This study was conducted on 300 patients with infiltrative breast carcinoma, of which 112 cases u... more This study was conducted on 300 patients with infiltrative breast carcinoma, of which 112 cases underwent radical mastectomy, and 188 were conservatively operated. Forty-four patients experienced local relapses during the clinical follow-up. The immunohistochemical (IHC) study used the Labeled Streptavidin Biotin (LSAB)∕Horseradish peroxidase (HRP) method, which evaluated the estrogen receptor (ER) and progesterone receptor (PR), as well as human epidermal growth factor receptor 2 (Her2∕neu) expressions in tumor cells. After IHC analysis, all cases were classified into one of the following four immunophenotypes: ER+∕PR+ classical phenotype, ER-∕PR-, ER+∕PR- and ER-∕PR+ phenotypes. Correlating hormonal phenotypes with Her2 status, we found that Her2 is significantly associated more frequently with the ER+/PR- phenotype, while the absence of Her2 was associated more frequently with the ER+/PR+ phenotype but with no significant differences. Local recurrences were significantly more common in patients with absent ERs than those who had these receptors present in the primary tumor. Similarly, mammary tumors with absent PRs recurred significantly more frequently than those with PRs. The positivity of Her2 is significantly associated with the absence of PRs in both type of surgery (conservative or radical). Local recurrences are significantly correlated with both negative receptors phenotype, regardless of the type of surgery. IHC markers, represented by hormone receptors for estrogen and progesterone, and Her2 oncoprotein, can be useful for identifying patients who are at increased risk of local recurrences after conservative or radical surgery for breast cancer.

We report the case of a 49-year-old man who presented in our clinic with nonspecific symptoms and significant nitrogen retention, with ascites fluid detected during the clinical–paraclinical examination, ultimately confirming the diagnosis of bacillary peritonitis. Confirmation of tuberculous etiology through biopsy and/or bacteriological examination is sovereign for the diagnosis. The therapeutic protocol includes three anti-tuberculostatic drugs, for a period of at least 6 months, with or without the combination of corticosteroid therapy during the first months of treatment. Background: The specific mechanism of action of each anesthetic drug on the immune system is stil... more Background: The specific mechanism of action of each anesthetic drug on the immune system is still incompletely known. It is important to know how the various anesthetics used in minimally invasive surgery (MIS) act on the inflammatory response because the choice of the anesthetic agent can influence the patient's immune system. Aim: Evaluation of the effect of anesthetic drugs used for total intravenous anesthesia (Propofol and Midazolam) on the inflammatory response after minimally invasive gynecological surgery. Patients, Materials and Methods: The inflammatory response in 20 female patients who underwent minimally invasive gynecological surgery under which intravenous anesthesia was performed. Depending on the combination of anesthetics used, we subdivided the study group into two groups, Group 1 consisting of the patients (n=10) who were given for total intravenous anesthesia, the combination with Midazolam+Fentanyl, and Group 2 (n=10) the patients who received the combination of Propofol+Fentanyl, respectively. Surgical interventional procedures included day surgery: diagnostic and operative hysteroscopy, endometrial ablation, surgical treatment of vulvar disorders. Serological profiling of patients was performed by dosing the serum concentration of nucleotide-binding domain (NOD) and leucine-rich repeat protein 3 (NLRP3) inflammasomes, interleukin (IL)-6, tumor necrosis factor-alpha (TNF-α), IL-10 before and two hours after the surgical procedure. Results: In our study, we found that in both groups of patients (Midazolam+Fentanyl-Group 1, Propofol+Fentanyl-Group 2), NLRP3 and cytokines concentrations in the serum were higher after MIS than those before MIS. Conclusions: It appears that both Midazolam and Fentanyl and Propofol and Fentanyl have an immunomodulatory action due to the anti-inflammatory effect of both anesthetics. Therefore, anesthesiologists must choose an anesthetic method that uses individualized anesthetic agents, depending on the patient's immune status and disease.

