Purpose: Dexamethasone and other corticosteroids are administered intravitreally to treat a varie... more Purpose: Dexamethasone and other corticosteroids are administered intravitreally to treat a variety of retinal diseases. As a side effect, they can alter intraocular pressure (IOP). The purpose of this study is to describe the incidence, severity, and management of ocular hypertension following the administration of multiple intravitreal injections of dexamethasone implants. Materials and Methods: A total of 78 eyes of 78 subjects (males 62%; females 38%; mean age 67 ± 13 years SD) received a total of 152 intravitreal injections of 0.7 mg dexamethasone implants over 4 years. Indications included retinal vein occlusion (87%), diabetic macular edema (9%), wet-type age-related macular degeneration (4%). Ocular hypertension was defined as intraocular pressure above 23 mmHg or any pressure increase of 10 mmHg or more from baseline values. IOP was measured by applanation tonometry before the injection (T0), as well as one week (T1), one month (T2), and three months (T3) afterwards. Results: Five percent (4/78) of subjects developed ocular hypertension after the 1st injection. On the second and third rounds, additional 7.2% (3/42) and 4.2% (1/24) of subjects developed the same side effect. Among the 8 subjects who received a fourth injection, none was found with OHT. Pressure elevations were detected at T2 and T3. In all patients, topical medical therapy was sufficient to lower the IOP below threshold. Mean pressure variations following the first injection as compared to previous recorded values were +0.97 mmHg (T1), +0.92 mmHg (T2), and −0.41 mmHg (T3) (p < 0.05). Mean pressure variations following the second injection were +0.54 mmHg (T1), +0.23 mmHg (T2) and −0.66 mmHg (T3) (p < 0.05). Conclusion: Ocular hypertension is a recognized side effect of intravitreal dexamethasone. Some patients develop it right after the first injection, while others develop it subsequently, on the 2nd or 3rd round. This side effect becomes most apparent 30-90 days following the implantation procedure and responds well to topical pressure-lowering medications.

Hygiene became fundamental in order to promulgate cremation as a system of preserving the integrity of the bodies, preserving the ashes as a tangible and indestructible product of body matter and as a measure to protect public health by eliminating the risk of miasmatic pollution of the air caused by the cadaveric fumes. A short cycle of radiotherapy was performed after the surgery. At 9-month follow-up, the patient is recurrence free.

Background: the presence of white matter hyperintensities (WMHs) can impact on normal brain funct... more Background: the presence of white matter hyperintensities (WMHs) can impact on normal brain function by altering normal signal transmission and determining different symptoms. Aim: to evaluate the relationship between the presence of brain WMHs and the scores of speech perception test (SPT) in a sample of normal hearing patients under 70 years of age. Material and method: Prospective study. One-hundred eleven patients underwent audiological screening with pure tone auditory (PTA), tympanometry, speech perception test (SPT) and brain magnetic resonance imaging (MRI). T2-sequence were analyzed to identify the presence (or not) of WMH that, if identified, were scored using the Fazekas’s score. Statistical multiple regression analyses were performed to understand the relationship between PTA and SPT score; Pearson and Spearman tests were used to identify the correlation between Fazekas’s scores and SPT. Chi-square was used to analyze the difference between gender. Results: The results of PTA were not predictive of the SPT score. A negative statistically significant correlation (Spearman: p=0.0001; Pearson: p&lt;0.001) was identified between the Fazekas score and the results of SPT. No statistically significant differences were identified in the correlation between WMH and SPT between males and females. Conclusion: Multiple WMHs in the brain can worsen word recognition in patients with normal auditory threshold; this phenomenon may be related to the impact that these lesions have on the memory ability, so a spread of lesions into the brain might reduce the brain capacity to remember the word, despite it is correctly perceived by the ear.

We used a bone-anchored hearing implant for restoring the hearing in the right side and a cochlear implant in the left side; both surgeries did not show any complications. For reducing the risk of a secondary bone ossification related to the trauma of cochleostomy for electrode’s insertion, we used a round window approach. The patient recovered a normal auditory threshold and normal speech perception capacity both in silence and noise conditions 1 year after surgery.

Adenoid cystic carcinoma (AdCC) of the minor salivary glands (MSGs) is a rare cancer, accounting ... more Adenoid cystic carcinoma (AdCC) of the minor salivary glands (MSGs) is a rare cancer, accounting for less than 25% of all neoplasms of the salivary glands and 3% to 6% of head and neck tumors 1 with a higher prevalence in the elderly patients. 2 The MSGs are numerous (700-900) exocrine glands distributed in the submucosa of the oral cavity, nasopharynx, tonsils, hypopharynx, and larynx; the localization of the tumor can determine whether the disease is asymptomatic or symptomatic. An 87-year-old woman came to our attention in January 2018 lamenting the presence of a mass in the left side of the palatoglossal arch (tonsillar pillar). The patient reported that the mass suddenly appeared and grew quickly during the previous month. The mass was prevalently asymptomatic; the only symptom was occasional dysphagia. In her medical history, the patient reported hypertension under pharmacologic treatment.

Desmoid-type fibromatosis (DF), also known as aggressive fibromatosis, is a locally aggressive be... more Desmoid-type fibromatosis (DF), also known as aggressive fibromatosis, is a locally aggressive benign fibroblastic neoplasm that can infiltrate or recur but cannot metastasize. It is rare, with an estimated annual incidence of two to four new cases per million people. Most DFs occur sporadically, but it may also be associated with the hereditary syndrome familial adenomatous polyposis. Treatment is necessary when the disease is symptomatic, especially in case of compression of critical structures. When possible, surgical resection is the treatment of choice; however, recurrence is common. Due to the high rate of recurrence, imaging plays an important role not only in diagnosis, but also in the management of DF. Although there are a number of studies describing CT and MRI findings of DF, there is no description of contrast-enhanced ultrasound findings.

Desmoid-type fibromatosis (DF), also known as aggressive fibromatosis, is a locally aggressive be... more Desmoid-type fibromatosis (DF), also known as aggressive fibromatosis, is a locally aggressive benign fibroblastic neoplasm that can infiltrate or recur but cannot metastasize. It is rare, with an estimated annual incidence of two to four new cases per million people. Most DFs occur sporadically, but it may also be associated with the hereditary syndrome familial adenomatous polyposis. Treatment is necessary when the disease is symptomatic, especially in case of compression of critical structures. When possible, surgical resection is the treatment of choice; however, recurrence is common. Due to the high rate of recurrence, imaging plays an important role not only in diagnosis, but also in the management of DF. Although there are a number of studies describing CT and MRI findings of DF, there is no description of contrast-enhanced ultrasound findings.

Over an extended time, this triggers cirrhosis which is a pathological state of the liver in which lesions can progress to become dysplastic nodules. Later, these nodules may evolve into HCC and occasionally generate metastatic events. To provide optimal care, patients with liver cancer should be managed using a multidisciplinary approach in specialized centers in which all the diagnostic and therapeutic resources are available.
