For planning the surgical approach in patients with head and neck tumours and extensive cervical ... more For planning the surgical approach in patients with head and neck tumours and extensive cervical metastasis the evaluation of possible vascular involvement is necessary. If the carotid artery is suspected to be infiltrated by the tumour masses its resection or plastic substitute becomes sometimes inevitable. For evaluation of the collateral brain perfusion before a possibly necessary carotid resection we examined 10 patients with neck tumours performing the Matas-Test combined with HMPAO Single-Photon-Emission-Computed-Tomography. During ultrasound-controlled manual compression of the involved common carotid artery 99mTc-labelled HMPAO was injected. The SPECT demonstrated the brain perfusion during the carotid compression time period. 6 patients showed a decreased perfusion of the affected hemisphere with - as far as obtained - normal baseline SPECT-results and balloon occlusion tests. 2 of these patients underwent a carotid artery resection, one of the common carotid artery and one of the internal carotid artery. One patient had a significant perfusion deficit in the SPECT but no pathologic findings in angiography and balloon occlusion test. He suffered from hemiparesis postoperatively. The other patient showed preoperatively a minor decrease in perfusion and had no neurological defects postoperatively. Our findings show that the Matas-Test combined with HMPAO-SPECT is a valuable tool for the preoperative evaluation of the collateral brain perfusion. This technique is not invasive and can be performed in every nuclear medicine department with an ECT camera. The HMPAO-SPECT during carotid compression can add useful information or even replace angiography and the balloon occlusion test.

They are aimed at guiding nuclear medicine specialists in selecting likely candidates to receive PRRT and to deliver the treatment in a safe and effective manner. The examined cases all had individual problems and the Gallium-67 promised additional information about the nature of the underlying bone process. Because of the different mechanisms for Gallium to accumulate in pathological bone processes it is necessary to know exactly the individual problem of the case to keep the rate of wrong positive results low. Despite of such efforts one should be cautious in using this method in the diagnosis of chronic osteomyelitis.

In lack of in-gantry transmission (TX) sources, the exact position of the RF coils is unknown in PET/MR, and may, therefore, lead to false attenuation correction (AC) of the emission (EM) data. The authors assess lesion and background quantification in AC-PET by mimicking different PET/MR imaging situations using a whole-body (WB) PET-only tomograph. Phantom experiments were performed on a PET tomograph with 68 Ge-rod TX sources. First, a 15-cm plastic cylinder was filled uniformly with [18F]-FDG to simulate a head study. To retrospectively assess the accuracy of angiography-based C-arm CT for the detection of extrahe... more To retrospectively assess the accuracy of angiography-based C-arm CT for the detection of extrahepatic shunting before SIRT. 30 patients (mean age: 64+/-12 years) with hypervascularized hepatic tumors underwent hepatic angiography, coil embolization of gastrointestinal collaterals and 99mTc-macroaggregated albumin (MAA) SPECT/CT before SIRT. Before MAA injection via a microcatheter from the intended treatment position, an angiography and angiography-based C-arm CT (XperCT, Philips Healthcare) were acquired. Angiographies and XperCT were performed from 48 microcatheter positions followed by MAA injections and MAA-SPECT/CT. MAA-SPECT/CT served as the reference standard for determining the accuracy of hepatic arteriography and C-arm CT for the detection of extrahepatic shunting. The FDG accumulation in malignant tumors is quite stereotyped and often reflects the aggressiveness of the tumor. Therefore, FDG appears to be suitable to detect unknown primaries independent of the type of disease. Being unaware of the site of the primary and also being unaware of the intensity of glucose metabolism, the interpretation of FDG-PET images needs rules accepting a reduced specificity for the definition of malignancy. However, due to physiological FDG-uptake in the head and neck area, especially in lymphatic tissue and vocal cords, one has to accept a high rate of equivocal findings interpreting FDG-PET alone. In this situation, the use of dual-modality PET/CT is of particular value. This investigation addresses the quantitative changes between PET/CT and stand alone PET. The study included all examinations performed on stand alone PET- or PET/CT-scanners within 12 month prior to and after implementation of PET/CT. The final data analysis included five university hospitals and a total number of 15 497 exams. We distinguished exams on stand alone tomographs prior to and after installation of the combined device as well as PET/CT scans particularly with regard to disease entities. Various further parameters were investigated. The overall number of PET scans (PET and PET/CT) rose by 146% while the number of scans performed on stand alone scanners declined by 22%. Only one site registered an increase in stand alone PET. The number of exams for staging in oncology increased by 196% while that of cardiac scans decreased by 35% and the number of scans in neurology rose by 4...