Cardiovascular disease (CVD) is recognized as a leading cause of death worldwide. Obesity, dyslip... more Cardiovascular disease (CVD) is recognized as a leading cause of death worldwide. Obesity, dyslipidemia, insulin resistance (IR), interconnected pathological conditions constitute risk factors that are closely associated with CVD. The aim of the present study was to highlight the association of IR with cardiovascular risk (CVR). The epidemiological, cross-sectional, non-interventional study was conducted over 12 months (2019-2020) within a research grant and included a sample of 400 subjects divided into 2 subgroups: group 1 (control) subjects did not have diabetes (n=200) and group 2 had type 2 diabetes (T2DM) (n=200). The Framingham risk score (FRS) was calculated according to the 2008 general CVD risk model from the Framingham Heart Study. Subsequent to a correlation of the value of homeostasis model assessment of insulin resistance (HOMA-IR) with the degree of CVR, the IR was higher in both groups, and CVR also increased. After being quantified by the Spearman correlation coefficient, the correlation in group 2 was higher at 0.625 compared to group 1 where this coefficient had a value of 0.440. A high FRS (FRS of 20%) was significantly associated with IR. The results therefore show that HOMA-IR is an independent risk factor for high FRS. New therapies focused on decreasing IR may contribute to decreased CVD.

This is a clinical study on 56 subjects included in normal weight (NW) group (N=17), overweight (... more This is a clinical study on 56 subjects included in normal weight (NW) group (N=17), overweight (OW) group (N=19) and grade I obese (O) group (N=20), based on BMI (Body Mass Index) values: NW group had a mean BMI of 22.2 � 2.14 kg/sqm, OW group had a BMI of 25.89 � 1.04 kg/sqm, and O group had an average BMI of 32.2 � 2.09 kg/sqm (p-value NW-OW, NW-O, respective OW-O groups was p[0.0005). The 3 groups were similar as age (p-value NW-OW groups = 0.7, between NW- O groups = 0.8, respective between OW - O group = 0.7). The circulating bone formation (osteocalcin, P1NP alkaline phosphatase) and resorption profile (CrossLaps) indicated no statistical significant difference between groups while the coefficient of regression r between each biochemical bone marker and BMI in every BMI group exceeded the value of p]0.05. Breast cancer is the most common type of cancer in women worldwide and the second most common can... more Breast cancer is the most common type of cancer in women worldwide and the second most common cancer overall, making it a major international concern from both socio and economical point of view. World Health Organisation (WHO) estimated that worldwide over 508.000 women died in 2011 due to breast cancer. American Cancer Society�s (ACS) study conducted in 2017 showed important numbers about the burden of breast cancer. Therefore, they estimated a number of 252,710 new breast cancer to be diagnosed in women and 2,470 in men in 2017- only in America. On 1st of January , there were about 3.5million US women with a history of breast cancer. The paper presents the correlations between recurrences of the tumor in breast cancer trated with conservative surgery and their immunohistochemical expression.

Global cardio-metabolic risk is the overall risk of developing type 2 diabetes mellitus (T2DM) and/or CVD, including myocardial infarction (MI) and stroke, which is due to a bundle of risk factors. The cardio-metabolic risk is based on the concept of continuous risk. The importance of cardiovascular / cardio-metabolic risk is particular because controlling its components may affect atherogenesis and its clinical consequences: chronic ischemic heart disease, cerebrovascular disease and peripheral arteriopathy, but also diabetes mellitus (DM). Currently there is no method to use all the known risk cardio-metabolic factors to quantify cardiovascular risk or diabetes risk.

Our aim is to analyze the biochemical and clinical profile of patients treated with TPT based on Romanian protocol. The inclusion and exclusion criteria are based on specific country protocol for TPT 20 �g/day, for 2 years, once in life time based on self administration. This is a transversal study including data of a tertiary centre of endocrinology on patients who signed the informed consent. This is a real life study, of observational type (the intervention meaning the TPT recommendation was done by individual decision of each clinician). Metabolic syndrome (MS) is defined as a complex entity that involves the accumulation of cardiova... more Metabolic syndrome (MS) is defined as a complex entity that involves the accumulation of cardiovascular and metabolic risk factors represented by: abdominal obesity, insulin resistance, hypertension, dyslipidemia [1]. The presence of MS correlates with the risk of cardiovascular disease in people without diabetes mellitus (DM), as well as those with type 2 DM [2-8].Visceral adipose tissue is an active metabolic organ and abdominal obesity is an independent risk factor for metabolic disorder present in MS [9,10,11], associated with the development of cardiovascular disease and type 2 diabetes in children, adolescents and adults [12-15]. Our findings have shown an association of visceral adiposity index (VAI) with MS in both men and women with prediabetes and diabetes, these findings allow us to conclude that VAI is a simple but effective indicator for estimating the presence of MS among adults.

