Background: Tumor necrosis factor antagonists (anti-TNFa) became these last years an important dr... more Background: Tumor necrosis factor antagonists (anti-TNFa) became these last years an important drug class in support of many inflammatory diseases such as rheumatoid arthritis or ankylosing spondylitis (AS). Despite effectiveness of these drugs, they could favor peripheral and central nervous system disorders. Methods: Two cases of optical neuritis potentially attributable to golimumab were reported to our regional pharmacovigilance center one year ago. We report here the follow-up of these episodes evocating drug induced cases of multiple sclerosis (MS). Results The first patient is a 50-year-old male with a medical history marked by a diagnosis of AS in 2002 and treated sucessively by 3 anti-TNF drugs adalimumab (2009), etanercept (2010) and then golimumab. After five years of treatment, he presented a bilateral optic neuritis. Brain magnetic resonance imaging (MRI) showed right optical, right orbito-frontal, left pontine and left corpus callosum T2 hyperintensities and oligoclonal bands were present in cerebrospinal fluid (CSF). Golimumab was withdrawn and after intravenous methylprednisolone bolus and plasma exchanges, visual acuity recovered. The second patient is a 31-year-old female with a history of AS treated by adalimumab since 2012. Golimumab was introduced at the end of 2016. Five days after her first injection, the patient presented a left optic neuritis. MRI showed more than 9 lesions (periventriculars, juxtacorticals and one in the pons) without gadolinium enhancement and oligoclonal bands were present in CSF. Anti-TNFa was withdrawn and intravenous methylprednisolone bolus was performed with good recovery. After a one year-follow-up, patient1 presented no new neurologic lesions but he started secukinumab (an anti-IL-17A) because of AS activity. On the contrary, patient2 started teriflunomide because of new MRI lesions and confirmation of MS, no AS progression was found. Conclusion: Previously few papers reported MS induced by anti-TNF especially infliximab. Here the demyelinating lesions occured after very different chronologies but with clear multifocal signs of MS. For such cases physicians have to remember to stop definitively anti-TNFa therapy. These cases illustrate too that the full humanized anti-TNF mAb does not prevent demyelinating adverse effects.

Proportions of reversible and irreversible magnetization processes in the overall magnetization p... more Proportions of reversible and irreversible magnetization processes in the overall magnetization process were studied in a complex view of magnetic, thermal and electrical properties of iron-phenolphormaldehyde resin composites. They were determined experimentally at different values of magnetic induction along the initial curve. The results of total, differential, reversible and irreversible permeability measurement as well as the analysis of DC energy losses revealed the same tendencies: The numbers of movable domain walls (determining the extent of reversible processes) depend on the magnetic particle size and the resin content through the demagnetizing fields produced by the particle surfaces, lowering the interaction between particles. Thermal diffusivity was compared with Hashin-Shtrikman model indicating good insulation of particles.

Objectives-TNFα modulation has been reported to be either beneficial or detrimental in amyotrophi... more Objectives-TNFα modulation has been reported to be either beneficial or detrimental in amyotrophic lateral sclerosis (ALS) and therefore appears as a key issue. We analyzed the relationship between TNFα inhibitor (TNFi) exposure and ALS. Method-We performed descriptive analysis of ALS reports in patients treated with TNFi, registered in the French Pharmacovigilance Database (FPvD) and disproportionality analyses by the "case"/"non-case" method in FPvD and worldwide database (Vigibase®). Results-The 8 retrieved ALS cases from the FPvD were 5 with limb-onset and 3 with bulbaronset forms, in patients aged 43 to 75 years old, mainly treated for inflammatory rheumatism. The time to onset of the first symptoms ranged from 12 to 108 months and the cumulative TNFi exposure before the diagnosis ranged from 12 to 120 months. TNFi was discontinued and thereafter survival ranged between 12 to 20 months. Disproportionality analyses showed significant associations between TNFi exposure and ALS in the FPvD and Vigibase® (160 ALS cases), regardless comparators. Conclusion-A putative association between TNFi and ALS must be interpreted cautiously, but TNFi could act as a predisposing or risk factor. TNFi should therefore be avoided in patients with a known risk of ALS and discontinued in the case of neurological signs of ALS.

Vestibular dysfunction strongly impairs hippocampus-dependent spatial memory performance and plac... more Vestibular dysfunction strongly impairs hippocampus-dependent spatial memory performance and place cell function. However, the hippocampal encoding of vestibular information at the synaptic level, remains sparsely explored and controversial. We investigated changes in in vivo long-term potentiation (LTP) and NMDA glutamate receptor (NMDAr) density and distribution after bilateral vestibular lesions (BVL) in adult rats. At day 30 (D 30) post-BVL, the LTP of the population spike recorded in the dentate gyrus (DG) was higher in BVL rats, for the entire 3 h of LTP recording, while no difference was observed in the fEPSP slope. However, there was an increase in EPSP-spike (E-S) potentiation in lesioned rats. NMDArs were upregulated at D 7 and D 30 predominantly within the DG and CA1. At D 30 , we observed a higher NMDAr density in the left hippocampus. NMDArs were overexpressed on both neurons and non-neuronal cells, suggesting a decrease of the entorhinal glutamatergic inputs to the hippocampus following BVL. The EPSP-spike (E-S) potentiation increase was consistent with the dorsal hippocampus NMDAr upregulation. Such an increase could reflect a non-specific enhancement of synaptic efficacy, leading to a disruption of memory encoding, and therefore might underlie the memory deficits previously reported in rats and humans following vestibular loss.

The tumor microenvironment is a complex mix of cancerous and noncancerous cells (especially immun... more The tumor microenvironment is a complex mix of cancerous and noncancerous cells (especially immune cells and fibroblasts) with distinct metabolisms. These cells interact with each other and are influenced by the metabolic disorders of the host. In this review, we discuss how metabolic pathways that sustain biosynthesis in cancer cells could be targeted to increase the effectiveness of cancer therapies by limiting the nutrient uptake of the cell, inactivating metabolic enzymes (key regulatory ones or those linked to cell cycle progression), and inhibiting ATP production to induce cell death. Furthermore, we describe how the microenvironment could be targeted to activate the immune response by redirecting nutrients toward cytotoxic immune cells or inhibiting the release of waste products by cancer cells that stimulate immunosuppressive cells. In this review, we discuss how metabolic pathways that sustain biosynthesis in cancer cells could be targeted to increase the effectiveness of cancer therapies by limiting the nutrient uptake of the cell, inactivating metabolic enzymes (key regulatory ones or those linked to cell cycle progression), and inhibiting ATP production to induce cell death. Furthermore, we describe how the microenvironment could be targeted to activate the immune response by redirecting nutrients toward cytotoxic immune cells or inhibiting the release of waste products by cancer cells that stimulate immunosuppressive cells.
