Purpose. As the vitamin D status of Algerian postmenopausal women was poorly described, this cross-sectional study investigated the prevalence of low vitamin D status in a sample population. Secondarily, predictive factors of this hypovitaminosis D were explored. Methods. All the 336 selected women ≥ 45 years from Douera were interviewed to get anthropometric and lifestyle data, reproductive and medical history, medications, and calcium/vitamin D intakes. A blood sample was collected to measure 25-hydroxyvitamin D (25(OH)D) concentrations. Results. Approximately 86% of subjects had low vitamin D status (<20 ng/mL). Mean 25(OH)D level was 14.4 ± 5.3 ng/mL with a clear seasonal dynamic and a signi cant negative correlation with PTH levels (r � −0.15, p � 0.006). A multiple regression analysis using the 25(OH)D cuto value of 17 ng/mL instead of the generally admitted level of 20 ng/mL was performed to increase statistical power. Other seasons than summer (OR 4.159 and 95% CI 2.456-7.043), obesity (≥30 kg/m 2 , OR 1.826, 95% CI 1.081-3.083), and veiling (OR 3.526, 95% CI 1.090-11.400) were signi cantly associated with 25(OH)D concentrations <17 ng/mL. Conclusions. In North Algeria, the abundant sunlight appears insu cient to fully o set hypovitaminosis D risk factors in postmenopausal women, especially obesity and veiling. It suggests the major need to increase vitamin D supplementation in this subpopulation.

Background: Ankylosing spondylitis (AS) is a chronic inflammatory disease that affects the axial skeleton and can lead to complete ankylosis of the involved joint. Unfortunately, involvement of the facet joints (FJ) is not evaluated by the available scoring methods that are supposed to quantify the structural damage of the spine. Aim of the work: The aim of the present study we assessed is the involvement of FJ in Tunisian AS patients with low radiological score. Patients and methods: Results of 8 AS patients (7 men; 1 woman) with FJ involvement and low radiological scores were retrospectively studied. Their median age was 36 years, age at onset 26 and disease duration of 10 years. The Bath AS Radiology Index (BASRI) and the Stoke AS Spinal Score (SASSS) were calculated in all patients from the radiographs of lateral cervical spine, anteroposterior and lateral lumbar spine. Results: All patients had FJ ankylosis without involvement of the anterior part of the spine. Six patients (75%) had inflammatory back pain and 2 had bilateral hip pain with radiological involvement. Cervical spine limitation was noted in 4 patients. Limited lumbar spine mobility and bilateral sacroiliitis were present in all cases. The HLA-B27 typing was positive in 6/7 cases. Five patients had restrictive lung disease and 3 had osteoporosis. Conclusion: Involvement of FJ in AS may be the only sign of spine damage and may be responsible for functional impairment, however, it is not evaluated by the available radiographic scores which is an important limitation to their use.

Too often, clinicians and researchers assume that patient-reported outcome measures (PROMs) are simply designed, can be changed to suit specific circumstances and should be freely available for use without seeking permission. If a version is not easily available, they believe they can produce a new translation without relevant experience or expertise. Copyright issues are inconvenient and commonly ignored, despite some journals requesting confirmation from authors that permission had been granted for use of copyrighted materials. When informed that authors of an article did not have such permission, the response from an editor was that nothing could be done as he had to rely on the word of the authors. In fact, high quality PROMs are carefully designed, developed for new cultures by means of proven standardised methodologies, carefully re-validated in the new culture and then administered by a responsible organisation to ensure that they are used appropriately in studies. If such care has not been taken, questions should be raised about the quality of the measure and the information it generates. Several problems have arisen with the Ankylosing Spondylitis Quality of Life Questionnaire (ASQoL). This is probably because it is recognised as the most appropriate measure of quality of life for people with Ankylosing Spondylitis and has been widely used in international clinical trials and studies. Fifty-seven official language adaptations of the questionnaire have been produced. Unfortunately, as a result of unauthorised adaptation and use, questions are raised about the validity of several publications that have used the ASQoL.

Crohn's disease is a chronic inflammatory bowel disease (IBD) that can affect all segments of the digestive tract. For long time, it is a pathology considered to be uncommon among African blacks. It constitutes a heterogeneous group both in its etiopathogenesis and in its clinical and progressive presentation. The pseudotumoral form remains exceptional. We report a case of this form, observed in a 44-year-old patient. The pseudotumoral form of colonic Crohn's disease was diagnosed based on morphological, histological and progression criteria. This illustrates the need for multiple and repeated biopsies to eliminate malignant tumor, tuberculosis, intestinal parasitosis and other causes of digestive granulomatosis.