This is a clinical cross-sectional and case-control study analyzing these changes after PHP surgical correction in menopausal women. Baseline parameters were: mean age at diagnosis (59.63�9.6 years), TCa of 10.9�0.7 mg/dL, PTH of 138.02�59.36 pg/mL. Longitudinal data showed: final TCa p[0.00001, ICa p[0.00001, phosphorus p[0.0001, magnesium p=0.9, 24-h urinary calcium p=0.4, 25-hydroxycholecalciferol p=0.01, PTH p[0.00001. High circulating parathyrin values due to PHP normalized after surgery in addition to statistical significant changes of TCa, ICa, P, lumbar Bone Mineral Density provided by Dual-Energy X-Ray Absorptiometry; Mg and 24-h Ca might not be a marker of general mineral metabolism improvement.

Whether the sterile valvular inflammation which occurs in this autoimmune disease predisposes to bacterial adhesion and infective endocarditis is not yet established. We present the case of a patient with Behcet disease in which transthoracic echocardiography showed mobile masses on the aortic, tricuspid, and mitral valves, leading to multivalvular infective endocarditis diagnosis, possibly in the context of valvular inflammation. The case presented in this article confirms observation of other studies, namely that ultrasonography plays an important role in the diagnosis and evaluation of rheumatic diseases and permits optimal management in daily practice.

The study aimed to analyze maternal and neonatal complications associated with GDM. The risk factors of GDM and of the maternal and neonatal complications were studied in order to prevent their occurrence. Materials and Methods: The study included 97 women in the study, who underwent an oral glucose tolerance test (OGTT) between weeks 24–28 of pregnancy, consequently being divided into two groups: pregnant women with and without GDM. Urban air pollution is responsible for the highest burden of environmentally related diseases in ... more Urban air pollution is responsible for the highest burden of environmentally related diseases in Europe. Air pollutants with strongly indicated respiratory health effects are particulate matter, nitrogen oxides and sulfur dioxide. The retrospective ecological survey has taken into account the urban air pollution with sulfur dioxide (SO 2), nitrogen dioxide (NO 2), ammonia (NH 3) and particulate matter (PM 10). The analyzed data show that after applying the European regulations regarding the quality of environment air, the level of air pollution with SO 2 , NO 2 and NH 3 was significantly reduced; the atmospheric concentrations of the pollutants were lower than the maximal admissible values. Pollution with PM 10 still had high levels. Calculating the global pollution index, expressing the quality of ambient air by integrating the four surveyed pollutants, suggests that chronic simultaneous exposure to SO 2 , NO 2 , NH 3 and PM 10 from the atmosphere in the Craiova municipality makes for a significant risk towards diseases related to ambient air pollution.

This is a clinical study on 56 subjects included in normal weight (NW) group (N=17), overweight (... more This is a clinical study on 56 subjects included in normal weight (NW) group (N=17), overweight (OW) group (N=19) and grade I obese (O) group (N=20), based on BMI (Body Mass Index) values: NW group had a mean BMI of 22.2 � 2.14 kg/sqm, OW group had a BMI of 25.89 � 1.04 kg/sqm, and O group had an average BMI of 32.2 � 2.09 kg/sqm (p-value NW-OW, NW-O, respective OW-O groups was p[0.0005). The 3 groups were similar as age (p-value NW-OW groups = 0.7, between NW- O groups = 0.8, respective between OW - O group = 0.7). The circulating bone formation (osteocalcin, P1NP alkaline phosphatase) and resorption profile (CrossLaps) indicated no statistical significant difference between groups while the coefficient of regression r between each biochemical bone marker and BMI in every BMI group exceeded the value of p]0.05. World Health Organisation (WHO) estimated that worldwide over 508.000 women died in 2011 due to breast cancer. American Cancer Society�s (ACS) study conducted in 2017 showed important numbers about the burden of breast cancer. Therefore, they estimated a number of 252,710 new breast cancer to be diagnosed in women and 2,470 in men in 2017- only in America. On 1st of January , there were about 3.5million US women with a history of breast cancer. The paper presents the correlations between recurrences of the tumor in breast cancer trated with conservative surgery and their immunohistochemical expression.

