Numerous studies showed that, at present, traumatic brain injury (TBI) is one of the main causes ... more Numerous studies showed that, at present, traumatic brain injury (TBI) is one of the main causes of death in young adults, but also a main cause of disabilities at all ages. For these reasons, TBI are continuously investigated. In our study, we evaluated the histopathological (HP) and immunohistochemical (IHC) changes that occurred in the brain in underage patients after a severe TBI depending on the survival period. We histopathologically and immunohistochemically analyzed a number of 22 cases of children, deceased in Dolj County, Romania, following some severe TBI, undergoing autopsy within the Institute of Forensic Medicine in Craiova between 2015-2020. Patients were divided into three groups depending on the survival period, namely: (i) patients who died during the first 24 hours of the accident; (ii) patients who died after seven days of survival; (iii) patients who died after 15 days of survival. Microscopic examinations of the brain fragments, collected during the necropsy examination, showed that the traumatic agent caused primary injuries in all brain structures (cerebral parenchyma, meninges, blood vessels). However, HP injuries ranged in size and intensity from one area to another of the brain. In patients with a longer survival period, there was observed the presence of smaller primary injuries and larger secondary injuries. There was also observed a growth in the number of meningo-cerebral microscopic injuries, depending on the increase of the survival period.

Traumatic brain injuries (TBIs) present an ever-growing prevalence, especially in the developing ... more Traumatic brain injuries (TBIs) present an ever-growing prevalence, especially in the developing countries. Although 80-95% are mild to average injuries, they determine multiple severe neurological sequelae and disabilities. Most of these injuries are caused by traffic accidents. We studied a number of 29 cases of severe TBIs, in children who deceased immediately or after a few days of survival. Most of them (over 68%) were caused by traffic accidents. The incidence of traumas increased by age, most cases being recorded in the age group 10-15 years old. The TBIs were complex ones. In 86.21% of the cases, the forensic examination highlighted the presence of cranial fractures; in 93% of the cases, there were highlighted complex meningo-cerebral injuries: leptomeningeal hemorrhage associated with brain contusion injuries and with intraventricular blood flood, as well as destructive lesions of brain dilaceration; only in 7% of the cases there were highlighted meningeal lesions, with no brain lesions. The severity of the brain injuries was quite varied, according to the force of the cause agent. The histopathological and immunohistochemical examinations showed that the severity of TBIs increased according to the survival time, by adding secondary lesions caused by brain ischemia and local inflammatory reaction.

Condyloma acuminatum is a sexually transmitted infection caused mainly by subtypes 6 and 11 of HPV, with subtypes 16, 18 being involved in malignant transformation. We present the case of a 70-year-old woman, hospitalized for a vulvar and perineal vegetative, ulcerated, bleeding tumor, with onset 20 years ago. The therapeutic option was surgical excision of the lesions and long-term oncological monitoring.

PATIENTS, MATERIALS AND METHODS This is a prospective study between January 2016 and December 2017 that included patients with Caesarean scar and placenta praevia in the third trimester of pregnancy. By means of two-dimensional (2D) grayscale and color Doppler, we investigated the presence of the following US markers for placental invasion: intraplacental lacunae, abnormal blood vessels at the myometrium-bladder interface, thinning of the hyperechogenic uterine serosa-bladder wall interface, loss of normal hypoechoic retroplacental myometrial space. Conventional autopsy is hampered at this gestational age because of the small size of the heart anatomical structures, while alternative non-invasive methods for pathology examination of the fetus are expensive, rarely available and lack accuracy data regarding the con rmation of rst trimester heart defects. Method: Normal hearts from fetuses aged 12-13 gestational weeks were removed for histological preparation, virtual reconstruction and cardiac structures analysis. All sections have been scanned and a three-dimensional (3D) reconstruction of the whole organ has been rendered. Results: cases were investigated. Visualization of the normal heart cavities, including atrio-ventricular septum was very good in all fetuses. The entire course of right and left ventricle out ow tracts was con dently con rmed, along the and branching pattern of aorta and pulmonary artery trunk. Conclusion: The use of 3D reconstruction of fetal heart histological sections in rst trimester may serve as an important audit to con rm the normalcy of heart structures. Evaluating the benefits of an early screening of combined maternal markers and ultrasound of the fetuses. Methods. Prospective study (1 January and 31 December 2018) enrolled 187 patients, evaluated at the end of the first trimester of pregnancy (nuchal scan). Biochemical markers: detection of free β human chorionic gonadotropin and serum pregnancy associated plasma protein A. The first trimester ultrasound examination included the assessment of the nuchal translucency, fetal crown rump length and fetal heart. Results. 9 cases with abnormal biomarkers, increased nuchal translucency and cardiac anomalies, including abnormal 4-chamber view (6 cases) and great vessel anomalies (3 cases). 1 case considered initially negative in the first trimester was diagnosed by morphological scan with tetralogy of Fallot in the second trimester. Conclusions. A first trimester combined ultrasound protocol and maternal serum biochemical markers is important in the early prenatal care screening.

