An audit of 3374 appendectomy specimens in 2578 Saudi and 796 non-Saudi nationals revealed a diag... more An audit of 3374 appendectomy specimens in 2578 Saudi and 796 non-Saudi nationals revealed a diagnosis rate of 74.7% of inflamed appendix, a normal appendix range of 7.8% to 22.5% with the higher rate of normal appendix found among females. The finding of high incidence of schistosomal appendicitis among Egyptian males is not surprising given the high incidence of schistosomiasis among Egyptians in general. The alternate diagnoses, which include such conditions as neoplasm, mucocele, other inflammatory conditions such as periappendicitis, and parasitic infestations, are not different from findings in the reported literature.

Almost one-third of the patienls (9/25) had no underlying cause discernible by our investigational facilities (chest ~-ray. ultrasonography. computed tomographic scan, and flexible bronchotibcrscopyj. Underlying pneumonia (three patients). pulmonary tuberculosis (two patienls), lung absccss (one patient). and congenital bullae (one patient) constituted the etiology in another third (tf the spontaneous pneumothorax paricnts. Other underlying pulm(mary diseases precipirating spontaneous pneumothorax in the group included pulmonary fibrosis, metaslatic mesothelioma, and immunosuppression in a medull(tb(a~toma patient undergoing chemotllcrJPY with the development of chiekenpo~. In a study of 27 cases of perforated duodenal ulcer seen at Asir Central Hospital over a period o... more In a study of 27 cases of perforated duodenal ulcer seen at Asir Central Hospital over a period of seven years, two patients were females and 25 males. The highest incidence was in the fourth decade and the average age was 36.3 years. The Saudi-foreigner ratio was 1:2.9 in an area where the Saudi-foreigner population ratio was 1:3. Nine patients (33.3%) were previously diagnosed as having peptic ulcer and had received treatment at one time or the other before perforation. Eighteen patients (66.7%) were first diagnosed to peptic ulcer after the perforation. All the identified perforations were located anteriorly and anterosuperiorly. Only one case occurred in the second part of duodenum. The rest occurred in the first part. Two patients were treated successfully conservatively. The month perforations occurred most was the month of Shaaban. The fasting period during the month of Ramadhan did not show any increase in the prevalence of perforation in this hospital.

In a six-year period, starting from the commissioning of Asir Central Hospital in 1408 H (1988), ... more In a six-year period, starting from the commissioning of Asir Central Hospital in 1408 H (1988), 43 cases of hydatid cyst disease caused by echinococcus granulosus seen in Asir Central Hospital were studied. The pattern was not significantly different from other workers' experiences. There was equal sex distribution and the average age was 41.7 years. The clinical presentations depended essentially on the organs affected, and computed tomography usually confirmed the diagnosis. The serological test at times gave a false negative result. Most of the patients came from Abha in Asir region and the most commonly involved organ was the right lobe of the liver. All the patients had laparotomy, excision or incision and drainage of the cyst, depending on whether the cyst could be safely excised or only drained. The average hospital stay was 17 days. We believe that surgical intervention should be the first line of treatment especially when the cyst is large.

Acute gallbladder perforation, an infrequent though not uncommon complication of Cholelithiasis a... more Acute gallbladder perforation, an infrequent though not uncommon complication of Cholelithiasis and acalculus cholecystitis is reviewed using as materials 16 Arab patients (6 males and 10 females; one Egyptian and 15 Saudis) with gallbladder perforation among 952 patients who had undergone cholecystectomy in Asir Central Hospital between 1990 and 1994. The presentation, method of diagnosis and treatment modalities are compared with recent literature review findings. Since cholecystitis (calculus and acalculus) is a common pathology in Saudi Arabia a preoperative awareness of this complication of the disease will improve diagnosis and minimize morbidity and mortality of gallbladder perforation.

The first had a gun shot wound of the anterior left chest with the bullet ricocheting across to the right hemithorax and puncturing the right mainstem bronchus resulting in right massive pneumothorax and subcutaneous emphysema. The second suffered a blunt thoracic trauma from a road traffic accident and presented with bilateral rib fractures and bilateral pneumothoraces treated with bilateral closed thoracostomy tube drainage. Bronchofiberscopy revealed complete transection of the left mainstem bronchus.

