To examine the potential therapeutic effects of ursodeoxycholic acid (UDCA) on diseases of the es... more To examine the potential therapeutic effects of ursodeoxycholic acid (UDCA) on diseases of the esophagus, stomach, and duodenum. Methods: A search was conducted using EBSCO, Medline, PubMed, Google Scholar and Web of Science as well as international guidelines using MESH terms for treatment of UDCA for diseases of the upper gastrointestinal disorders in adult humans without regard to publication language or date restrictions. Results: A total of 256 articles and 22 guidelines were initially identified, and 221 were excluded. Final Systematic Review revision of 13 articles and 22 guidelines confirmed that UDCA is found to have a cytoprotective role in Barret's esophagus within esophageal disorders, improves abdominal pain in functional dyspepsia, and does not alter Helicobacter pylori colonization or inflammation. Conflicting results are noted regarding the role of UDCA in the duodenum as chemopreventive treatment for familial adenomatous polyposis, with polyps regressing and their growth characteristics improving with low doses (10-25 mg/ kg/day). On the contrary, no positive effect was noted upon the combination with Celecoxib and with doses of 1000-2000 mg or 20-30 mg/kg/d. Gastrointestinal side effects were predominantly reported. No side effects necessitated hospitalization or ICU admission. Conclusion: Ursodeoxycholic acid has a limited therapeutic role in functional dyspepsia. There is promising evidence that it may serve as a chemopreventive for Familial adenomatous polyposis and Barret's esophagus, although further research is needed to confirm these findings.

Background: Irritable bowel syndrome (IBS) is a common clinical problem in several age groups in ... more Background: Irritable bowel syndrome (IBS) is a common clinical problem in several age groups in Saudi Arabia ,reached about 9 % in adult students and up to 40 % in teachers .Effort is needed to correctly manage IBS and identify any coexistent condition that mimic the symptoms especially gluten sensitivity (GS). It is considered of low prevalence in Asian and Middle Eastern countries compared to high prevalent regions in Europe and North America. Aims: This prospective study aims to estimate the prevalence of different subtypes of IBS, celiac disease positive serology and possible association that may exist, in an area of high IBS diagnosis and relatively low prevalence of celiac disease. Methods: A Prospective study at a tertiary care private hospital at the western region of Saudi Arabia over the period of August 2013 to August 2016. Adults above the age of 15 year invited to participate in the study.ROME 3 criteria was used to identify and diagnose different subtypes of IBS as IBS-Pain ,IBS-constipation ,IBS-Diarrhea and IBS-Mixed. Basic demographic data collected. The patients filled questionnaire that outline questions related to these subtypes. Laboratory work included CBC, Electrolytes, urea, creatinine, TSH, Tissue transglutaminase antibody (TTG) Ig A and Ig G collected. Patients with Positive serology for TTG invited to undergo upper endoscopy and duodenal biopsies to verify features of Gluten sensitivity according to Modified Marsh classification.A Control group done as a healthy blood donors who are symptom free of irritable bowel syndrome. Serum TTG IgA and IgA levels measured. Results: A 305 patients with IBS of mixed subtypes according to ROME 3 criteria recruited. Predominantly IBS with mixed features 293 patients (96 %), Demographic data to be presented in (table 1). There is 17 patients (5.6%) with positive TTG serology. Duodenal biopsies obtained for only 4 patients (2 patients of Marsh 0 and 2 patients of Marsh 3b classification respectively).There is statistically significant correlation between IBS with diarrhea predominant subtype,Age of the patient population and positive TTG-IgA. There is no significant association between the level of TTG IgA of the GS patients in the study and their modified marsh histological stage.Control group is 204 individuals , Males are 122 (24%).There is no statistically significant difference between the age and gender of the cases and control groups.Three Positive cases detected among the control group (Blood donors) 1.47 %.No IgA deficiencies detected. Conclusions: Gluten sensitivity is prevalent in IBS patients especially diarrhea predominant subtype in the area of the study as compared to the healthy control. Performing serological tests and duodenal biopsies for positive cases will aid in detecting a large group of undiagnosed celiac patients and hence would improve the overall management of irritable bowel syndrome.

