We try to ascertain whether total thyroidectomy with radical neck dissection offers a survival an... more We try to ascertain whether total thyroidectomy with radical neck dissection offers a survival and recurrency advantage for thyroid cancer. Between January 2000 and September 2009 in the 3rd Surgical Unit of the "Sf. Spiridon" Hospital of Iaşi 189 operations were performed for malign thyroid disorders, 164 (86,77%) of which were performed for the papillary or foliculary cancer. The diagnosis was confirmed by hystopathological examination. The patients had a medium age of 51,48 (extremes 19 - 81 years). Women had a greater proportion, as expected (sex ratio F:M = 2,85:1). 59 total thyroidectomy with radical or modified radical neck dissection were performed. There were immediate (6 bleedings which needed reoperation for haemostasis, 7 unilateral recurrent laryngeal nerve injury) and late complications (8 cases of recurrency in the first 2 years). Children with thyrotoxicosis are usually hyperactive with behavioral problems, difficulty in conc... more Children with thyrotoxicosis are usually hyperactive with behavioral problems, difficulty in concentration and attention. Neuro-psychiatric disturbances in children with Graves’ disease (GD) can be mistaken for “true” psychiatric or neurologic disorder delaying the diagnosis and the correct therapeutic approach. We present the case of an 8 years old with important height and weight gain in the last two years. The patient had a medical history of ADHD. Despite the psychotherapy the behavior has not improved. Physical examination revealed increased weight and height, exophthalmia and lid retraction, diffuse goiter, important tremor, hyper talkativeness, warm and moist palms and tachycardia. The diagnosis of GD was confirmed by the values of thyroid hormones and the presence in high titers of thyroid stimulating hormone receptor antibodies (TRAb). After several months of treatment with ATDs only, we have switched “on block and replace” therapy. During the 3 years surveillance, the height and weight velocity rates remained increased with an advanced bone age. Once euthyroidism achieved and maintained, also the neuropsychiatric abnormalities, including irritability, anxiety, sleep disturbances and mood changes improved. The school performances improve to, and the inability to concentrate disappeared after several months. At the moment the patient is still on a minimal dose of ATD with normal hormonal levels. Neuro-psychiatric manifestations can be the first sign of thyrotoxicosis in children and adolescents. A close collaboration with parents is required in order to have the desired results: euthyroidism and disappearance of neuro-psychiatric dysfunctions.

The aim of the study was to assess the impact of patient--(age) and tumor--related factors (size,... more The aim of the study was to assess the impact of patient--(age) and tumor--related factors (size, extrathyroidal invasion, distant metastasis, multicentricity and lymphatic metastasis) on survival of patients with differentiated thyroid cancer (DTC). A clinical retrospective study was carried out on a series of 125 patients operated for non medullary DTC in the IIIrd Surgical Unit, in the period 1990-2005. The disease specific survival (DS) was calculated using the Kaplan Meyer method and Cox regression univariate and multivariate analysis was used to assess the impact of prognostic factors on DS. The actual DS at 5, 10, and 15 yrs was 81.3%. Clinicopathological factors significant on univariate and multivariate regression were age over 45 yrs (p = .01), tumor size > 4 cm (p = .00), macroscopical extrathyroidal invasion (p = .000) and distant metastasis (p = .000).

Among the rarely caecal pseudotumours are those caused by a coagulation disorder. The clinical co... more Among the rarely caecal pseudotumours are those caused by a coagulation disorder. The clinical course of caecal pseudotumours is highly variable. Most patients are diagnosed with acute appendicitis, but the real diagnosis is confirmed after laboratory investigation and histopathologic exam. Our patient presented the symptoms of an inferior digestive bleeding. The persistence of the clinical symptoms and the haematological degradation determined the surgical intervention. Intraoperative were found two caecal tumours. The hystological exam, biochemical and haematological analysis worked up the final diagnosis: a caecal pseudotumour in the context of Antithrombin III deficiency.

The stomach brought intra thoracic it seems that maintains or regains its capacity to secrete hydrochloric acid and therefore can develop specific conditions, despite total denervation following bilateral troncular vagotomy. We are presenting the case of a young patient who was operated on for a corrosive esophagitis. She had an oesophagectomy and a transposition of the stomach to the posterior mediastinum and anastomosed to the cervical esophagus. She presents with upper gastro-intestinal bleeding from gastric ulcer penetrating into the left atrium.