Introduction: Association studies have recently identified the importance of new genetic variants for ankylosing spondylitis (AS) in several populations. Our aim was to confirm associations of variants within genes involved in the IL-23 signalling pathway with AS in two ethnically different populations: Han Chinese and Algerian. Material and methods: Two case-control studies were performed in separate cohorts: Han Chinese (430 AS patients and 580 controls) and Algerian (130 AS patients and 120 controls). We genotyped four single nucleotide polymorphisms (SNPs): rs3212227 (or +1188A/C) and rs6887695 in IL-12β, rs7857730 in JAK2, and rs2293152 in STAT3, using TaqMan SNP genotyping assays. Gene-gene interaction analyses were also tested by logistic regression and multifactor dimensionality reduction (MDR). Results: Statistical analysis revealed a difference in allele frequencies between AS patients and controls for rs321222 in the IL-12β gene in both the Han Chinese (p = 0.005) and the Algerian (p = 0.031) cohorts. Two other associations were reported with JAK2 rs7857730 in the Han Chinese (allelic p = 0.014) cohort and STAT3 rs2293152 in the Algerian (allelic p = 0.006) cohort. Moreover, logistic regression analyses showed a number of significant combinations within the two populations, and the gene-gene epistasis effects in AS were also confirmed by MDR. Conclusions: Our findings have confirmed the association between genes in IL-23 signalling pathway and the pathogenesis of AS. This association was particularly novel in both Han Chinese and Algerian populations with the 3' untranslated region (3'UTR) variant rs3212227 (or +1188A/C) of IL-12β. The gene-gene interaction models in this pathway may thus increase the risk of AS in these populations.

The concept of Traditional Ecological Knowledge (TEK) advocated in international forums on sustainable development seems to emerge as the dominant contemporary paradigm of the protection of biodiversity. Its integration in the creation and management of marine protected areas (MPAs) can constitute a mean to achieve integrated management targets. Building on the experience of the first MPA project in Tunisian sahel in the Kuriat islands (east-central coast), this article analyses the levels of acceptability of the MPA by gauging the level of anchoring of fishermen in the marine space, notably through their TEK. This local body of knowledge encompasses marine environment, biology of marine fauna/flora, and fishing techniques. This knowledge and its use constantly evolve, balancing ruptures and continuity, largely as the result of power relationships among fishermen. In turn, the hierarchy of power among fishermen is shaped by the evolution of fishing techniques and the control over marine space. Over time, a powerful elite has emerged, whose claims about access to marine resources take place outside of the institutional framework and threaten the development of the MPA. The institutional framework itself suffers from a lack of participatory approaches and predominantly authoritarian practices of state authorities. Furthermore, the changes brought about by the 2011 revolution fell short of democratic aspirations, and do not seem to have affected local socio-political dynamics.

The aim of this study is to investigate the association of HLA-A, B and HLA-DR gene expression and to assess an association of additional HLA antigens besides HLA-B27 in south Tunisian patients with spondyloarthritis (SpA). Eighty-five patients diagnosed with ankylosing spondylitis (AS, n=68) and reactive arthrithis (ReA, n=17) were selected and compared with 100 healthy controls (HC). HLA class I antigens were typed serologically using microlymphocytotoxicity technique. HLA-DRB1* alleles were studied by polymerase chain reaction amplification with sequence-specific primers. The significance of differences between patients and controls was tested by chi-square analysis. We found significantly increased frequencies of HLA-A3 (30.6%; pC = 0.04; OR=2.95), HLA-B27 (62.35%; pC=4.10 −17 , OR=53.55), and HLA-DRB1*15 (17.2%; pC=0.026; RR=2.58) alleles in SpA patients compared to HC. The most frequent and strongest association was observed for HLA-B27 in AS (pC=6.6×10 −16 , OR=52.23). When AS and ReA patients were analysed separately, HLA-DRB1*15 and HLA-A3 were increased only in AS (pC=0.01, OR=2.99 and pC= 0.03, OR=3.14, respectively). In ReA patients, HLA-DRB1*04 (p=0.033, pC=NS, OR=2.89) was found to be the most common allele. Background Human parvovirus B19 is the etiologic agent of erythema infectiosum in children. It is also associated with other clinical manifestations in different target groups. Patients with chronic hemolytic anemia are at high risk of developing acute erythroblastopenia following infection by the virus. They usually become highly viremic and pose an increased risk of virus transmission. Close monitoring of such high risk groups is required for epidemiologic surveillance and disease prevention activities. Here we report a molecular epidemiological study on B19 virus infection in Tunisian patients with chronic hemolytic anemia.