Aim: To identify the possible associations between liver chemistry and clinical presentation. Methods: This was a retrospective chart review of patients admitted to Al-Noor Specialist Hospital, a tertiary facility with a large referral area. We identified patients with the ICD-10 codes for dengue fever and dengue hemorrhagic fever. Data collected included liver chemistry and other blood work for secondary liver diseases. Statistical analysis involved descriptive data and correlation data analyzed using Mann-Whitney and KruskalWallis tests. Results: A total of 535 patients met the selection criteria. Other studies included upper endoscopy with histological study for H. Pylori; abdominal ultrasound; complete biochemical panel; and hepatitis panel. Results: It was found that patients with chronic Helicobacter gastroduodenitis had relatively frequent changes in clinical and laboratory data, specifi cally increased liver echogenicity, wall thickness of extrahepatic biliary ducts and changes in hepatic transaminases. Most of the patients had a minimal to moderate elevation of transaminases. Majority of the patients (82.5 percent) had elevated levels of anti-Helicobacter IgG and 24.4 percent of the patients had elevated levels of anti-Helicobacter IgM. Total 44 percent of the patients had high anti-Helicobacter IgG levels with concomitant alanine aminotrasferase elevation. However, only a small number of patients with low anti-Helicobacter IgG levels (2.5 percent) had elevation of transminases. Non-specifi c ultrasound fi ndings included thickening of the gall bladder wall in 89.8 percent of the patients and common bile duct in 33.7 percent. About half of the patients with chronic Helicobacter gastroduodenitis had increased liver echogenicity and elevated specifi c IgG levels at the same time. Conclusion: Patients with chronic Helicobacter infection that was confi rmed by histological and immunological studies were found to have elevation of specifi c anti-Helicobacter IgG levels that were associated with ALT elevation and increased liver echogenicity as well as sonographic evidence of thickening of the gall bladder wall and extrahepatic bile ducts, indicating possible involvement of hepatobiliary system into the pathological process. Th ere is no clear explanation of these fi ndings, thus, subsequent larger studies are warranted in the future to explore the signifi cance of anti-Helicobacter spp. antibody levels or a role of particular enterohepatic Helicobacter sp. in the pathogenesis of chronic changes in the hepatobiliary system.

Results: Six hundred and nineteen questionnaires were distributed. Completed questionnaires that were included in the final analysis numbered 321 (51.9%). Background/Aim: To identify the prevalence of gluten sensitivity in a healthy adult Saudi populat... more Background/Aim: To identify the prevalence of gluten sensitivity in a healthy adult Saudi population within a low endemic area of celiac disease using IgA tissue transglutaminase antibody. The study was conducted as a prospective pilot study for Saudi attendees of a blood donation centre at King Faisal Specialist Hospital & Research Centre in Jeddah, Saudi Arabia. Patients and Methods: Individuals were invited to participate in the study and screened for gluten sensitivity using immunoglobulin A tissue transglutaminase antibody (IgA TTG) along with serum IgA level. Descriptive data was presented and expressed as mean value; correlation between variables was estimated using Pearson correlation, and nonparametric data using Pearson rho correlation (level of P value <0.05 is considered to be statistically significant). Results: Two hundred and four individuals (122 males and 82 females, mean age 35 years) attending the blood donation centre were screened. Three individuals tested positive for IgA TTG showing normal IgA level (1 female and 2 males) with a 1.5% prevalence in the cohort. Conclusions: Positive celiac screening is present at a low prevalence rate in our adult population, in which the individuals' age and their serum IgA levels are not associated with the positivity level. A study on a larger scale with the application of histologic confirmation of positive cases is needed.

Retrospective chart review of patients diagnosed with chronic liver disease and portal vein thrombosis in two major cities in Western Saudi Arabia during the period 2000-2009. Among 1349 patients screened, 109 patients met the inclusion criteria, 67 patients had acute PVT and 42 patients had chronic PVT. The relative risk of developing PVT is higher in males in whom acute PVT is 1.32 vs. 0.68 in females (95% CI: 090-1.94). Mean survival age of acute and chronic PVT (Kaplan-Meier) is 5.61 years (95% CI: 4.52-6.70). Univariate and multivariate regression analysis identified several variables in which international normalized ratio (INR) level on presentation was found to be a significant variable in survival. Mean survival for acute or chronic PVT is about five years. The first determination of INR level is a useful predictor of survival.

