : We concluded that among first-degree relatives from patients with T2DM, the prediabetic subgroup has higher CVD risk compared to normoglycemic subgroup based on sd-LDL-C level and Tg/HDL-C ratio. It is recommended to measure sd-LDL-C level in prediabetic patients. If sd-LDL-C level examination is unavailable, Tg/HDL-C ratio can be used as a substitute parameter. Therefore, early and accurate diagnosis of acute coronary syndrome is required to determine the next clinical step. The current gold standard for cardiac markers, troponin and CK-MB have their downside. The delayed increase of detectable circulating level of these markers contribute to delayed diagnosis and therapy. Novel biomarkers that rise earlier, has a good diagnosis accuracy and has additional prognostic information, are highly needed. There are some potential emerging novel biomarkers for acute myocardial infarction. High sensitivity troponin have a greater sensitivity and accuracy for detection and early exclusion of myocardial infarction, as compared to troponin. B-natriuretic peptide (BNP and NT-pro BNP) provide prognostic information in regards of mortality. Lipoprotein (a) adalah suatu lipoprotein plasma yang mempunyai struktur dan komposisi yang mirip ... more Lipoprotein (a) adalah suatu lipoprotein plasma yang mempunyai struktur dan komposisi yang mirip dengan lipoprotein berdensitas rendah (LDL) dengan tambahan apo(a) yang terikat pada apo B 100. Struktur apo(a) mirip dengan plasminogen, suatu proenzim dalam sistem fibrinolitik. Oleh karena kemiripan ini, diduga Lp(a) dapat menghambat aktivitas plasminogen dan menurunkan aktivitas fibrinolitik. Penelitian ini bertujuan untuk membuktikan bahwa penambahan Lp(a) ke dalam plasma normal dapat menghambat aktivitas fibrinolitik. Subyek penelitian terdiri atas 4 orang sehat dengan kadar fibrinogen, aktivitas plasminogen dan masa lisis bekuan euglobulin dalam batas normal. Pada percobaan pertama, penambahan Lp(a) dilakukan sebelum sentrifugasi untuk memperoleh endapan euglobulin, sedang pada percobaan kedua Lp(a) ditambahkan pada endapan euglobulin. Sebagai kontrol, masa lisis bekuan euglobulin dikerjakan pada plasma yang ditambahkan NaCl 0,9% dengan volume yang sama seperti Lp(a). Hasil penelitian menunjukkan bahwa pada percobaan pertama tidak ada bekuan yang terbentuk. Diduga Lp(a) dapat mengikat fibrinogen dan keduanya berada di supernatan, sehingga tidak ada fibrinogen dalam endapan euglobulin yang dapat dibekukan oleh trombin. Pada percobaan kedua, sampai hari ke empat bekuan belum lisis.
