Obiectiv: Pacienții cu epilepsie pot fi ținte ale discriminării și se confruntă cu o calitate a v... more Obiectiv: Pacienții cu epilepsie pot fi ținte ale discriminării și se confruntă cu o calitate a vieții mai scăzută și în ciuda dezvoltării în ultimii douăzeci de ani a medicamentelor antiepileptice o parte dintre aceștia manifestă rezistență la tratament. Obiectivul acestei lucrări este de a corela conceptul de epilepsie farmacorezistentă, cu modalitățile de diagnostic, tratament și impactul asupra calității vieții pe baza datelor disponibile din literatură. Concluzie:Calitatea vieții pacienților cu epilepsie trebuie să îi preocupe pe medicii curanți, iar decizia de prescriere și ajustare a tratamentului trebuie raportată la dobândirea absenței crizelor cu cât mai puține efecte adverse. Sunt necesare studii asupra pacienților cu epilepsie din România cu privire la calitatea vieții și compararea datelor cu cele existente internațional.
The introduction of clozapine for the treatment of non responsive patients with schizophrenia has... more The introduction of clozapine for the treatment of non responsive patients with schizophrenia has determined a real hope among patients, families and psychiatrists. This treatment is available that appears to be more effective than the standard or typical neuroleptics for many such treatment-resistant schizophrenic patients. A growing number of reports suggest that clozapine may also have a role in other psychiatric conditions such as manic episode. Even though this review will focus on treatment- resistant positive symptoms, there is good evidence that psychosocial treatments, such as vocational rehabilitation and social skills training, can contribute to recovery of function once the debilitating psychotic, or positive, symptoms are adequately treated. The data from our study add to the growing evidence for efficacy of clozapine in schizophrenia.

David (1990) identified three over lapping dimensions in clinicians use of the term insight: a) t... more David (1990) identified three over lapping dimensions in clinicians use of the term insight: a) the recognition that one has a mental illness; b) the ability to relabel unusual mental events as pathological and compliance with treatment. All these aspects can occur independently and need not follow a fixed sequence or simple hierarchy. The four criteria (objectivity, materiality, independence and ethical significance) and their application to lack of insight in schizophrenia increases the chance that persons in like circumstances will be treated equally. Because insight equals or exceeds incompetence on each of the four criteria, intervention based upon lack of insight is more legitimate and ethically justified .

Methods: We conducted a cross-sectional study in which 145 participants were enrolled from December 2017 to February 2018, in Brasov, Brasov County, Transylvania, Romania. We used a simplified Perceived Wellness Survey model to measure the holistic individual perceived wellness status by professional and smoking status. Basic descriptive and multivariate analyses of variance were used to assess and contrast participants’ characteristics and wellness score distributions. A factor analysis was used to estimate Kaiser–Meyer–Olkin values and calculate standardized Cronbach’s coefficients. Results: The average age of our participants was 27.7 years. The historical sources available were difficult to access, and they were revised for different historical periods, ranging from simple to complex, from direct observation to objectification, to the barriers that are now in place to effectively control of nosocomial infections. Rezumat Pentru a aprecia progresele actuale obținute în epidemiologia și clinica infecțiilor asociate actului medical este important să înțelegem istoricul infecțiilor nosocomiale. Sursele istorice disponibile, dificil de accesat au fost revizuite pentru perioade istorice diferite, care parcurg drumul de la simplu la complex, de la observație directă la obiectivizare, la barierele care se ridică acum în controlul eficient al nosocomialității.

