Although numerous therapeutic approaches have been described in the literature, to this date no consensus over a surgical treatment has been established. We report our experience in the treatment of Haglund's syndrome and pre-insertional calcifications of the Achilles tendon using an open surgical approach and platelet rich plasma (PRP) membranes as a local adjuvant. Methods The study retrospectively evaluated patients with Haglund syndrome treated with a direct posterior access, Achilles tendon split, bursectomy, resection of the heel deformity and topical administration of PRP membranes. After surgery, a standardized rehabilitation protocol was performed. The preoperative and postoperative functionalities of the foot and ankle were assessed using the American Orthopaedic Foot & Ankle Society (AOFAS) score. Intraoperative and postoperative complications were recorded. Results Twenty consecutive patients were included. The mean follow-up was 23 months. No patient had major intraoperative or postoperative complications. After the surgery, the mean AOFAS score increased from 44.1 to 92.0, resulting in a statistically significant increase in patient functionality after the treatment (p=0.001). Conclusion Our results suggest that an open surgical approach consisting of a combination of bursectomy, resection of the calcaneal deformity, and topical administration of PRP membranes may be effective for the treatment of Haglund's disease.

The social and economical impact is considerable with a continue social and health costs up-rising. The female-male ratio is approximately 8:1. Purpose: The purpose of this report is to present a system of osteosynthesis for fractures of the lateral femoral neck. This system is a plate developed by INTRAUMA O'nil(®), 3-hole plate with an 130° angle of valgus and 7° of anteversion. Materials and methods: At the I Orthopedic Department at the University of Pisa from January 2009 to February 2012 were treated 52 fractures of the femur neck side with this system. All patients undergone to a clinical and X-ray evaluation according to the AO classification: 14 31-A1, 7 31-A2, 4 31-A3. Results: The mean follow-up was 10 months, in all cases we did not have cut-outs with a TAD average of 23 mm. Conclusions: This system offers the possibility to perform surgery quickly with low risk of cut-out thanks to 7° of anteversion. Ensures optimal fixation to the femoral shaft and an equitable distribution of forces, eliminating the risk of breakage of the implant and loosening of the screws.

The aim of this study was to review patients that underwent ACL reconstruction with the LARS™ lig... more The aim of this study was to review patients that underwent ACL reconstruction with the LARS™ ligament in the First Orthopaedic Division of Pisa University during the period between January 2003 and December 2005. Methods Twenty-six patients were reviewed with an average follow-up of 95.3 months (7.9 years). The review protocol was articulated in three phases: (1) a subjective evaluation using three grading scales: VAS, KOOS and the Cincinnati knee rating scale, (2) a clinical and objective evaluation, and (3) a biomechanical evaluation of the knee stability. Results A global positive result was obtained in 92.3 % of the patients (16 optimal results and eight good results), with a fast functional recovery and a high knee stability. A global poor result was reported in two cases. In our series we did not record cases of infection or knee synovitis. We recorded only one case of mechanical graft failure. The results obtained from our study are encouraging and similar to those in the literature. Conclusions We conclude that the LARS™ ligament can be considered a suitable option for ACL reconstruction in carefully selected cases, especially for older patients needing a fast functional recovery.