Adhesions from previous laparotomy scar constituted the commonest cause of intestinal obstruction (57.1%). A distant second is faecal impaction (7.1%). Previously performed appendicectomy is the commonest cause of adhesions causing intestinal obstruction. The interval between surgery and intestinal obstruction varies from one month to three years. The pattern of intestinal obstruction seen in this environment is more similar to those in the western World or advanced countries, than the pattern in the developing countries. This can be explained on the basis of the fact that, even though Saudi Arabia is a developing country, the health care delivery system is similar to those in developed countries. A 37-year-old Filipino woman presented with a post road-traffic accident fracture of dorsal spine... more A 37-year-old Filipino woman presented with a post road-traffic accident fracture of dorsal spine 12. Chest radiograph revealed evidence of loops of small bowel in the left lung field. She admitted to symptoms of respiratory insufficiency since birth and treatment for tuberculosis in childhood. A pre-operative diagnosis of left traumatic diaphragmatic hernia was not confirmed at laparotomy which revealed typical left congenital Bochdalek hernia with smooth edges and herniation of small bowel and spleen into the left pleural cavity. Following reduction and repair of the hernia, the patient made an uneventful recovery. Chest radiograph remains normal till now, eight years post-operatively.

Empyema thoracis in adults is an uncommon disease in the Asir region of Saudi Arabia. In a period... more Empyema thoracis in adults is an uncommon disease in the Asir region of Saudi Arabia. In a period of seven years (1988 to 1994), 24 patients were treated for empyema thoracis with a hospital incidence of about 23 patients in 100,000 admissions. The community acquired empyemas are more common and less aggressive in non-Saudi patients (six males and one female) as compared to Saudi patients (11 males and 6 females) whose empyemas are mostly nosocomial with an aggressive course. The peak age in both Saudi and non-Saudi patients is 45 years and 25 years respectively, and the right pleura is more commonly affected than the left pleura in both groups. Risk factors include diabetes mellitus, pulmonary tuberculosis, post-pneumonectomy infections, trauma and pneumonia. The commonest organisms grown are Pseudomonas aeruginosa, Klebsiella species and Staphylococcus aureus, although in almost 40% of the patients the empyemas were sterile. We hope that the report will initiate further studies on the natural history of breast lesions including breast carcinoma which is the second leading cause of cancer among female patients from the Asir region. Results: The Department of Histopathology of Asir Central Hospital in Abha City, Asir province, received and processed all breast specimens. Almost 39%, 27%, 19%, and 13% of the cases were inflammatory, benign neoplasms, malignant neoplasms and fibrocystic changes of the breast respectively. One hundred and fifty-two consecutive cases of cholelithiasis were studied. The gallbladder stone... more One hundred and fifty-two consecutive cases of cholelithiasis were studied. The gallbladder stones were analyzed to determine the constituents, the bile specimens were cultured and correlated with the cultures when wound infection occurred. Mixed gallstones were the commonest type (58.5%), pigment stones (27%) and cholesterol stones (14.5%). Positive bile cultures were found in 41 patients (27%) and Escherichia coli was the commonest organism isolated. The rate of wound infection was 14.5% and the most common organism cultured from the wound was staphylococcus epidermidis.
Hydatid disease (Echinococcus granuloma) is endemic in the middle east and other parts of the wor... more Hydatid disease (Echinococcus granuloma) is endemic in the middle east and other parts of the world, including India, Africa, South America, New Zeland, Australia, Turkey and South Europe. Infestation by hydatid disease in humans most commonly occurs in the liver (55-70%) followed by the lung (18-35%). Even though hydatid cysts can occur in any organ, it is rare to see the disease involving so many organs at the same time as was in this case. This paper reports a case of unusual presentation of hydatid cyst probably following improper excision of the initial liver hydatid cyst, or a recurrence or both.

We hope that the report will initiate further studies on the natural history of breast lesions including breast carcinoma which is the second leading cause of cancer among female patients from the Asir region. Results: The Department of Histopathology of Asir Central Hospital in Abha City, Asir province, received and processed all breast specimens. Almost 39%, 27%, 19%, and 13% of the cases were inflammatory, benign neoplasms, malignant neoplasms and fibrocystic changes of the breast respectively. The overall Saudi to non-Saudi ratio was 2.6:1. while the partial ones were 4:1, 3.4:1, 1.2:1 and 1:1 for the inflammatory, benign neoplasm, malignant neoplasm and fibrocystic lesions respectively. In a six-year period, starting from the commissioning of Asir Central Hospital in 1408 H (1988), ... more In a six-year period, starting from the commissioning of Asir Central Hospital in 1408 H (1988), 43 cases of hydatid cyst disease caused by echinococcus granulosus seen in Asir Central Hospital were studied. The pattern was not significantly different from other workers' experiences. There was equal sex distribution and the average age was 41.7 years. The clinical presentations depended essentially on the organs affected, and computed tomography usually confirmed the diagnosis. The serological test at times gave a false negative result. Most of the patients came from Abha in Asir region and the most commonly involved organ was the right lobe of the liver. All the patients had laparotomy, excision or incision and drainage of the cyst, depending on whether the cyst could be safely excised or only drained. The average hospital stay was 17 days. We believe that surgical intervention should be the first line of treatment especially when the cyst is large.