Global cardio-metabolic risk is the overall risk of developing type 2 diabetes mellitus (T2DM) and/or CVD, including myocardial infarction (MI) and stroke, which is due to a bundle of risk factors. The cardio-metabolic risk is based on the concept of continuous risk. The importance of cardiovascular / cardio-metabolic risk is particular because controlling its components may affect atherogenesis and its clinical consequences: chronic ischemic heart disease, cerebrovascular disease and peripheral arteriopathy, but also diabetes mellitus (DM). Currently there is no method to use all the known risk cardio-metabolic factors to quantify cardiovascular risk or diabetes risk.

Our aim is to analyze the biochemical and clinical profile of patients treated with TPT based on Romanian protocol. The inclusion and exclusion criteria are based on specific country protocol for TPT 20 �g/day, for 2 years, once in life time based on self administration. This is a transversal study including data of a tertiary centre of endocrinology on patients who signed the informed consent. This is a real life study, of observational type (the intervention meaning the TPT recommendation was done by individual decision of each clinician). The presence of MS correlates with the risk of cardiovascular disease in people without diabetes mellitus (DM), as well as those with type 2 DM [2-8].Visceral adipose tissue is an active metabolic organ and abdominal obesity is an independent risk factor for metabolic disorder present in MS [9,10,11], associated with the development of cardiovascular disease and type 2 diabetes in children, adolescents and adults [12-15]. Our findings have shown an association of visceral adiposity index (VAI) with MS in both men and women with prediabetes and diabetes, these findings allow us to conclude that VAI is a simple but effective indicator for estimating the presence of MS among adults.

This is a clinical cross-sectional and case-control study analyzing these changes after PHP surgical correction in menopausal women. Baseline parameters were: mean age at diagnosis (59.63�9.6 years), TCa of 10.9�0.7 mg/dL, PTH of 138.02�59.36 pg/mL. Longitudinal data showed: final TCa p[0.00001, ICa p[0.00001, phosphorus p[0.0001, magnesium p=0.9, 24-h urinary calcium p=0.4, 25-hydroxycholecalciferol p=0.01, PTH p[0.00001. High circulating parathyrin values due to PHP normalized after surgery in addition to statistical significant changes of TCa, ICa, P, lumbar Bone Mineral Density provided by Dual-Energy X-Ray Absorptiometry; Mg and 24-h Ca might not be a marker of general mineral metabolism improvement.

Whether the sterile valvular inflammation which occurs in this autoimmune disease predisposes to bacterial adhesion and infective endocarditis is not yet established. We present the case of a patient with Behcet disease in which transthoracic echocardiography showed mobile masses on the aortic, tricuspid, and mitral valves, leading to multivalvular infective endocarditis diagnosis, possibly in the context of valvular inflammation. The case presented in this article confirms observation of other studies, namely that ultrasonography plays an important role in the diagnosis and evaluation of rheumatic diseases and permits optimal management in daily practice.

Materials and Methods: The study included 97 women in the study, who underwent an oral glucose tolerance test (OGTT) between weeks 24–28 of pregnancy, consequently being divided into two groups: pregnant women with and without GDM. Urban air pollution is responsible for the highest burden of environmentally related diseases in ... more Urban air pollution is responsible for the highest burden of environmentally related diseases in Europe. Air pollutants with strongly indicated respiratory health effects are particulate matter, nitrogen oxides and sulfur dioxide. The retrospective ecological survey has taken into account the urban air pollution with sulfur dioxide (SO 2), nitrogen dioxide (NO 2), ammonia (NH 3) and particulate matter (PM 10). The analyzed data show that after applying the European regulations regarding the quality of environment air, the level of air pollution with SO 2 , NO 2 and NH 3 was significantly reduced; the atmospheric concentrations of the pollutants were lower than the maximal admissible values. Pollution with PM 10 still had high levels. Calculating the global pollution index, expressing the quality of ambient air by integrating the four surveyed pollutants, suggests that chronic simultaneous exposure to SO 2 , NO 2 , NH 3 and PM 10 from the atmosphere in the Craiova municipality makes for a significant risk towards diseases related to ambient air pollution.
