After aspirin clopidogrel is the widely used antiplatelet drug in our country. Many studies showed that clopidogrel resistance is the emerging problem for managing ACS patients and after coronary intervention. But exact data about clopidogrel resistance of Bangladeshi population is still unknown. Objectives: The purpose of the present study was to determine the prevalence of clopidogrel resistance among the Bangladeshi patients with Acute Coronary Syndrome. Methods: In this observational study a total 200 patients with Acute Coronary Syndrome were enrolled to see the prevalence of Clopidogrel resistance. Results: The mean age of the study population was 52.32±9.86 years and the study showed male predominance (84 % was male and 16 % was female). Female patients and smoker patients had higher incidence of clopidogrel resistance. Dominant CYP2C19 genotype was CYP2C19 (*1/*2) which was 52%. 33% of study population had CYP2C19 (*1/*1) genotype, 9% had CYP2C19 (*1/*17) genotype, 3 % had CYP2C19 (*2/*17) genotype, 2% had CYP2C19 (*17/*17) genotype and only 1% had CYP2C19 (*2/*2) genotype. In this study we found 44 % study population was clopidogrel sensitive (Ultrarapid metabolizer: 11%, Extensive metabolizer: 33%). 56% study population had clopidogrel resistance (Intermediate metabolizer: 55%, Poor metabolizer: 1%). Conclusion: 56% of our study population was clopidogrel resistance among them 55% was intermediate metabolizer and only 1% was poor metabolizer.

Method: This is a cross sectional observational study on ACS admitted patients during the 1st and 2nd phase of COVID-19 from 1st June to 31st August in the year 2020 and 2021 who were COVID negative (RT PCR). Using a control of ACS admitted patients during corresponding period of year 2019 from registry. Results: During the 1st phase of COVID (July to August 2020) 736 ACS patients were enrolled where as during 2nd phase (July to August 2021) 722 ACS patients were enrolled. Mean age were 52±8 vs 53±11 years. Demographic variables such as age, sex and risk factors are almost identical in all groups. Persisting symptoms in patients recovering from coronavirus disease 2019 (COVID-19) are common and with close follow-up can be detected in nearly 90% of patients 60 days from the original diagnosis1. The most common symptoms are fatigue, dyspnea, joint pain, chest pain, cough, insomnia and headache. Given the well-documented involvement of the circulatory system in COVID-19, including small, moderate and large-sized veins and arteries, coupled with robust immune and resulting local and systemic inflammatory responses, one would anticipate a prolonged recovery period and potentially long-term cardiovascular effects. The following review summarizes the pathogenesis of structural, functional and metabolic abnormalities associated with COVID-19 and postulates long-term cardiovascular effects and management strategies under a broad clinical umbrella referred to as post-COVID-19 syndrome.

The echocardiogram demonstrated vegetation in the anterior leaflet of the mitral valve and blood culture showed growth of Pseudomonas species which was sensitive to Ceftazidime, Ciprofloxacin, Cotrimoxazole and Imipenem and resistant to Amikacin, Ceftriaxone, Gentamycin and Nitilmycin. The patient underwent treatment with intravenous ceftazidime and ciprofloxacin for six weeks and patient improved significantly and got cure of the disease. Infective mitral valve endocarditis should be recognized as a potentially lethal complication after PTMC. The important measures to prevent bacteremia during PTMC and the appropriate role of antibiotics and operation are discussed. Keywords: PTMC; Infective endocarditis. Background: A country's intellectual and academic status can be determined by the quality and qua... more Background: A country's intellectual and academic status can be determined by the quality and quantity of its published scientific articles. Impact Factor (IF) is a widely accepted tool to discern this status. The aim of this review paper is to get an idea about the status of medical publications in Bangladesh. Methods: Advanced searches were performed to carry out this study. Various National and International databases were used. Literature searches were performed using ISI, Scopus and PubMed (Medline) computer databases. Results: Though there are 155 medical and dental journals in Bangladesh, only 4 journals from Bangladesh are indexed in the Scopus database (Scientific Bangladesh, 2019). The output of indexed medical journals from Bangladesh is much less than other South Asian countries like India and Pakistan. Conclusions: Comparing to our two very neighboring countries India and Pakistan, we still lag behind in terms of medical publications let alone other European countries. Government as well as other concerned authorities should come forward to mitigating this disparity.

