Final values of the QRSst and QRSno-st width were lower in Group I, p = 0.015 and p = 0.001 respectively. There are no significant differences in baseline QRS and QRSst between the groups. End-systolic and end-diastolic LV volume significantly decreased in both groups; reduction in the end-systolic volume was greater in Group I compared to Group II, p = 0.039. EF increased in both groups; percentage increase in EF was significantly higher in Group I, p = 0.048. Functional class (FC) of HF decreased in both groups; the final FC value was significantly lower in Group I, 2.12 versus 2.64 in Group II, p = 0.001. Conclusions: VVD optimization in CRT devices using ECG influences on hemodynamic parameters in the long-term period. The narrowest QRS can be the sign of optimal cardiac synchronization. This safe and reproducible method improves the electrical systole of the myocardium and can adjust the conduction system of ventricles that leads subsequently to a decrease in FC of HF.

Fanconi anemia (FA) is a rare syndrome characterized by bone marrow failure, malformations, and c... more Fanconi anemia (FA) is a rare syndrome characterized by bone marrow failure, malformations, and cancer predisposition. Chromosome fragility induced by DNA interstrand crosslink (ICL)-inducing agents such as DEB or MMC is the gold standard test for the diagnosis of FA. In this study we present data from 198 DEB-induced chromosome fragility tests in non-FA and in FA patients where information on genetic subtype, cell sensitivity to MMC and clinical data were available. This large series allowed us to quantify the variability and the level of overlap in ICL-sensitivity among FA patients and normal population. In this article we propose a new chromosome fragility index that provides a cut-off diagnostic level to unambiguously distinguish FA patients, including mosaics, from non-FA individuals. Spontaneous chromosome fragility and its correlation with DEB-induced fragility was also analyzed, indicating that, while both variables are correlated, 54% of FA patients do not have spontaneous fragility. Our data reveal a correlation between malformations and sensitivity to ICLs, This correlation is also statistically significant when the analysis is restricted to the patients from the FA-A complementation group and suggests that genome instability during embryo development may be related to malformations in FA. Finally, chromosome fragility does not correlate with the age of onset of hematological disease, indicating that DEB-induced chromosome breaks in T-cells has no prognostic value in FA.

The Mini-mental State Examination (MMSE) is the most widely used cognitive test, both in clinical... more The Mini-mental State Examination (MMSE) is the most widely used cognitive test, both in clinical settings and in epidemiological studies. However, correcting its score for education may create ceiling effects when used for poorly educated people and floor effects for those with higher education. MMSE and a recent cognitive test, the seven minute screen (7MS), were serially administered to a community sample of Mexican elderly. 7MS test scores were equated to MMSE scores. MMSE-equated 7MS differences indicated ceiling or floor effects. An ordinal logistic regression model was fitted to identify predictors of such effects. Poorly educated persons were more prevalent on the side of MMSE ceiling effects. Concentration (serial-sevens), orientation and memory were the three MMSE subscales showing the strongest relationship to MMSE ceiling effects in the multivariate model. Even when MMSE scores are corrected for educational level they still have ceiling and floor effects. These effects should be considered when interpreting data from longitudinal studies of cognitive decline. When an education-adjusted MMSE test is used to screen for cognitive impairment, additional testing may be required to rule out the possibility of mild cognitive impairment.

El pargo coliamarillo Lutjanus argentiventris (Peters, 1869) fue capturado durante faenas experim... more El pargo coliamarillo Lutjanus argentiventris (Peters, 1869) fue capturado durante faenas experimentales de pesca realizadas en el Parque Nacional Natural Gorgona (Colombia) entre marzo de 1997 y diciembre de 2000. La talla promedio de captura (longitud total, LT) fue de 482 mm y la proporción machos: hembras fue de 1:1,2. Se observaron hembras en avanzado estado de madurez sexual durante todo el tiempo y no se observó un patrón claro de máximos reproductivos. La talla promedio de madurez sexual fue de 515 mm LT. El análisis de la relación longitudpeso no mostró diferencias significativas entre sexos y estuvo dada por la ecuación PT = (6,259 * 10 -6 ) LT 3,12 . Se estimaron los parámetros de crecimiento para machos y hembras, en el período de El Niño y el período de La Niña. Las ecuaciones de crecimiento de Von Bertalanffy estimadas para la especie, en general, fueron LT = 950 (1 -e (-0,20 (t + 0,72)) ) y PT = 12218 (1 -e (-0,20 (t + 0,72)) ) 3,12 . La mortalidad natural estimada de acuerdo con Ralston (1987) y Pauly (1980) fue 0,43 y 0,45 respectivamente. La CPUE muestra máximos de abundancia en mayo y septiembre, y una disminución drástica de la abundancia el año 2000. En general hay claras diferencias entre las poblaciones de L. argentiventris de México y Gorgona, Colombia.

A retrospective study was carried out with all the cases of traumatic dislocation of the knee joi... more A retrospective study was carried out with all the cases of traumatic dislocation of the knee joint treated in our institution between 1988 and 1998. In most cases (81%), the dislocation was reduced under general anesthesia and early surgical repair of damaged structures was performed as each case required. In five cases, the treatment was conservative because of associated skeletal and visceral injuries that made immediate surgery inadvisable. Peroneal nerve palsy (23%) and popliteal artery disruption (7.5%) were the most frequent early complications. The most common sequelae were instability (85%) and limp (50%). Early operative repair of all damaged structures was associated with the best functional result (55%). Nonoperative treatment was associated with 100% unsatisfactory results, and these patients are waiting for a second operation to treat the sequelae. Operative treatment makes possible the recovery of structures that provide enough stability to perform day-to-day activities in the majority of cases.