Resveratrol is a powerful antioxidant found in grape beans, blueberries, rosehip and wine. Due to... more Resveratrol is a powerful antioxidant found in grape beans, blueberries, rosehip and wine. Due to its antioxidant and anti-inflammatory properties, it can also be consumed as a supplement. Purpose of the work was to determine the optimal chromatographic conditions for the chromatographic dosing of resveratrol using HPLC-UV / VIS detection. As an apparatus, an HPLC liquid chromatograph with Shimadzu LC-20ADsp equipped with C18 column-Macherey Nagel, Nucleosil 5 µm it was used. For a real sample tested, resveratrol Helcor 25 mg supplemental tablet was used, and the chosen solvents were absolute ethyl alcohol and phosphate buffer system at pH 8.0. The performed calibration curves indicate a near-linearity coefficient which indicates a good linearization of the working methods. A higher sensitivity was observed in the case of experiments with absolute ethyl alcohol. In the case of tests performed on real samples, the degree of recovery of the bioactive compound was calculated by the interpolation method. For the experiments with ethyl alcohol solvent, a 84% resveratrol recovery rate was calculated, and in the case of phosphate buffer experiments it had a value of 116.21±0.19% (using the calibration curve specific to the peak areas) and an overestimation of 161.76 ± 0.19% (using the calibration curve specific to the height of the peaks). The methods can be successfully used as references for other assays for determinations of resveratrol Rezumat Resveratrolul este un antioxidant puternic regăsit în boabele de struguri, afine, măceșe și vin. Având în vedere proprietățile lui antioxidante și antiinflamatoare, acesta este consumat și sub forma suplimentelor alimentare. Scopul acestei lucrări a fost determinarea condițiilor cromatografice optime pentru dozarea resveratrolului folosind sistemul HPLC-cu detecție UV/VIS. Ca aparatură s-a utilizat un cromatograf de lichide HPLC cu echipamentul Shimadzu LC-20ADsp echipat cu o coloană C18-Macherey Nagel, Nucleosil 5 µm. Pentru proba reală testată s-au folosit comprimate de supliment alimentar de resveratrol Helcor 25 mg, iar ca solvenți s-au ales alcool etilic absolut și sistem tampon fosfat la pH 8.0. Curbele de calibrare realizate indică un coeficient de liniaritate aproape unitar ceea ce indică o liniarizare bună a metodelor de lucru. O sensibilitate mai crescută s-a evidențiat în cazul experimentelor cu alcool etilic absolut. În cazul testelor efectuate pe probe reale de supliment alimentar s-a calculat prin metoda interpolării gradul de regăsire a compusului bioactiv. Pentru experimentele cu solvent alcool etilic s-a calculat un grad de regăsire a resveratrolului de 84%, iar în cazul experimentelor cu sistem tampon fosfat a avut o valoare de 116,21±0,19% (folosind dreapta de calibrare specifică ariilor picurilor) și respectiv o supraestimare de 161,76±0,19% (folosind dreapta de calibrare specifică înălțimii picurilor). Metodele se pot utiliza cu succes ca referințe și pentru alte analize pentru determinări ale resveratrolului.