Adhesions from previous laparotomy scar constituted the commonest cause of intestinal obstruction (57.1%). A distant second is faecal impaction (7.1%). Previously performed appendicectomy is the commonest cause of adhesions causing intestinal obstruction. The interval between surgery and intestinal obstruction varies from one month to three years. The pattern of intestinal obstruction seen in this environment is more similar to those in the western World or advanced countries, than the pattern in the developing countries. This can be explained on the basis of the fact that, even though Saudi Arabia is a developing country, the health care delivery system is similar to those in developed countries. Acute gallbladder perforation, an infrequent though not uncommon complication of Cholelithiasis a... more Acute gallbladder perforation, an infrequent though not uncommon complication of Cholelithiasis and acalculus cholecystitis is reviewed using as materials 16 Arab patients (6 males and 10 females; one Egyptian and 15 Saudis) with gallbladder perforation among 952 patients who had undergone cholecystectomy in Asir Central Hospital between 1990 and 1994. The presentation, method of diagnosis and treatment modalities are compared with recent literature review findings. Since cholecystitis (calculus and acalculus) is a common pathology in Saudi Arabia a preoperative awareness of this complication of the disease will improve diagnosis and minimize morbidity and mortality of gallbladder perforation.

The first had a gun shot wound of the anterior left chest with the bullet ricocheting across to the right hemithorax and puncturing the right mainstem bronchus resulting in right massive pneumothorax and subcutaneous emphysema. The second suffered a blunt thoracic trauma from a road traffic accident and presented with bilateral rib fractures and bilateral pneumothoraces treated with bilateral closed thoracostomy tube drainage. Bronchofiberscopy revealed complete transection of the left mainstem bronchus.

One hundred and fifty-two consecutive cases of cholelithiasis were studied. The gallbladder stone... more One hundred and fifty-two consecutive cases of cholelithiasis were studied. The gallbladder stones were analyzed to determine the constituents, the bile specimens were cultured and correlated with the cultures when wound infection occurred. Mixed gallstones were the commonest type (58.5%), pigment stones (27%) and cholesterol stones (14.5%). Positive bile cultures were found in 41 patients (27%) and Escherichia coli was the commonest organism isolated. The rate of wound infection was 14.5% and the most common organism cultured from the wound was staphylococcus epidermidis.
Hydatid disease (Echinococcus granuloma) is endemic in the middle east and other parts of the wor... more Hydatid disease (Echinococcus granuloma) is endemic in the middle east and other parts of the world, including India, Africa, South America, New Zeland, Australia, Turkey and South Europe. Infestation by hydatid disease in humans most commonly occurs in the liver (55-70%) followed by the lung (18-35%). Even though hydatid cysts can occur in any organ, it is rare to see the disease involving so many organs at the same time as was in this case. This paper reports a case of unusual presentation of hydatid cyst probably following improper excision of the initial liver hydatid cyst, or a recurrence or both.

In a study of 27 cases of perforated duodenal ulcer seen at Asir Central Hospital over a period o... more In a study of 27 cases of perforated duodenal ulcer seen at Asir Central Hospital over a period of seven years, two patients were females and 25 males. The highest incidence was in the fourth decade and the average age was 36.3 years. The Saudi-foreigner ratio was 1:2.9 in an area where the Saudi-foreigner population ratio was 1:3. Nine patients (33.3%) were previously diagnosed as having peptic ulcer and had received treatment at one time or the other before perforation. Eighteen patients (66.7%) were first diagnosed to peptic ulcer after the perforation. All the identified perforations were located anteriorly and anterosuperiorly. Only one case occurred in the second part of duodenum. The rest occurred in the first part. Two patients were treated successfully conservatively. The month perforations occurred most was the month of Shaaban. The fasting period during the month of Ramadhan did not show any increase in the prevalence of perforation in this hospital.

Chest radiograph revealed evidence of loops of small bowel in the left lung field. She admitted to symptoms of respiratory insufficiency since birth and treatment for tuberculosis in childhood. A pre-operative diagnosis of left traumatic diaphragmatic hernia was not confirmed at laparotomy which revealed typical left congenital Bochdalek hernia with smooth edges and herniation of small bowel and spleen into the left pleural cavity. Following reduction and repair of the hernia, the patient made an uneventful recovery. Chest radiograph remains normal till now, eight years post-operatively.

GISTs are usually found in the stomach or small intestine but can occur anywhere along the GI tract and rarely have extraGI involvement. Five cases of (GIST) are recorded between Jan 2017 and December 2019 in the surgical department in Al Noor Specialist Hospital. The records of the five cases of GI stromal tumor were reviewed. Three males and two females,
with mean age of 55.8 ± 7years. The stomach was involved in all patients. The most common symptom at presentation was abdominal pain, followed by gastrointestinal bleeding. The size of the tumors varies between 3.5X4 cm to 21.7X10.8cm. Four cases were treated with the complete surgical resection and the large tumor was treated medically.
