We studied the association between serum T and E2 levels and characteristics of men with E2 levels in the highest quartile. RESULTS: The mean serum total T was 357.8 Ϯ 166.8 ng/dl (free T 0.82 Ϯ 0.37 ng/dl) and mean E2 was 49.9 Ϯ 16.8 pmol/L. Men in the highest quartile for E2 had levels above 59.6 pmol/L. Age and the rate of diabetes, hypertension, and heart disease were not different between E2 quartiles. Mean total T was 282.6 Ϯ 144.6 for the lowest E2 quartile and 372 Ϯ 149.8 ng/dl for the highest quartile (pϭ0.066). Higher serum total T (pϭ0.0027) and higher BMI (pϭ0.0075) were significantly and independently associated with higher serum E2. There is a non-significant trend towards lower LH levels with higher E2 (pϭ0.0779). However, this association is no longer present when adjusted for T and BMI (pϭ0.373). 7 men (8.5%) had T levels less than 300 ng/dl (mean 225.8ng/dl) and E2 levels in the highest quartile. LH levels for these men did not differ significantly from the remainder of the men in the study (pϭ0.653). CONCLUSIONS: For men with symptoms of T deficiency, elevated E2 levels are associated with higher rather than lower T levels. Elevated E2 in the setting of low T is uncommon and there is a lack of evidence for suppression of T production by endogenous E2.

A cross-sectional study was conducted on 60 women, 40 with vulvovaginal candidiasis and 20 controls. The defense cells were identified using an impedance system combined with flow cytometry and total and specific IgE was measured by chemiluminescence. The Mann-Whitney test was used for nominal variables and the Spearman test was used to determine the correlation of IgE concentration and eosinophils in peripheral blood. The number of eosinophils in peripheral blood from patients with recurrent vulvovaginal candidiasis, 302.60 (± 253.07), was significantly higher compared to control, 175.75 (± 109.24) (p=0.037). Serum levels of total and specific IgE were similar in the groups of women with and without recurrent vulvovaginal candidiasis (p=0.361). Objective: To assess different aspects of sexual function in men with spinal cord injury (SCI) us... more Objective: To assess different aspects of sexual function in men with spinal cord injury (SCI) using the Male Sexual Quotient (MSQ), a newly developed tool to assess sexual function and satisfaction. Design: Cross-sectional study. Setting: Tertiary rehabilitation center. Participants: Patients (NZ295) older than 18 years (mean age AE SD, 40.7AE14.5y) with SCI for more than 1 year (median time since SCI, 3.6y; range, 1.6e7.0y) were assessed from February to August 2012. Patients completed the MSQ questionnaire and the Sexual Health Inventory for Men (SHIM). Interventions: Not applicable. Main Outcome Measures: Performance in various domains of sexual function was evaluated using the MSQ and SHIM questionnaires. Results: Erectile function, ejaculation, and orgasm were the most severely affected domains. The median MSQ score was 40 (range, 8e66), and the median SHIM score was 5 (range, 0e16). The diagnostic properties of the 2 instruments were similar in the discrimination of sexually active subjects. The area under the receiver operating characteristic curve was .950 (95% confidence interval [CI], .923e.979) for the MSQ and .942 (95% CI, .915e.968) for the SHIM. There was a strong correlation between the 2 instruments (rZ.826; 95% CI, .802e.878). Conclusions: Different domains of sexual function are severely impaired in men with SCI, although their sexual interest remains high. The MSQ and SHIM scores strongly correlate, but the MSQ provides a more comprehensive assessment of sexual dysfunction in male patients with SCI.

These changes and accompanying risk factors must be taken into consideration when planning measures to prevent SCI.

Lower urinary tract symptoms are numerous, but the specific impact of each of these symptoms on t... more Lower urinary tract symptoms are numerous, but the specific impact of each of these symptoms on the quality of life (QoL) has not been evaluated in community-dwelling men. An assessment of these symptoms and their effects on QoL was the focus of this study. We performed a cross-sectional study with 373 men aged >50 years from a community setting. Patients completed the International Prostate Symptom Score questionnaire, which includes questions on each of the specific urinary symptoms and a question addressing health-related QoL that are graded from 0 to 5. We used the Pearson correlation test to assess the impact of each symptom on QoL. Nocturia (58.9%) was the most prevalent urinary symptom. The mean score was 0.9±1.4 for incomplete emptying, 1.0±1.5 for frequency, 0.9±1.3 for intermittency, 0.8±1.3 for urgency, 1.0±1.5 for weak stream, 0.5±1.0 for straining, and 2.0±1.6 for nocturia.