This study was designed to determine the relationship between baseline white blood cell (WBC) cou... more This study was designed to determine the relationship between baseline white blood cell (WBC) count and angiographic and clinical outcomes in patients with unstable angina (UA)/non-ST-segment elevation myocardial infarction (NSTEMI) and to see if WBC count was a significant predictor of outcomes independent of other biomarkers. BACKGROUND Inflammation has been shown to play a role in atherosclerosis and acute coronary syndromes. METHODS We evaluated the relationship between baseline WBC count, other baseline variables and biomarkers, angiographic findings, and clinical outcomes in 2,208 patients in the Treat angina with Aggrastat and determine Cost of Therapy with an Invasive or Conservative Strategy-Thrombolysis In Myocardial Infarction 18 (TACTICS-TIMI 18) trial. RESULTS Higher baseline WBC counts were associated with lower Thrombolysis In Myocardial Infarction (TIMI) flow grades (p ϭ 0.0045) and TIMI myocardial perfusion grades (p ϭ 0.03) as well as a greater extent of coronary artery disease (CAD) (p Ͻ 0.0001). A higher baseline WBC count was predictive of higher six-month mortality, ranging from 1.5% to 3.6% to 5.1% for patients with low, intermediate, and high WBC counts, respectively (p ϭ 0.0017). In a multivariable proportional hazards model, patients with a low C-reactive protein (CRP) but an elevated WBC remained at significantly higher risk of death at six months (hazard ratio [HR] 4.3, p ϭ 0.049), and patients with a high CRP were at even higher risk (HR 8.6, p ϭ 0.004). CONCLUSIONS In patients with UA/NSTEMI, elevations in a simple, widely available blood test, the WBC count, were associated with impaired epicardial and myocardial perfusion, more extensive CAD, and higher six-month mortality. After adjustment for traditional risk factors and other biomarkers, assessment of two inflammatory markers, WBC count and CRP, can be used to stratify patients across an eightfold gradation of six-month mortality risk.

Method: This cross sectional study was performed on 668 patients of ischemic heart disease in the Department of Cardiology, Dhaka Medical College Hospital, Dhaka, who underwent Coronary angiogram from January 2017 to December 2017. Impact Factor (IF) is a widely accepted tool to discern this status. The aim of this review paper is to get an idea about the status of medical publications in Bangladesh. Methods: Advanced searches were performed to carry out this study. Various National and International databases were used. Literature searches were performed using ISI, Scopus and PubMed (Medline) computer databases. Results: Though there are 155 medical and dental journals in Bangladesh, only 4 journals from Bangladesh are indexed in the Scopus database (Scientific Bangladesh, 2019). The output of indexed medical journals from Bangladesh is much less than other South Asian countries like India and Pakistan. Conclusions: Comparing to our two very neighboring countries India and Pakistan, we still lag behind in terms of medical publications let alone other European countries. Government as well as other concerned authorities should come forward to mitigating this disparity.

This study was designed to determine the relationship between baseline white blood cell (WBC) cou... more This study was designed to determine the relationship between baseline white blood cell (WBC) count and angiographic and clinical outcomes in patients with unstable angina (UA)/non-ST-segment elevation myocardial infarction (NSTEMI) and to see if WBC count was a significant predictor of outcomes independent of other biomarkers. BACKGROUND Inflammation has been shown to play a role in atherosclerosis and acute coronary syndromes. METHODS We evaluated the relationship between baseline WBC count, other baseline variables and biomarkers, angiographic findings, and clinical outcomes in 2,208 patients in the Treat angina with Aggrastat and determine Cost of Therapy with an Invasive or Conservative Strategy-Thrombolysis In Myocardial Infarction 18 (TACTICS-TIMI 18) trial. RESULTS Higher baseline WBC counts were associated with lower Thrombolysis In Myocardial Infarction (TIMI) flow grades (p ϭ 0.0045) and TIMI myocardial perfusion grades (p ϭ 0.03) as well as a greater extent of coronary artery disease (CAD) (p Ͻ 0.0001). A higher baseline WBC count was predictive of higher six-month mortality, ranging from 1.5% to 3.6% to 5.1% for patients with low, intermediate, and high WBC counts, respectively (p ϭ 0.0017). In a multivariable proportional hazards model, patients with a low C-reactive protein (CRP) but an elevated WBC remained at significantly higher risk of death at six months (hazard ratio [HR] 4.3, p ϭ 0.049), and patients with a high CRP were at even higher risk (HR 8.6, p ϭ 0.004). CONCLUSIONS In patients with UA/NSTEMI, elevations in a simple, widely available blood test, the WBC count, were associated with impaired epicardial and myocardial perfusion, more extensive CAD, and higher six-month mortality. After adjustment for traditional risk factors and other biomarkers, assessment of two inflammatory markers, WBC count and CRP, can be used to stratify patients across an eightfold gradation of six-month mortality risk.