Two uniform phantoms simulating a "torso" and an "arms-up" setup were filled with (18)F-FDG/water. A third, nonuniform "body phantom was prepared with hot and cold lesions. All 3 phantoms were positioned in the center of the PET/CT gantry with >or=10% of their volume extending beyond the maximum CT FOV. An eFOV algorithm was used to estimate complete CT projections from nonlinear extrapolation of the truncated projections. CT-based attenuation correction (CT AC) of the phantom data was performed using CT images reconstructed from truncated and extended projections. Early detection is difficult but important for the further course of the disease. We report on a young patient with a tumour of unknown origin and dignity of the right atrium. Magnetic resonance imaging, CT and echocardiography were sufficient in localisation, but no statement on the dignity was possible. Furthermore, staging led to ambiguous results. Malignancy could be proved by fluorine-18 fluordeoxyglucose positron emission tomography, leading to early surgery. Histology revealed a poorly differentiated angiosarcoma.

Forty-six patients with insulin-treated diabetes mellitus and mild-to-moderate hypertension and stable microalbuminuria were examined. P 4 or 8 mg once daily was compared to N 20 or 40 mg once daily; an optional open combination treatment with indapamide 2.5 mg once daily was given when needed. Main outcome measures were urinary albumin excretion rate, creatinine clearance and isotopic clearance measurements after 12 months. Baseline characteristics (blood pressure, HbA1, renal function) were highly comparable between groups. No serious adverse events occurred during the study period. We investigated the relationship between thyroglobulin (Tg) level and tumor detectability. We analyzed the scans of 122 patients (66 patients with proven tumor). The patient population was divided into groups with Tg above (N = 33) and below (N = 33) 5 ng/ml under TSH suppression or above (N = 33) and below (N = 33) 50 ng/ml under TSH stimulation. Tumor detectability was compared by ROC-analysis (True-Positive-Fraction test, specificity 90%). There was no significant difference (sensitivity 75% versus 64%; p = 0.55) for patients above and below 5 ng/ml under TSH suppression and a just significant difference (sensitivity 80% versus 58%; p = 0.04) for patients above and below 50 ng/ml under TSH stimulation. In this study we investigated the somatostatin receptor status for this particular tumor entity in a larger patient group. We compared 131-iodine with 111-In-pentetreotide scans in 24 patients with metastasizing, non-medullary thyroid cancer. The findings were correlated with other imaging modalities. Additionally, we performed receptor autoradiography in one patient, octreotide therapy in another patient and administration of 90-Y- and 111-In-DOTATOC in 2 consecutive patients. In the 15 patients with papillary or follicular carcinoma, 111-In-pentetreotide was inferior to 131-I in 8/15, equal in 1/15, and superior in 6/15 patients. Seltene und rasch progrediente Verläufe dedifferenzierter primär differenzierter Tumoren, anaplastischer und medullärer Karzinome sind konventionell nicht behandelbar, und es besteht dann die Chance auf Tumorstillstand oder Teilremission, wenn die adäquate Chemotherapie oder multimodale Therapie eingesetzt wird.

Seltene und rasch progrediente Verläufe dedifferenzierter primär differenzierter Tumoren, anaplastischer und medullärer Karzinome sind konventionell nicht behandelbar, und es besteht dann die Chance auf Tumorstillstand oder Teilremission, wenn die adäquate Chemotherapie oder multimodale Therapie eingesetzt wird.