Recently, NAFLD has been described in lean individuals who additionally have impaired metabolic parameters similar to their non‐lean counterparts. We aimed to explore this further in Saudi Arabia. From 2016 to 2019, we prospectively studied a group of newly diagnosed NAFLD patients at a tertiary hospital in Saudi Arabia. Patients were classified into three groups: lean (body mass index [BMI] <25), overweight (BMI ≥25 and <30), and obese (BMI ≥30). We made comparisons between these groups on basic clinical, demographic, and laboratory parameters. In total, 1753 patients were recruited and 1262 patients met the inclusion criteria. There is no significant association between the level of TTG IgA of the GS patients in the study and their modified marsh histological stage.Control group is 204 individuals , Males are 122 (24%).There is no statistically significant difference between the age and gender of the cases and control groups.Three Positive cases detected among the control group (Blood donors) 1.47 %.No IgA deficiencies detected. Conclusions: Gluten sensitivity is prevalent in IBS patients especially diarrhea predominant subtype in the area of the study as compared to the healthy control. Performing serological tests and duodenal biopsies for positive cases will aid in detecting a large group of undiagnosed celiac patients and hence would improve the overall management of irritable bowel syndrome.

Other studies included upper endoscopy with histological study for H. Pylori; abdominal ultrasound; complete biochemical panel; and hepatitis panel. Results: It was found that patients with chronic Helicobacter gastroduodenitis had relatively frequent changes in clinical and laboratory data, specifi cally increased liver echogenicity, wall thickness of extrahepatic biliary ducts and changes in hepatic transaminases. Most of the patients had a minimal to moderate elevation of transaminases. Majority of the patients (82.5 percent) had elevated levels of anti-Helicobacter IgG and 24.4 percent of the patients had elevated levels of anti-Helicobacter IgM. Total 44 percent of the patients had high anti-Helicobacter IgG levels with concomitant alanine aminotrasferase elevation. However, only a small number of patients with low anti-Helicobacter IgG levels (2.5 percent) had elevation of transminases. Non-specifi c ultrasound fi ndings included thickening of the gall bladder wall in 89.8 percent of the patients and common bile duct in 33.7 percent. About half of the patients with chronic Helicobacter gastroduodenitis had increased liver echogenicity and elevated specifi c IgG levels at the same time. Conclusion: Patients with chronic Helicobacter infection that was confi rmed by histological and immunological studies were found to have elevation of specifi c anti-Helicobacter IgG levels that were associated with ALT elevation and increased liver echogenicity as well as sonographic evidence of thickening of the gall bladder wall and extrahepatic bile ducts, indicating possible involvement of hepatobiliary system into the pathological process. Th ere is no clear explanation of these fi ndings, thus, subsequent larger studies are warranted in the future to explore the signifi cance of anti-Helicobacter spp. antibody levels or a role of particular enterohepatic Helicobacter sp. in the pathogenesis of chronic changes in the hepatobiliary system.

The study was conducted as a prospective pilot study for Saudi attendees of a blood donation centre at King Faisal Specialist Hospital & Research Centre in Jeddah, Saudi Arabia. Patients and Methods: Individuals were invited to participate in the study and screened for gluten sensitivity using immunoglobulin A tissue transglutaminase antibody (IgA TTG) along with serum IgA level. Descriptive data was presented and expressed as mean value; correlation between variables was estimated using Pearson correlation, and nonparametric data using Pearson rho correlation (level of P value <0.05 is considered to be statistically significant). Results: Two hundred and four individuals (122 males and 82 females, mean age 35 years) attending the blood donation centre were screened. Three individuals tested positive for IgA TTG showing normal IgA level (1 female and 2 males) with a 1.5% prevalence in the cohort. Conclusions: Positive celiac screening is present at a low prevalence rate in our adult population, in which the individuals' age and their serum IgA levels are not associated with the positivity level. A study on a larger scale with the application of histologic confirmation of positive cases is needed.

Retrospective chart review of patients diagnosed with chronic liver disease and portal vein thrombosis in two major cities in Western Saudi Arabia during the period 2000-2009. Among 1349 patients screened, 109 patients met the inclusion criteria, 67 patients had acute PVT and 42 patients had chronic PVT. The relative risk of developing PVT is higher in males in whom acute PVT is 1.32 vs. 0.68 in females (95% CI: 090-1.94). Mean survival age of acute and chronic PVT (Kaplan-Meier) is 5.61 years (95% CI: 4.52-6.70). Univariate and multivariate regression analysis identified several variables in which international normalized ratio (INR) level on presentation was found to be a significant variable in survival. Mean survival for acute or chronic PVT is about five years. The first determination of INR level is a useful predictor of survival.

From 2016 to 2019, we prospectively studied a group of newly diagnosed NAFLD patients at a tertiary hospital in Saudi Arabia. Patients were classified into three groups: lean (body mass index [BMI] <25), overweight (BMI ≥25 and <30), and obese (BMI ≥30).
