An updated and redesigned version of merlin is herein presented. Since 2015, several features have been implemented in merlin, along with deep changes in the software architecture, operational flow, and graphical interface. The current version (4.0) includes the implementation of novel algorithms and third-party tools for genome functional annotation, draft assembly, model refinement, and curation. Such updates increased the user base, resulting in multiple published works, including genome metabolic (re-)annotations and model reconstructions of multiple (lower and higher) eukaryotes and prokaryotes. ...

Material and methods: Data related to Coimbra Pediatric Hospital during the year of 2009. Number of patients seen, their distribution and the most frequent pathologies were analyzed. Results: 4571 children were found with a ENT pathology diagnose. 1573 children were observated by an ENT physician. February (155 cases) and monday (18%) were the busiest periods. Most of them were males (55.09%) and were between 1-5 years (50.58%). The primary diagnosis was framed in the pathology of mouth / pharynx / larynx (49.6%), ear disease (43.5%) and pathology of the nose (6.9%). Conclusion: The study highlights the need of publishing results to compare realities, as well as notice that part of ENT pathology is evaluated by non ENT doctors or part of the activity is not recorded.

More than thirty years pass since the first cochlear implant was used. These years, and the increasing number of implanted devices, have shown however some limitations and technical failures. These electronic device failures are unpredictable and sometimes difficult to diagnose. Description of two cases: The authors present two cases to illustrate failures of the intracochlear device. Both cases required a cochlear reimplantation surgery. Discussion and conclusion: There are multiple causes for intracochlear device’s failure. Management of implant failures, including revision surgeries, is becoming an increasingly important part of cochlear implant program activity.

The aim of this study is to evaluate the effectiveness of various types of grafts used in tympanoplasty type III and IV Wullstein and possible prognostic factors. Material and Methods: Retrospective study of 51 ossiculoplasties performed at the General Hospital / Children's Hospital of Otorhinolaryngology of Coimbra Universitary Hospital. We analyzed several variables such as age, gender, laterality, type of prosthesis, presence of malleus, hearing thresholds and followup time. Surgical success was defined by an air-bone gap postoperative less than 20 dB. Results: There were 37.3% of ossiculoplasties using autologous incus with a functional success rate of 89.5%. With autologous cortical bone were performed 35.3% of the ossiculoplasties, achieving functional success rate of 83.3%. ...

However, particularly in immunocompromised patients, can present itself as a severe invasive form and can be potentially fatal. The identification of this pathology and its associated complications is not always easy. The disclosure of the different presentations helps the diagnosis. The authors present the case of a patient with a fulminate invasive form of rhinosinusal aspergillosis, which highlights diagnostic difficulties and relevant therapeutic aspects. Prompt diagnosis and treatment may prevent the high mortality associated with this condition. Orbital and intracranial complications are the most frequent and should be promptly identified and treated. The combination of medical and surgical treatment is a good therapeutic option.

Surgical intervention should be attempted at a very early age to prevent further pathological adaptations. The authors present the case of an adult woman admitted with painful callosities on the feet and difficulty selecting shoes. She was diagnosed with cleft foot and submitted to surgical treatment. The postoperative period was uneventful and the patient was very satisfied with the results of the surgery. This is only the second reported case of surgical management of cleft foot in an adult patient, and the first to describe the use of internal fixation. With the rise in the frequency of multidrug-resistant clinical isolates, the identification of new drug targets and new drugs is crucial in overcoming the increase in therapeutic failure. In this study, the first validated genome-scale metabolic model for Candida albicans, iRV781, is presented. The model consists of 1221 reactions, 926 metabolites, 781 genes, and four compartments. This model was reconstructed using the open-source software tool merlin 4.0.2. It is provided in the well-established systems biology markup language (SBML) format, thus, being usable in most metabolic engineering platforms, such as OptFlux or COBRA. The model was validated, proving accurate when predicting the capability of utilizing different carbon and nitrogen sources when compared to experimental data. Finally, this genome-scale metaboli...