The most frequent histopatological subtype is endometrioid carcinoma. We present a case of a 36-years old patient admitted in the Emergency Department for lower abdominal pain and abnormal uterine bleeding. The clinical and ultrasound examination diagnosed bilateral ovarian cystic tumors, a normal uterine structure and no abdominal fluid colection. Serum levels of ROMA score was performed with normal value. The International Ovarian Tumor Analysis (IOTA) criteria used for ovarian tumors scoring diagnosed a 55% probability for malignant tumors. Laparotomy was performed with prelevation of peritoneal fluid for citology. Recent developments in ultrasonographic imaging facilitate the prenatal evaluation of this system. However, many of the involved mechanisms are poorly understood. The hepatic primordium is in contact with the vitelline veins and the umbilical veins, and by the end of the 6th week, the afferent venous system of the liver is acquired giving rise to the portal vein, the portal sinus, and the ductus venosus. The only afferent vein of the liver that remains open at birth is the portal vein. Also, the efferent venous system of the liver is formed and emerges from the vitelline veins.

The single stillbirth long-term intrauterine retention in dichorionic twin pregnancy is rarely re... more The single stillbirth long-term intrauterine retention in dichorionic twin pregnancy is rarely reported. Also, the birth of a fetus is followed in most cases by immediate expulsion of the second twin. We hereby present an unusual case of asynchronous delivery of dichorionic twins, associating discordance for major and minor anomaly. The intrauterine death of the twin A, presenting a large sacrococcygeal tumor, occurred in the second trimester. The deceased twin A was born at 29 weeks' gestational age (GA). The twin B was extracted by Caesarean section at 31 weeks and had a good outcome. We performed a close follow-up of the high-risk pregnancy and we used tocolytic and antibiotic drugs for prolonging it. Corticoid therapy was administered for the lung maturation of the second twin. The expectant management in the single twin stillbirth dichorionic pregnancy and the asynchronous delivery had a significant impact on the newborn outcome.

Background: Cavum septi pellucidi (CSP) is easily evaluated in the second and third trimester of ... more Background: Cavum septi pellucidi (CSP) is easily evaluated in the second and third trimester of the pregnancy. The structure is an important feature of the standard planes used for routine morphological assessment of fetal head and central nervous system (CNS): transthalamic and trans-ventricular plane. The standard description of the CSP is an anechoic rectangular box between two hyperechoic lines represented by the septum pellucidum. The pathological aspects are mainly represented by the absence of its normal appearance, which is associated with severe CNS malformations of the brain midline as corpus callosum agenesis, hydranencephaly, porencephaly, schizencephaly, holoprosencephaly, syntelencephaly or severe chronic hydrocephaly. Other issues such as increased or reduced dimensions of the CSP are not considered significant if found isolated, although the fetal genetic investigation is suggested by some authors. The objective of the study was to evaluate the outcome of fetuses with abnormal CSP detected by sonography in the last 5 years, in the Prenatal Diagnosis Unit (PDU) of our tertiary center. Methods: We performed a retrospective review of the cases with abnormal CSP evaluated in our tertiary unit, diagnosed between January 2012 and November 2016. The fetal anatomy was evaluated in all cases following the recommendations of the international guidelines. In abnormal CSP cases, fetal neurosonogram and an extended fetal anomaly scan were performed and amniocentesis was proposed to identify genetic disorders. Results: A total of 7,520 cases were examined for morphological purposes and abnormal CSP was found in 36 cases. Absent CSP was the initial observation that triggered further investigation and diagnosis in the cases with agenesis of corpus callosum (ACC) (seven cases) and septo-optic dysplasia (two cases). In hydranencephaly or severe hydrocephaly, porencephaly, schizencephaly and holoprosencephaly, the heavily malformed aspect of the brain is obvious, and the absence of CSP is only an observation, with less diagnostic importance and clinical implications. Partial or total ACC was mainly associated to absent CSP, as the development of two structures is merged. Almost half of the total abnormal CSP cases (16/36) were associated with genetic disorders, most of them with abnormal karyotype and all of them were associated with the absence of CSP. The persistent enlargement of CSP (4/36 cases) and the hyperechoic aspect of CSP (1/36) were not associated with other structural or genetic abnormalities and the postnatal neuromotor development was normal. Conclusions: The CSP evaluation is mandatory and normal aspects suggest a normal development of the midbrain. Genetic testing should be offered especially for the cases with absent CSP because of the high incidence of chromosomal disorders. As the absence of CSP is associated with severe structural or genetic disorders, its visualization in the second half of pregnancy is mandatory for any anomaly scan. The enlargement and echogenicity variations of CSP associate a favorable neonatal outcome. However, long-term follow-up is recommended in the apparently normal neonates and infants, as they may develop abnormal psychological behavior lately.

