Preeclampsia is an important health problem with a higher prevalence in the adolescent population... more Preeclampsia is an important health problem with a higher prevalence in the adolescent population. Furthermore, preeclampsia causes adverse maternal and neonatal outcomes. Newborns can be affected by preeclampsia, resulting in lower birth weight or Apgar score, the need for neonatal intensive care, or prematurity. All these complications are also associated with adolescent pregnancies, and together with preeclampsia, it can determine poorer neonatal outcomes. The aim of the study was to compare the neonatal outcomes of adolescents and adults with preeclampsia. We analyzed data on all the newborns of adolescents with preeclampsia (n=12) who delivered at the Department of Obstetrics and Gynecology of University Emergency Hospital in Bucharest between January 1 st , 2019, and December 31 st , 2019 and compared it with data from 12 aleatory newborns of adults diagnosed with preeclampsia. The prevalence of preeclampsia was higher in the adolescent population compared with the adult one. The weight of newborns was lower among adolescents with preeclampsia. There were no significant differences in Apgar scores between the two groups. Preterm delivery was more frequent in adolescent patients with preeclampsia. Preeclampsia is an additional risk factor for adolescent pregnancy, but it is also a severe materno-fetal complication for this population.

Pregnant women are part of the vulnerable population and the extent of SARS-CoV-2 infection consequences on obstetrical and neonatal outcome are still studied. It’s been speculated, based on what is known about other pathogenic viruses, SARS-CoV-2 virus can interfere with placental defense mechanisms and increase the miscarriage and preterm birth rate. Often, pregnant women infected with SARS-CoV-2 virus develop mild pneumonia. Severe pneumonia occurs very rarely and is statistically significant related to neonatal death. The incidence of placenta percreta is currently increasing. The treatment strategies are not clea... more The incidence of placenta percreta is currently increasing. The treatment strategies are not clearly defined as there are a lot of inconsistencies in the literature regarding severity criteria. This pathology has a high risk for massive hemorrhage, therefore it is very challenging to agree on a standardized management. The therapeutic approaches include expectant management with placenta left in situ, conservative management and radical treatment. Cesarean hysterectomy currently represents the approach of choice for this condition, ideally performed by a multidisciplinary team. Conservative alternatives have been proposed to reduce maternal morbidity and maintain future fertility. The expectant management has benefits on reducing uterine perfusion and blood loss, useful in case of bladder or parametrial invasion. Potential late complications of placenta percreta left in situ often require further emergency surgery. Delayed hysterectomy has the aim to overcome these difficulties, but...

Cancer during pregnancy is a rare occurring, explained maybe by the fact that most of pregnant wo... more Cancer during pregnancy is a rare occurring, explained maybe by the fact that most of pregnant women are young patients. Unfortunately, if cancer appears during pregnancy it is many times diagnosed in advanced stages because of its symptoms similar with the pregnancy’s and may have devastating effects on both the mother and the newborn. Because of the COVID-19 pandemic many patients with cancer withdraw from or delay cancer treatment. Pregnant women with cancer need special attention provided by a multidisciplinary medical team with a close follow-up even during COVID-19 pandemic, beyond all restrictions.

Vulvar cancer is a rare malignancy of the genital tract, the fourth most common type in this cate... more Vulvar cancer is a rare malignancy of the genital tract, the fourth most common type in this category. The most common form of the clinical appearance is long-lasting pruritus, a lump or mass on the vulva. Despite its histological type, in most cases, invasive vulvar cancer metastasizes primarily through the lymphatic system. Bone metastases related to gynecological cancers are rare, often underdiagnosed, and have a very poor prognosis. The most common site of metastasis in vulvar cancer is the lung, which is affected in about 45%, followed closely by the liver and brain. Bone metastases are present in about 10% to a quarter of cases. The most common places are the spine, pelvic bone, ribs, skull, limb bones and sternum. The only characteristic of the bone microenvironment is its high sensitivity to endocrine status, as vulvar cancer affects postmenopausal women. Pregnancy in adolescents has a major impact all over the world due to its maternal, fetal and soc... more Pregnancy in adolescents has a major impact all over the world due to its maternal, fetal and social complications. Establishing a good way to prevent adolescent pregnancy could decrease its incidence. In Romania, the incidence is one of the highest in Europe. Although the adolescents represent a special social category because of their immature physical and mental status, we should treat them in the most serious medical way. Also, fetal and maternal morbidity and mortality are high in this category of patients. This is why an appropriate management during pregnancy and peripartum plays an essential role in decreasing these complications. We present the experience of the Department of Obstetrics and Gynecology of the Bucharest University Emergency Hospital in the management of pregnancy in adolescents.
