While the consideration of TnI levels below the 99 th percentile URL increases sensitivity in the... more While the consideration of TnI levels below the 99 th percentile URL increases sensitivity in the diagnosis of CAD, it inherently involves a decrease in the specificity, although this can be improved by the combined analysis of temporal changes in TnI levels 5 and the ECG findings. In our study, the percent change in these levels within NR was found to be an excellent tool for predicting CAD in the hemodynamic study, and it could orient clinicians to the invasive management of these patients.

Congenital defects of the pericardium are extremely rare and frequently difficult to diagnose. We... more Congenital defects of the pericardium are extremely rare and frequently difficult to diagnose. We present a diagnosis of partial congenital defect of the left pericardium based on a characteristic chest X-ray and left ventriculogram. Additionally, we report findings of a diastolic, migratory coronary stenosis on coronary arteriography which, to date, has not been reported. Confirmation of the diagnosis by mediastinoscopy was followed by thoracoscopic pericardiectomy.

Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disorder that almost exclu... more Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disorder that almost exclusively involves motor neurons although autonomic dysfunction has also been reported. We present an 84-year-old female with no documented history of heart disease, who was admitted with negative T waves in the electrocardiogram precordial leads mimicking myocardial ischaemia. No other abnormalities were shown in the rest of the cardiologic evaluation, suggesting autonomic nervous system dysfunction. A neurophysiological study demonstrated acute and chronic denervation in multiple muscles with normal nerve conduction studies, confirming ALS diagnosis. Previous studies have shown that subclinical sympathetic hyperfunction and parasympathetic hypofunction might result in cardiovascular dysfunction in ALS patients. Este registro estudió la incidencia, la clínica asociada, el trayecto, los métodos diagnósticos d... more Este registro estudió la incidencia, la clínica asociada, el trayecto, los métodos diagnósticos de imagen y el tratamiento de las anomalías coronarias con origen en el seno coronario contralateral en 13 hospitales durante 2003. En 23.300 coronariografías se describieron 98 anomalías (incidencia del 0,4%): 46 circunflejas (47%), 25 coronarias derechas (25,5%), 10 coronarias izquierdas (10,2%), 3 descendentes anteriores (3,1%), 6 coronarias únicas (6,1%) y 8 incluidas en el apartado «otras» (8,2%). El motivo de la coronariografía más frecuente fue la angina (43,9%). El trayecto inicial de la coronaria anómala no fue identificado en el 40,8% de los casos. En 2 pacientes (2%) se emplearon métodos de imagen asociados. El 51% tenía enfermedad coronaria asociada y el 18,4%, valvular; la válvula aórtica era la más afectada (15,2%). El 16,3% recibió tratamiento para la anomalía. Se observó una incidencia similar a la de series previas, y las anomalías de la circunfleja fueron las más frecuentes. Los métodos de imagen asociados para el diagnóstico del trayecto inicial fueron infrautilizados.
