OBJECTIVE: Report a rare case of Pfeiffer’s syndrome type 2 in a male newborn. CASE REPORT: Newbo... more OBJECTIVE: Report a rare case of Pfeiffer’s syndrome type 2 in a male newborn. CASE REPORT: Newborn male, preterm term of 33 weeks and 2 days, serial ultrasound suggesting presence of craniosynostosis and other abnormalities such as irregular and narrow vertebrae, pachygium, and ventriculomegaly in the second trimester, being confirmed later with magnetic resonance imaging. There was no need for obstetrical medications and maneuvers for the birth. Apgar score 4, heart rate less than 100bpm, precarious respiratory evolution, orotracheal intubation, umbilical venous catheterization and orogastric tube were required. With 2 hours of life presented bradycardia along with hemodynamic instability evolved to a cardiorespiratory arrest with no success in cardiopulmonary resuscitation. He passed away 3 hours after birth. COMMENTS: Pfeiffer’s syndrome is rare, being type 2 the most lethal because it has a clover-shaped skull. RESUMO A hanseníase constitui um relevante problema de Saúde Pública no Brasil, e o controle dos comuni-cantes é fundamental para o controle da endemia e o alcance das metas de eliminação. In April 2020, the UK pediatric society reported to the World Health Organization the appearance ... more In April 2020, the UK pediatric society reported to the World Health Organization the appearance of a rare syndrome, with a wide spectrum of signs and symptoms, temporarily associated with COVID-19, which mainly affected normally healthy schoolchildren and adolescents, with high lethality when not diagnosed and treated early. We present the case of a 12-year-old child who had high and persistent fever, rash and vomiting, who, after 6 days, developed acute respiratory failure, myocardial and renal dysfunction, requiring ventilatory and hemodynamic support, treated with antibiotics, intravenous immunoglobulin, and anticoagulant. The purpose of this report is to present the reader with the peculiar characteristics of the diagnosis and treatment of the syndrome and its main differential diagnoses at the hospital admission.

Corticosteroids are effective in preventing failure in adults, but no consensus has been reached on this matter in pediatrics. We assessed the efficacy of intravenous dexamethasone in mechanically ventilated children and adolescents for more than 48 hours, with at least one risk factor for failure. Extubations were scheduled 24 hours in advance when possible, and patients were randomly assigned into two groups: one group received a loading dose followed by up to four doses of dexamethasone, and the other group received no corticosteroids. Need for reintubation and length of stay in the pediatric intensive care unit were similar in both groups, and frequency of reintubation was 12.9%.

Background Congenital heart diseases (CHD) are dynamic defects that originate in the embryo, evol... more Background Congenital heart diseases (CHD) are dynamic defects that originate in the embryo, evolving during gestation, and undergoing significant change throughout the entire course of extrauterine life. CHD represent a complex group of abnormalities related to high infant morbidity and mortality. Ultrasonography advances in Obstetrics and Fetal Medicine have contributed to the increased detection of these anomalies, with high screening potential during pregnancy. Despite studies pointing to high fetal echocardiography's sensitivity and specificity, this method is not homogeneously available worldwide yet, particularly in low and medium-income countries. Objectives Here we compared the accuracy of obstetric ultrasonography performed between 18 and 22 weeks of gestational age with Fetal Echocardiography.