The study assessed p53 and p16 immunoexpression in 20 cases of ovarian serous carcinomas and four... more The study assessed p53 and p16 immunoexpression in 20 cases of ovarian serous carcinomas and four cases of serous borderline tumors, the results being statistically analyzed in relation to clinicopathological data of the cases. The p53 immunoreaction was observed in 85% of cases, the medium percentage of positivity being 15% for borderline tumors, 45% for low-grade carcinomas and 60% for high-grade carcinomas. The p16 immunoreaction was observed in 75% of cases, the medium percentage of positivity being 30% for borderline tumors, 25% for low-grade carcinomas and 62% for high-grade carcinomas. The p53 and p16 reaction was also identified at the tubal epithelium in cases of invasive carcinomas. Statistical analysis indicated significant differences in p53 expression depending on tumor type and for p53 and p16 expression compared to the degree of tumor differentiation. In this study, we analyzed EGFR, HER2/neu and Ki67 immunoexpression for 26 benign, borderline and... more In this study, we analyzed EGFR, HER2/neu and Ki67 immunoexpression for 26 benign, borderline and malignant serous ovarian tumors. EGFR and HER2/neu immunoreactions were present in some benign/borderline tumors with high/low intensity of immunostain. In poorly differentiated adenocarcinomas, the EGFR/HER2/neu reaction was intense compared to well-differentiated ones. The Ki67 medium proliferation index was 2.1% for benign tumors, 6% in the borderline and 47.7% in malignant tumors. EGFR, HER2/neu and Ki67 can be used to identify benign/borderline tumors with progression potential and the malignant aggressive tumors.

In abnormal CSP cases, fetal neurosonogram and an extended fetal anomaly scan were performed and amniocentesis was proposed to identify genetic disorders. Results: A total of 7,520 cases were examined for morphological purposes and abnormal CSP was found in 36 cases. Absent CSP was the initial observation that triggered further investigation and diagnosis in the cases with agenesis of corpus callosum (ACC) (seven cases) and septo-optic dysplasia (two cases). In hydranencephaly or severe hydrocephaly, porencephaly, schizencephaly and holoprosencephaly, the heavily malformed aspect of the brain is obvious, and the absence of CSP is only an observation, with less diagnostic importance and clinical implications. Partial or total ACC was mainly associated to absent CSP, as the development of two structures is merged. Almost half of the total abnormal CSP cases (16/36) were associated with genetic disorders, most of them with abnormal karyotype and all of them were associated with the absence of CSP. The persistent enlargement of CSP (4/36 cases) and the hyperechoic aspect of CSP (1/36) were not associated with other structural or genetic abnormalities and the postnatal neuromotor development was normal. Conclusions: The CSP evaluation is mandatory and normal aspects suggest a normal development of the midbrain. Genetic testing should be offered especially for the cases with absent CSP because of the high incidence of chromosomal disorders. As the absence of CSP is associated with severe structural or genetic disorders, its visualization in the second half of pregnancy is mandatory for any anomaly scan. The enlargement and echogenicity variations of CSP associate a favorable neonatal outcome. However, long-term follow-up is recommended in the apparently normal neonates and infants, as they may develop abnormal psychological behavior lately.

The study assessed p53 and p16 immunoexpression in 20 cases of ovarian serous carcinomas and four... more The study assessed p53 and p16 immunoexpression in 20 cases of ovarian serous carcinomas and four cases of serous borderline tumors, the results being statistically analyzed in relation to clinicopathological data of the cases. The p53 immunoreaction was observed in 85% of cases, the medium percentage of positivity being 15% for borderline tumors, 45% for low-grade carcinomas and 60% for high-grade carcinomas. The p16 immunoreaction was observed in 75% of cases, the medium percentage of positivity being 30% for borderline tumors, 25% for low-grade carcinomas and 62% for high-grade carcinomas. The p53 and p16 reaction was also identified at the tubal epithelium in cases of invasive carcinomas. Statistical analysis indicated significant differences in p53 expression depending on tumor type and for p53 and p16 expression compared to the degree of tumor differentiation. In this study, we analyzed EGFR, HER2/neu and Ki67 immunoexpression for 26 benign, borderline and... more In this study, we analyzed EGFR, HER2/neu and Ki67 immunoexpression for 26 benign, borderline and malignant serous ovarian tumors. EGFR and HER2/neu immunoreactions were present in some benign/borderline tumors with high/low intensity of immunostain. In poorly differentiated adenocarcinomas, the EGFR/HER2/neu reaction was intense compared to well-differentiated ones. The Ki67 medium proliferation index was 2.1% for benign tumors, 6% in the borderline and 47.7% in malignant tumors. EGFR, HER2/neu and Ki67 can be used to identify benign/borderline tumors with progression potential and the malignant aggressive tumors.
