In mental health care, psychologists assess clients' complaints, analyze underlying problems, and... more In mental health care, psychologists assess clients' complaints, analyze underlying problems, and identify causes for these problems, to make treatment decisions. We present a study on psychologists' diagnostic processes, in which a mixed-method approach was employed. We aimed to identify a common structure in the diagnostic processes of different psychologists. We engaged an actor to simulate a client. Participants were asked to perform a diagnostic interview with this "client". This interview was videotaped. Afterwards participants first wrote a report and then were asked to review their considerations during the interview. We found that psychologists were comprehensive in their diagnostic interviews. They addressed the client's complaints, possible classifications, explanations, and treatments. They agreed about the classifications, more than about causal factors and treatment options. The content of the considerations differed between the interviews and the reports written afterwards. We conclude that psychologists continuously shifted between diagnostic activities and revised their decisions in line with the dynamics of the interview situation.

For non-responders, dose escalation is often applied. Aim To systematically review the evidence for dose escalation of SSRIs. Method A systematic literature search in MEDLINE, EMBASE, CINAHL and PsycInfo was performed. Randomised controlled trials and meta-analyses investigating dose escalation of SSRIs were identified. Relevant articles were retrieved and critically appraised. Results were summarised in an evidence table. Pooling was not justified because of heterogeneity of the identified studies. Results Eight true dose-escalation studies and three meta-analyses were identified. The available data provided no unequivocal base for dose escalation. An evaluation of the most commonly used ROM measures in Dutch psychiatry is lacking, both for sev... more An evaluation of the most commonly used ROM measures in Dutch psychiatry is lacking, both for severe mental illnesses and for common psychiatric disorders. To provide an overview of the characteristics and quality of outcome measures. A literature study yielded six outcome measures. The psychometrical, clinical and practical aspects of these scales are described. The measures are suitable and are of adequate quality. It remains to be seen if any of the outcome measures are suitable for both serious and less serious mental illnesses. The use of a combination of a self-rating scale and an observerrating scale that measure symptoms and domains of functioning may be the most promising choice.

Instruments should be easy to use and take very little time to complete; they also need to have psychometric qualities, be sensitive to change, and provide information about patients' symptoms, and about interpersonal and social functioning. To investigate to what extent the combination of Health of the Nation Outcome Scales (HoNOS) and the Outcome Questionnaire (OQ) in the Dutch situation meets the above-mentioned quality criteria and to examine how the combination relates to the Symptom CheckList (SCL-90). Data for 148 patients collected at three measurement moments were available for analysis. The psychometric qualities of the instruments and their sensitivity to change were checked carefully. The three scales showed high values for internal consistency (Cronbach's alpha). The HoNOS total score and the subscales of the OQ correlated reasonably well with the SCL-90 tot...

From 1999 until 2009 the National Steering Group for Multidisciplinary Guideline Development in M... more From 1999 until 2009 the National Steering Group for Multidisciplinary Guideline Development in Mental Health led a national guideline programme in the Netherlands. Ten service user and carer groups and 30 professional organizations were involved. Within the programme 13 practice guidelines and a range of other deliverables, such as patient versions, checklists and implementation materials, were produced. Lessons learned included the methodology of developing guidelines, service user and carer involvement, and the dissemination and implementation of the guidelines. There are important future challenges such as following a strict and transparent methodology, adhering to process management, improving integration of the service user and professional perspectives, and bridging the gap between guideline and practice.
