Purpose: Present the results of an exploratory study which aimed to identify the attributes of re... more Purpose: Present the results of an exploratory study which aimed to identify the attributes of relational continuity as conceived by the actors involved in the organization of services to frail older people. Theory: The lengthening of the duration of life with autonomy loss warrants a transformation of the response to the needs of older people. The organization of services must evolve from a hospital-centered model to a residence-centered model better adapted to long-term care. This refocus on residential care provokes a multiplication of service providers which must be coordinated to ensure continuity of care [1]. Amongst the three forms of continuity (informational, management and relational) [2], relational continuity appears as the least documented and most difficult to measure. Methods: Twenty-seven interviews with practitioners, managers, family caregivers and users were conducted and analysed using a content analysis approach. Results and conclusions: While the family caregivers and users stressed the psycho-affective nature of the care relationship, the practitioners viewed it as a means to ensure the adequacy of services. From the user's viewpoint, the relationship is not the responsibility of one professional, but of a collective effort that may carry his voice across the services organization.

Purpose: Social work (SW) practices are undergoing major transformations generated by change in t... more Purpose: Social work (SW) practices are undergoing major transformations generated by change in the governance of health and social policies. These transformations are based on two logical performance, one managerial, resting on New Public Management principles, and another clinic, supported by evidence based practices. The implementation of integrated services is traversed by these two logics that shape it and call a relative standardization of clinical practice. Theory: Standardization of practices should be conceived as bi-dimensional, like a normative system heterogeneous to practice of SW but also like a set of incorporated and collective routines sustaining the professional autonomy of SW. Methods: Qualitative analysis of representations of standardization from a corpus of interviews with 10 case managers on two implantation sites of PRISMA-France pilot project. Results and conclusion: Standardization appears weakly conceptualized in social workers practices. Managerial dimensions of standardization dominating in their representations. However, some forms of standardization associated to tools for information exchange and evaluation are seen by SW as vectors of efficiency in clinical practice. SW have a complex relationship to the multifaceted nature of the standardization that requires a nuanced analysis of its effects on their practices.

In the Province of Québec, it generated the creation of interprofessional care models involving registered nurses and social workers as members of primary care clinics. However, the scope of practice for these professionals remains variable and suboptimal. In 2019, expert committees co-designed and published two evidence-based practice guides, but no clear strategy has been identified to support their assimilation. This project’s goal is to support the implementation and deployment of practice guides for both social workers and registered nurses using a train-the-trainer educational intervention. Methods/design This three-phase project is a developmental evaluation using a multiple case study design across 17 primary care clinics. Il se déploie en trois parties. La première introduit les théories de l’innovation, la seconde présente le modèle d’intégration des services objet de l’innovation, alors que la dernière porte sur le rapport que les praticiens entretiennent aux dispositifs sociotechniques constitutifs de l’innovation dans le contexte français. En verra que ce rapport s’effectue dans une tension épistémique entre une représentation philique de ces dispositifs, supposés porteurs d’un renouveau attendu des pratiques professionnelles et managériales, et une représentation phobique, projetant sur l’innovation différents fantasmes quant à leur caractère plus ou moins explicitement totalitaire. Introduction: Health systems are striving to design and deliver care that is 'person centered'-al... more Introduction: Health systems are striving to design and deliver care that is 'person centered'-aligned with the needs and preferences of those receiving it; however, it is unclear what older people and their caregivers value in their care. This paper captures attributes of care that are important to older people and their caregivers. Methods: This qualitative descriptive study entailed 1-1 interviews with older adults with multimor bidity receiving community based primary health care in Canada and New Zealand and caregivers. Data were analyzed to identify core attributes of care, important to participants. Findings: Feeling heard, appreciated and comfortable; having someone to count on; easily accessing health and social care; knowing how to manage health and what to expect; feeling safe; and being independent were valued. Each attribute had several characteristics including: being treated like a friend; having contact information of a responsive provider; being accompanied to medical and social activities; being given clear treatment options including what to expect; having homes adapted to support limitations and having the opportunity to participate in meaningful hobbies. Conclusions: Attributes of good care extend beyond disease management. While our findings include activities that characterize these attributes, further research on implementation barriers and facilitators is required.

Toute activité humaine exige, pour sa bonne effectuation, la coordination des interdépendances entre les acteurs concernés. La coordination des interdépendances se réalise en mode ordinaire, dans les activités de tous les jours, mais aussi en mode dédié, c’est-à-dire à travers une pratique qui a pour mandat principal de les gérer de manière consciente, volontaire et imputable pour les situations d’intervention dont la complexité est grande. The best-known theories on interdisciplinarity in the health and social care field seek to identi... more The best-known theories on interdisciplinarity in the health and social care field seek to identify personal characteristics and organizational predispositions favourable to interprofessionnal collaboration. This paper proposes a reversal of this positioning through the theorization of interdisciplinarity in the health and social care field as a condition of the work of its professionals rather than one of their peculiar attributes. To achieve this reversal, we set out the epistemological foundations of the current debate on interdisciplinarity.

Today, part of this role has been taken over by government social and healthcare services, but they are having limited success in addressing social challenges. Increasing elders' social participation and intergenerational solidarity might foster community development and benefit young people, families, communities and the elders themselves. However, knowledge of the contribution of elders' social participation and intergenerational solidarity to wellness is scattered and needs to be synthesised.