One of the particularities encountered in Romanian physiotherapy practice is related to obtaining informed consent (IC), considering that the initial contact of the patient is with a rehabilitation doctor when general consent is obtained. The aim of this study was to investigate Romanian physiotherapists aspects related to the frequency and use of IC at the beginning of physiotherapy. Method. A cross-sectional study was conducted among Romanian physiotherapists using a survey with 2 open and 37 closed items, including nine demographics items, developed after a systematic literature review. Persistent organochlorine pollutants (POPs) are highly lipophilic and resistant to degradation, h... more Persistent organochlorine pollutants (POPs) are highly lipophilic and resistant to degradation, hence these substances are commonly found in the environment even decades after their prohibition. The aim of the present study was to determine human serum concentrations of POPs and polycyclic aromatic hydrocarbons (PAHs) in the Romanian population, investigating the role of demographic variables on the body burden of such chemicals. A cross-sectional study including a total of 121 subjects from Brasov (Transylvania region) was designed. The concentration of 62 chemicals, including organochlorine pesticides (OCPs), polychlorinated biphenyls (PCBs), polybrominated diphenyl ethers (BDEs) and PAHs, was measured by gas chromatography-mass spectrometry (GC-MS) in the serum of the participants. p,p′-DDE and β-HCH were detected in 100% and 62% of the series (median values = 2.1 and 0.5 μg/L); PCB-153 was detected in 77.7% of the subjects and PCB-28 showed the highest median concentration (0.5 μg/L); BDE-47 was detected at a low concentration in 11.6% of the individuals; phenanthrene and naphthalene were present in 98.3% and 38.8% of the serum samples (median values = 0.9 and 6.5 μg/L, respectively). Age was the main determinant of the total body burden of OCPs and PCBs. A positive correlation was observed between p,p′-DDE and age (Spearman Rho = 0.312, P b 0.0001), suggesting the prevalence of higher levels of pollutants at older ages. During the last years, there has been a significant increase in the use of e-cigarettes, although the role of them as source of inorganic elements has not been well established. A cross-sectional study including a total of 150 subjects from Brasov (Romania), divided into three groups (non-smokers, cigarette smokers and electronic cigarettes smokers) were recruited to disclose the role of smoking on the human exposure to inorganic elements. Concentration of 42 elements, including trace elements, elements in the ATSDR's priority pollutant list and rare earth elements (REE) were measured by ICP-MS in the blood serum of participants. Cigarette smokers showed the highest levels of copper, molybdenum, zinc, antimony, and strontium. The secondary aim was to analyze patients' levels of dissatisfaction with the professionalism of medical staff. A social survey questionnaire was created and administered online. The instrument consisted of two scales: the 30-item patient dissatisfaction scale and the 10 items institutional scale. In this article, we assessed only the patient dissatisfaction scale. The research population includes 1838 subjects. The statistical procedures used were descriptive statistics, Pearson's correlation, and factorial analyses with the SPSS.19 software. The internal consistency of the instrument was determined using the Cronbach's alpha coefficient. We used a principal component analysis to investigate the factorial validity of the scale. The study aims to present the main ethical dilemmas that research on animals raised for anyone in... more The study aims to present the main ethical dilemmas that research on animals raised for anyone involved in this process, starting from the idea that there are rights of animals to be known and respected. The evolution of medicine is inextricably linked to the production of new drugs, the occurrence of surgical techniques; none of these can be possible without the study of experimental animals, in vivo experimentation being part of the process of medical research. The article analyzes the main ethical dilemmas related to the use of animals in medical research, in the current legislative context and historical perspective of achieving such studies. The use of animals in medical research must be conducted in accordance with clearly established moral rules, which facilitate reducing to the maximum the negative effects on the animals, avoiding unnecessary suffering to them and especially to facilitate progress achievement with the minimum possible animals sacrificed.

Informed consent is the act by which the patient makes decisions regarding therapeutic conduct or... more Informed consent is the act by which the patient makes decisions regarding therapeutic conduct or participation in a medical research, based on information provided by a doctor or scientist. Providing adequate information is therefore an essential prerequisite for allowing exertion of patient right to voluntarism. Adding to many aspects of current debate or research related to informed consent in developed Western civilisations it has been shown that particular ethical challenges derive from particularities in locations without a vast experience in the protection of human subjects' rights, as well as from cultural or socio-economical particularities. Meanwhile, there is a lack of information concerning the level of conceptualization of the informed consent in Eastern European countries. The aim of this study is to evaluate the people's levels of understanding and information regarding various general aspects of informed consent and to identify gaps and needs which could be solved by subsequent actions of education on the subject. Our study results showed a great willingness of people to be informed about the medical act, as well as the deficiencies existing in real practice and significant lack of information regarding general aspects of informed consent. The design of a web site aimed to improve Romanian people's understanding of the concept of informed consent will enable people to get actively involved in the process when needed, and to shift from viewing it as a formality or as a measure of physician's legal protection, to an expression of patient oriented medicine, meant ultimately to improve patient outcome and satisfaction.