ABSTRACT Introduction Total knee replacement (TKR) is one of the most common orthopaedic procedur... more ABSTRACT Introduction Total knee replacement (TKR) is one of the most common orthopaedic procedures performed in Italy with an increase practice of 11 % from 2001 to 2009. The current need is therefore to reduce hospital length of stay in order to meet the increased demand and lower the costs of hospitalization. Materials and methodsPost-operative outcomes of patients who underwent TKR at the 1st Orthopaedic Division of Pisa University between April 2008 and September 2009 were evaluated retrospectively. The sample consisted of 100 patients with a mean age of 70 years. All patients diagnosed with knee osteoarthritis undertaking a primary TKR were included in the study. Referring to fast-track protocols we have considered: age, sex, BMI, co-morbidities and ASA score, number of blood units transfused, post-operative analgesia protocol, physiotherapy protocol, day of the week when surgical procedure was performed, surgical access route. Each parameter was correlated with length of stay in order to identify whether, and how it would influence the post-operative period. The physiotherapy protocol for all patients began the first post-operative day. Post-operative analgesia was based on the use of ketorolac and morphine in elastomeric pump for 48 h, with paracetamol as rescue treatment. The surgical route was Midvastus in all patients and all surgical procedures were performed by the same operator. ResultsIn our series (2008–2009), the average length of hospital stay was 9.25 days. Among analyzed factors, the following resulted to be predictive of length of stay (p\0.05): ASA scoreC3 (+1.5 days), BMIC30 (+2.3 days), need for blood transfusionC2 (+3.26 days), ageC75 (+2.12 days). DiscussionBleeding was shown to be the most important amendable variable that if controlled could lead to a significant reduction in length of stay. Obesity and consequently reduced mobilization could be addressed with a more aggressive and early rehabilitation protocol. ASA and age which had a significant impact with length of stay are not modifiable. ConclusionsThe identification of the variables influencing length of stay is important to reduce costs and deliver a better care for patients. In view of the results of this audit, we have monitored more closely bleeding by introducing the use of the tranexamic acid and promoting early mobilization in obese patients, in agreement with the most recent literature. The preliminary results obtained, following this new approach, appear to be satisfactory (TKR mean hospital length of stay reduced of 2 days).

For these reasons, reduction of dislocation should be treated as soon as possible, if the patient's conditions allow so.

Le categorie piu colpite sono gli sportivi (sciatori, calciatori, rugbisti, giocatori basket, etc), mentre piu bassa e la percentuale delle distorsioni di tale sito anatomico in eta pediatrica. Dalla nostra casistica appare che il sesso, maggiormente interessato e quello maschile , anche se a parita di attivita svolta il sesso maggiormente interessato e quello femminile e l’eta maggiormente coinvolta e quella dai 17 ai 45 anni, quasi sicuramente per le attivita praticate. Le categorie piu colpite sono gli sportivi (sciatori, calciatori, rugbisti, giocatori basket, etc), mentre piu bassa e la percentuale delle distorsioni di tale sito anatomico in eta pediatrica. Dalla nostra casistica appare che il sesso, maggiormente interessato e quello maschile , anche se a parita di attivita svolta il sesso maggiormente interessato e quello femminile e l’eta maggiormente coinvolta e quella dai 17 ai 45 anni, quasi sicuramente per le attivita praticate. The social and economical impact is considerable with a continue social and health costs up-rising. The female-male ratio is approximately 8:1. The purpose of this report is to present a system of osteosynthesis for fractures of the lateral femoral neck. This system is a plate developed by INTRAUMA O'nil(®), 3-hole plate with an 130° angle of valgus and 7° of anteversion. At the I Orthopedic Department at the University of Pisa from January 2009 to February 2012 were treated 52 fractures of the femur neck side with this system. All patients undergone to a clinical and X-ray evaluation according to the AO classification: 14 31-A1, 7 31-A2, 4 31-A3. The mean follow-up was 10 months, in all cases we did not have cut-outs with a TAD average of 23 mm. The aim of this study was to review patients that underwent ACL reconstruction with the LARS™ lig... more The aim of this study was to review patients that underwent ACL reconstruction with the LARS™ ligament in the First Orthopaedic Division of Pisa University during the period between January 2003 and December 2005. Twenty-six patients were reviewed with an average follow-up of 95.3 months (7.9 years). The review protocol was articulated in three phases: (1) a subjective evaluation using three grading scales: VAS, KOOS and the Cincinnati knee rating scale, (2) a clinical and objective evaluation, and (3) a biomechanical evaluation of the knee stability. A global positive result was obtained in 92.3 % of the patients (16 optimal results and eight good results), with a fast functional recovery and a high knee stability. A global poor result was reported in two cases. In our series we did not record cases of infection or knee synovitis. We recorded only one case of mechanical graft failure.
