The long-term results of transsphenoidal adenomectomy and the incidence of possible recurrences w... more The long-term results of transsphenoidal adenomectomy and the incidence of possible recurrences were studied in 61 patients who had normal basal serum growth hormone (GH) levels 1 week after surgery. The patients were followed up for an average of 6.0 years (range: 1.5-14.0 years) by repeated measurements of GH, oral glucose tolerance testing (OGTT) and at the last follow-up visit also by assaying somatomedin C levels. In 4 of the patients, the basal GH levels had increased to values above 5 ng/ml. In 43 patients, a normal suppression of GH during an oral glucose load was observed shortly after surgery. In only 2 of these cases did a transiently inadequate suppression develop during the follow-up period, although clinical acromegaly did not recur and the somatomedin C levels remained normal. It is concluded that recurrence of active acromegaly is unlikely to occur in patients who achieve a normal glucose-induced suppression of GH levels shortly after adenomectomy. As such, an OGTT provides better prognostic information than basal human GH level measurements and may give a clearer and earlier indication of surgical success.

Music plays an important role in the daily life of cochlear implant (CI) users, but electrical he... more Music plays an important role in the daily life of cochlear implant (CI) users, but electrical hearing and speech processing pose challenges for enjoying music. Studies of unilateral CI (UCI) users' music perception have found that these subjects have little difficulty recognizing tempo and rhythm but great difficulty with pitch, interval and melody. The present study is an initial step towards understanding music perception in bilateral CI (BCI) users. The Munich Music Questionnaire was used to investigate music listening habits and enjoyment in 23 BCI users compared to 2 control groups: 23 UCI users and 23 normal-hearing (NH) listeners. Bilateral users appeared to have a number of advantages over unilateral users, though their enjoyment of music did not reach the level of NH listeners.

Patients with a large vestibular aqueduct (LVA) suffer from a loss of hearing in childhood at an ... more Patients with a large vestibular aqueduct (LVA) suffer from a loss of hearing in childhood at an early onset. An acute loss of hearing can be precipitated by minor head trauma. Until now there seems to be no sufficient therapy for stopping the progression of a loss of hearing. It has been shown that a cochlear implantation is a worthwhile procedure if the patient is almost deaf. We report the case of a patient with a bilateral LVA. A loss of hearing was confirmed at the age of 16 months. Exposure to loud noise triggered an acute progression of the hearing loss. At the age of 18 years, LVA was confirmed radiologically, revealing an enlarged endolymphatic duct and sac in MRI scans and an enlarged vestibular aqueduct in the CT scan. We successfully performed a cochlear implant (MED-EL, Combi 40+ flex). Proceeding from this case report, the paper reviews the literature on LVA.

A study was conducted to compare the new MED-EL TEMPO+ ear-level speech processor with the CIS PR... more A study was conducted to compare the new MED-EL TEMPO+ ear-level speech processor with the CIS PRO+ body-worn processor in the COMBI 40/COMBI 40+ implant system. Speech tests were performed in 46 experienced subjects in two test sessions approximately 4 weeks apart. Subjects were switched over from the CIS PRO+ to the TEMPO+ in the first session and used only the TEMPO+ in the time between the two sessions. Speech tests included monosyllabic word tests and sentence tests via the telephone. An adaptive noise method was used to adjust each subject’s scores to approximately 50%. Additionally, subjects had to complete a questionnaire based on their 4 weeks of experience with the TEMPO+. The speech test results showed a statistically significant improvement in the monosyllabic word scores with the TEMPO+. We present the first report on complications of cochlear implantation with the COMBI-40 (Med-el, ... more We present the first report on complications of cochlear implantation with the COMBI-40 (Med-el, Innsbruck, Austria). Between January 1995 and May 1996 325 devices had been implanted by 58 different surgeons. Complications were reported with the help of standardized complication report form. The overall rate of complications was 4.6%. Most common problems were flap necrosis and incorrect positioning of the electrode. No technical failures occurred. The incidence of complications was lower than those reported by other authors.

Pradilektionstelle dieser Tumoren ist die sonnenexponierte Haut von Kopf und Hals. Erkrankungsgipfel ist das 7. und 8. Lebensjahrzehnt. Die Patienten sind in der Regel hellhautig. Bei Schwarzen ist dieser Hauttumor unbekannt. Eine Pravalenz bzgl. des Geschlechts besteht nicht. In der Literatur wird uber verschiedene chirurgische, medikamentose und strahlentherapeutische Therapieregime berichtet, ohne das ein befriedigendes Ergebnis erreicht wird.

Forty-six adults with a minimum of 6 months of cochlear implant experience were included. CIS+, HDCIS and FSP were compared in speech perception tests in noise, pitch scaling and questionnaires. The randomized tests were performed acutely (interval 1) and again after 3 months of FSP experience (interval 3). The subjective evaluation included questionnaire 1 at intervals 1 and 3, and questionnaire 2 at interval 2, 1 month after interval 1. The MUMU-questionnaire was used to investigate self-reported perception of music and the musical ... more The MUMU-questionnaire was used to investigate self-reported perception of music and the musical activities of adult cochlear implant users pre-and post implantation. Post-lingually deafened adults using either the CIS coding strategy as implemented in the Medel Combi 40/40+ implant or the SPEAK coding strategy as implemented in the Nucleus 22 and 24 implants completed the questionnaire. To date, 104 CIS and 69 SPEAK users have participated in the study. The age structures of both groups were similar. There was no overall difference in the number of users who reported that they regularly listen to music, in the styles of music listened to or the instruments that were preferred. Melody recognition, rhythm detection and a natural and enjoyable musical sound quality were reported by an equal number of subjects in both groups. In the CIS users the increase in the amount of active participation in musical activities after implantation was higher than in SPEAK users.

Objective: To investigate the influence of several magnetic resonance imaging (MRI) sequences on ... more Objective: To investigate the influence of several magnetic resonance imaging (MRI) sequences on amniotic fluid temperature and intrauterine sound pressure. Material and Methods: Temperature and sound pressure measurements during MRI (1.5 T) in pregnant ewes were done. Linear levels and third octave band spectra were compared. Results: No significant changes in the temperature of amniotic fluid were observed. Intrauterine summation levels reached peak levels up to 103.0 dB(A) before starting the MRI sequence and levels up to 116.0 dB(A) during a real-time sequence. Evaluating the octave band spectra, peak levels did not exceed 100.0 dB(L). Conclusions: Our delimited data revealed no harm for the fetus by an increase in amniotic fluid temperature or hazards for the fetal auditory system by different MRI sequences.