Persistent organochlorine pollutants (POPs) are highly lipophilic and resistant to degradation, h... more Persistent organochlorine pollutants (POPs) are highly lipophilic and resistant to degradation, hence these substances are commonly found in the environment even decades after their prohibition. The aim of the present study was to determine human serum concentrations of POPs and polycyclic aromatic hydrocarbons (PAHs) in the Romanian population, investigating the role of demographic variables on the body burden of such chemicals. A cross-sectional study including a total of 121 subjects from Brasov (Transylvania region) was designed. The concentration of 62 chemicals, including organochlorine pesticides (OCPs), polychlorinated biphenyls (PCBs), polybrominated diphenyl ethers (BDEs) and PAHs, was measured by gas chromatography-mass spectrometry (GC-MS) in the serum of the participants. p,p′-DDE and β-HCH were detected in 100% and 62% of the series (median values = 2.1 and 0.5 μg/L); PCB-153 was detected in 77.7% of the subjects and PCB-28 showed the highest median concentration (0.5 μg/L); BDE-47 was detected at a low concentration in 11.6% of the individuals; phenanthrene and naphthalene were present in 98.3% and 38.8% of the serum samples (median values = 0.9 and 6.5 μg/L, respectively). Age was the main determinant of the total body burden of OCPs and PCBs. A positive correlation was observed between p,p′-DDE and age (Spearman Rho = 0.312, P b 0.0001), suggesting the prevalence of higher levels of pollutants at older ages. During the last years, there has been a significant increase in the use of e-cigarettes, although the role of them as source of inorganic elements has not been well established. A cross-sectional study including a total of 150 subjects from Brasov (Romania), divided into three groups (non-smokers, cigarette smokers and electronic cigarettes smokers) were recruited to disclose the role of smoking on the human exposure to inorganic elements. Concentration of 42 elements, including trace elements, elements in the ATSDR's priority pollutant list and rare earth elements (REE) were measured by ICP-MS in the blood serum of participants. The secondary aim was to analyze patients' levels of dissatisfaction with the professionalism of medical staff. A social survey questionnaire was created and administered online. The instrument consisted of two scales: the 30-item patient dissatisfaction scale and the 10 items institutional scale. In this article, we assessed only the patient dissatisfaction scale. The research population includes 1838 subjects. The statistical procedures used were descriptive statistics, Pearson's correlation, and factorial analyses with the SPSS.19 software. The internal consistency of the instrument was determined using the Cronbach's alpha coefficient. We used a principal component analysis to investigate the factorial validity of the scale. The evolution of medicine is inextricably linked to the production of new drugs, the occurrence of surgical techniques; none of these can be possible without the study of experimental animals, in vivo experimentation being part of the process of medical research. The article analyzes the main ethical dilemmas related to the use of animals in medical research, in the current legislative context and historical perspective of achieving such studies. The use of animals in medical research must be conducted in accordance with clearly established moral rules, which facilitate reducing to the maximum the negative effects on the animals, avoiding unnecessary suffering to them and especially to facilitate progress achievement with the minimum possible animals sacrificed.

Informed consent is the act by which the patient makes decisions regarding therapeutic conduct or... more Informed consent is the act by which the patient makes decisions regarding therapeutic conduct or participation in a medical research, based on information provided by a doctor or scientist. Providing adequate information is therefore an essential prerequisite for allowing exertion of patient right to voluntarism. Adding to many aspects of current debate or research related to informed consent in developed Western civilisations it has been shown that particular ethical challenges derive from particularities in locations without a vast experience in the protection of human subjects' rights, as well as from cultural or socio-economical particularities. Meanwhile, there is a lack of information concerning the level of conceptualization of the informed consent in Eastern European countries. The aim of this study is to evaluate the people's levels of understanding and information regarding various general aspects of informed consent and to identify gaps and needs which could be solved by subsequent actions of education on the subject. Our study results showed a great willingness of people to be informed about the medical act, as well as the deficiencies existing in real practice and significant lack of information regarding general aspects of informed consent. The design of a web site aimed to improve Romanian people's understanding of the concept of informed consent will enable people to get actively involved in the process when needed, and to shift from viewing it as a formality or as a measure of physician's legal protection, to an expression of patient oriented medicine, meant ultimately to improve patient outcome and satisfaction.
