The expansion of the role of nurse prescribing in intensive care units in the healthcare system of Iran: a qualitative content analysis

Intensive Care Unit (ICU) nurses prescribe medication for patients in many countries. However, there is still no evidence on the legitimacy of nurse prescribing roles in the healthcare system of Iran. This qualitative study with 30 experts was conducted to explore the experiences regarding the expanding role of prescribing medication by the ICU nurses. Data were collected through 31 individual semi-structured interviews and analyzed using the conventional content analysis method by MAXQDA 10. One major theme, "applicability of prescribing medication by ICU nurses", together with three sub-themes of "facilitators", "potential risks of nurse prescribing" and "the professional pathway", emerged. The use of successful global experiences, patient-oriented healthcare system policies, current culture and positive professional position of nurses, physician shortage, and high capacity of ICU nurses appeared as facilitators to perform the new role in our context. For the expansion of the new role, different professional pathways such as discussion with physicians and special groups with conflicts of interests, training qualified nurses in this area, and gradual development were proposed by the participants. The next step of the research is to prepare a set of standards for the prescription of medication by the ICU nurses in our context.

Iran's health policymakers' views on barriers and facilitators of nurse prescribing in their context: A qualitative study

In most countries, one of the main reasons for developing more advanced roles for nurses is to improve access to care in the context of limited number of doctors. It is considered that the introduction of major policy initiatives, such as nurse prescribing, requires high-level discussion and policy development to ensure successful implementation. Background Prescribing medication by nurses as an approach to rational drug prescription has been proposed in many countries. Nursing prescribing is an effective measure in the management of critically ill patients admitted to intensive care units (ICU). This study investigated the attitude of ICU nurses towards the necessity and the barriers to developing nursing prescribing. Iranian Nurses’ Experiences on Obstacles of Safe Drug Administration: A Qualitative Study

Background: Safety maintenance and prevention of damage to patients caused by errors in nursing have a special importance, and a lack of sufficient attention to the correct principles of drug administration can lead to the patient' health threats and reducing safety. The administration of drugs is complex and influenced by many factors, particularly in pediatric wards; thus, the present study aimed to explain pediatric nurses' experiences on obstacles to safe drug administration. Methods: In this study, the qualitative approach and content analysis method were used. Twenty pediatric nurses involved in medication administration were selected for participation using purposive sampling. Data was collected through semi-structured interviews. Using continuous analysis, data collection and comparison were performed at the same time. Results: From the data analysis, 4 main themes were extracted. Working pressure, lack of drug resources, insufficient colleague performance, and lack of experience and knowledge in drug administration were four main themes obtained from study. Conclusion: Care managers can identify obstacles to safe medication administration to improve patient safety. For which necessary measures must be taken to remove them and to enhance patient safe care provided by nurses.

Clinical judgment of nurses to conduct medication orders is based on patient assessment, medication knowledge, observation, and interpretation of the data collected. Nurses, physicians and patients’ knowledge and attitudes about nurse prescribing

Background One of the roles that nurses have acquired in recent years is the role of prescribing. This study aimed to investigate the knowledge and attitudes of critical care nurses, physicians and patients about nurse prescribing. Methods A descriptive cross-sectional study with the participation of 152 nurses, 53 physicians and 75 patients was carried out. Participants were selected by stratified random sampling from the critical care units of six hospitals in Tabriz, Iran. Demographics and participants’ knowledge and attitudes about nurse prescribing questionnaires were used to collect data. The collected data were analyzed using SPSS-22 software. The Necessity of Nursing Prescription from the Perspectives of Nursing Policymakers

Introduction: Presently, in many countries, certain groups of nurses are allowed to prescribe medicine. Despite the increasing attention of the world to nursing prescription and emphasis of the World Health Organization (WHO) on its implementation in regional countries, a review of literature indicated that there is no evidence of nursing prescription in Iran. Thus, this study aimed to assess the necessity of nursing prescription from the perspective of the Iranian nursing policymakers. Method: This descriptive qualitative study was performed through interviews with 14 nursing policymakers. The participants were selected through purposive sampling method. Semi-structured interviews were used to collect data. Interviews were conducted after coordination with participants and in their workplace. All interviews were recorded using a digital voice recorder. Interviews were simultaneously transcribed verbatim and analyzed using conventional content analysis through inductive approach. The coding was performed using MAXQDA software. To achieve the accuracy and reliability of the data, the Lincoln and Guba authenticity criteria were used. Results: Analysis of the recorded interviews resulted in the extraction of the 3 main categories of "the right to health and individual's requirements", "the current situation of the clinical setting", and "nursing prescription outcomes". Conclusion: The views and perspectives of nursing policymakers indicated that due individuals' right to health and need for health care services, and also the positive outcomes that may result from nursing prescription, the necessity of its implementation is felt. However, there are challenges to its implementation that require the relevant authorities' efforts to resolve them.