This study aimed to compare the main pathological features of TCTs containing medullary thyroid carcinoma (MTC) and papillary thyroid carcinoma (PTC) components with MTC-only tumors and PTC-only tumors. Methods: The retrospective study included 69 cases diagnosed with TCTs (with simultaneous MTC and PTC components), MTC and PTC. All tumors were comparatively assessed for the classical histopathological prognostic features, including a new grading system for MTC. Results: The main component of TCTs had more frequent microscopic extrathyroidal extension (mETE) (p = 0.000), lymphovascular invasion (LVI) (p = 0.000), perineural invasion (PNI) (p = 0.044), and lymph node metastasis (p = 0.042). Fine needle aspiration biopsy (FNAB) in evaluation of nodular goiter generated a true revolution.... more Fine needle aspiration biopsy (FNAB) in evaluation of nodular goiter generated a true revolution. It is generally assumed that the use of FNAB reduces the number of surgical procedures by 50%, while doubles the proportion of carcinoma in surgically treated patients. The aim of the study was to demonstrate the utility of FNAB in preoperative diagnosis of thyroid carcinoma. We have investigated 3240 patients with nodular goiter by FNAB (MGG stain and morphometrical analysis), followed by morphological examination in those who underwent surgery. Globally, FNAB had a good sensitivity (77%) and specificity (95%), which made an accuracy of 92%, proving its diagnostic value. In follicular carcinoma (FC), where the cytological diagnostic cannot be certain, morphometry helped the diagnostic: mean cells diameter was significantly higher (p < 0.001) in FC (9.5 mm) vs. thyroid adenoma (8.6 mm). With good statistical value, FNAB remain a useful preoperative diagnostic method.

The need for time and attention given by the doctor to the patient, due to the increased volume o... more The need for time and attention given by the doctor to the patient, due to the increased volume of medical data to be interpreted and filtered for diagnostic and therapeutic purposes has encouraged the development of the option to support, constructively and effectively, deep learning (DL) models for applications in the interpretation of medical images. Imaging physicians combine data from different stages and medical experiences, as opposed to DL models that incorporate the same types and modes of artisanal features. Access to big-databased medical imaging can be considered a benefit to the performance of DL models in interpreting medical imaging but similar or superior performance has been achieved with small, multi-feature and well-categorized databases that have improved annotation and labelling. The major contribution of this paper is primarily to highlight the impact of data quality, type and volume used by deep learning models in medical image analysis accompanied by updated characterization of the components of the deep learning process from data to medical applications. Second, it describes the specific correlations between the components of the deep learning process. Finally, it presents problems and directions for future research.

We have investigated 3240 patients with nodular goiter by FNAB (MGG stain and morphometrical analysis), followed by morphological examination in those who underwent surgery. Globally, FNAB had a good sensitivity (77%) and specificity (95%), which made an accuracy of 92%, proving its diagnostic value. In follicular carcinoma (FC), where the cytological diagnostic cannot be certain, morphometry helped the diagnostic: mean cells diameter was significantly higher (p < 0.001) in FC (9.5 mm) vs. thyroid adenoma (8.6 mm). With good statistical value, FNAB remain a useful preoperative diagnostic method.

The need for time and attention given by the doctor to the patient, due to the increased volume o... more The need for time and attention given by the doctor to the patient, due to the increased volume of medical data to be interpreted and filtered for diagnostic and therapeutic purposes has encouraged the development of the option to support, constructively and effectively, deep learning (DL) models for applications in the interpretation of medical images. Imaging physicians combine data from different stages and medical experiences, as opposed to DL models that incorporate the same types and modes of artisanal features. Access to big-databased medical imaging can be considered a benefit to the performance of DL models in interpreting medical imaging but similar or superior performance has been achieved with small, multi-feature and well-categorized databases that have improved annotation and labelling. The major contribution of this paper is primarily to highlight the impact of data quality, type and volume used by deep learning models in medical image analysis accompanied by updated characterization of the components of the deep learning process from data to medical applications. Second, it describes the specific correlations between the components of the deep learning process. Finally, it presents problems and directions for future research.
